Winston-Salem TRANSFLO Terminal
Dear Mr. Basinger:
On January 8, 2013, the Winston-Salem TRANSFLO Terminal, located at 5025 Overdale Road,
Winston-Salem, NC 27107, was inspected by your Department staff to determine compliance with
the National Pollutant Discharge Elimination System (NPDES) General Stormwater Permit
NCGO80000 and Certificate of Coverage NCG080813. This letter is to serve as documentation
that the following deficiencies are in the pror_ess of being corrected or addressed to prevent
further non-compliance.
Deficiencies
TRANSFLO's Stormwater Pollution Prevention Plan (SP3), documentation and monitoring
activities were reviewed during the inspection. The following deficiencies were noted:
1. The General Location Map (GLM) does not meet all the requirements specified in Part 11,
Section A, Paragraph 1(a) of the NCG0800000 General Stormwater Permit_
2. The Site Map (SM) does not meet all the requirements specified in Part ll, Section A,
Paragraph 1(c) of the NCG0800000 General Stormwater Permit
3. The SDOs have not been certified annually as required by Part 11, Section A, Paragraph
1(d)(sic] of the NCGO800000 General Stormwater Permit
4. There is no Feasibility Study in the SP3 as required by Part 11, Section A, Paragraph 2(a)
of the NCGO800000 General Stormwater Permit
5. The SP3 Annaual Reviews have not been accomplished for all years as required by Part
11, Section A, Paragraph 7 of the NCG0800000 General Stormwater Permit
6. Not all Qualitative Monitoring (QM) has been conducted as required by Part 11, Section D,
of the NCGO800000 General Stormwater Permit
TRANSFLO Response:
Deficiencies 1, 2 and 4
TRANSFLO agrees that the SP3 does not adequately address the deficiencies noted in
Items 1, 2 and 4 and has retained a consultant, ARCADIS U.S., Inc., to prepare a new
SP3 for the Winston-Salem Terminal to address these deficiencies. A site visit has been
scheduled with the consultant and terminal beginning on Tuesday, February 26tr', 2013.
Existing stormwater features will be verified and facility operations reviewed. Information
gathered during this field visit will be used to develop a new SM and GLM that accurately
depicts the current facility and operation and addresses the cited requirements of the
Permit. Additionally, facility operations will be reviewed to address Deficiency Item 4
regarding the Feasibility Study to determine what improvements, if any, can be made to
eliminate or reduce exposure of stormwater to materials and processes. This review and
any recommendations will be included in the new SP3.

Deficiency 3
TRANSFLO acknowledges that the annual certification has not been provided as
required by the permit. A certification statement has been developed by TRANSFLO and
will be included in the new SP3 (Refer to Attachment 1). To ensure that this Certification
Statement is completed annually, TRANSFLO has added this "task" to its Environmental
Compliance Program database used to track compliance and document completion of
tasks (Refer to Attachment 2),
Deficiency 5
TRANSFLO acknowledges that an annual review of the SP3 has not been performed
consistently each year as required by the Permit. TRANSFLO has developed an Annual
Comprehensive Site Evaluation/Inspection form that includes a section to address SP3
review (Refer to the attachment titled "Appendix D-2", see page 2 Section G). This form
will be included in the new SP3.
Deficiency 6
TRANSFLO acknowledges that Qualitative Monitoring has not been performed on both
outfalls consistently. TRANSFLO will review the requirements of the permit with the
Terminal Manager during its site visit on Tuesday February 261h, 2013. It is intended that
this review be an informal storm water training session for tiie Terminal ltiianager and
available staff. TRANSFLO recognizes that in addition to this training, the new SP3
should be clear and concise in its description of the requirements of the Qualitative
Monitoring and instructions for completing this requirement. The new SP3 will specify that
both outfalls are to be monitored and will refer to the Site Map for the location of each
outfall. Signs providing the Outfall Number, Permit Number and Coverage Number will be
placed near each outfall to identify the outfalls. In addition, TRANSFLO has revised the
wording of this "task" in its Environmental Compliance Program database to clearly
instruct the Terminal Manager to perform Qualitative sampling on both outfalls (Refer to
Attachment 3).
TRANSFLO anticipates that the new SP3 will be complete by mid -April and will forward a copy of
the plan to your office for review. If your schedule permits, please feel free to join us at the
terminal during our site visit and informal storm water training session.

Ron Boone, of the Winston-Salem Regional Office (WSRO) of the NC Division of Water
Quality (DWQ or the Division), conducted a compliance evaluation inspection (CEI) of Tranflo's
Winston-Salem terminal on January 8, 2013. The assistance and cooperation during the inspection of
Mr. Trent Cox, Terminal Manager; was greatly appreciated. An inspection checklist is attached for
your records and the inspection findings are summarized below.
The facility is located at 5025 Overdale Road, in Winston-Salem, Forsyth County, North
Carolina, at approximate coordinates 36.0315920N,-80.232652°W. Stormwater related to industrial
activity is discharged from stormwater discharge outfalls (SDO) 001 and 002 to South Fork Muddy
Creek, which is currently classified as a Class C water located in the Yadkin Pee -Dee River Basin.
Documentation-& Monitoring Review
Tranflo's Stormwater Pollution Prevention Plan (SP3), documentation, and monitoring activities
were reviewed during the inspection. The following deficiencies were noted:

into a UT to the South Fork of Muddy Creek and (C). The NOI is attached.
They have said they are doing vehicle maintenance so we will already monitor for
TSS, Oil and Grease and pH. They already apparently employ SMPs and have developed
a SP3.
Does the Winston-Salem Regional Office have any concerns about issuing this facility
a COC for this general permit? If.we don't receive any objections, we'll issue the
