ES-2 Cement Storage Silo CD-2 Cartridge Filter (264 square feet of filter area) INTRODUCTION: On August 17, 2017, Charles Gerstell, NCDEQ Multimedia Inspector, contacted Mr. Dwayne Richardson, Site Manager, and Ms. Tammy Miller, Accounts Manager, for an announced targeted multimedia compliance assurance visit. The prior compliance assurance visit was performed on August 22, 2016 by Mr. Blair Palmer, WSRO DAQ Environmental Specialist, and the facility was found to be operating in compliance at that time. The facility manufactures concrete blocks for mainly construction purposes. The hours of operation at this plant are Monday -Friday (7:00am — 4:30pm), and 50 weeks a year. The facility has a plant at 2390 Midway Road where all operations are conducted and a non -operational plant located at 133 Midway Road where ES-2 and its control system CD-2 are located. Facility personnel stated that the 133 Midway Road facility will remain out of operation and there are no plans to operate it in the foreseeable future. Mr. Richardson confirmed the contacts listed in the IBEAM database and no changes are needed in IBEAM. _ Since the last inspection, this facility applied for and was granted exemption from permitted status which was granted by letter on September 26, 2016. SAFETY: The required safety equipment for this facility consist of a hard hat, hearing protection, and safety shoes. Inspectors should be aware of some truck traffic. APPLICABLE REGULATIONS: The facility is subject to the following regulations: Title 15A North Carolina Administrative Code (NCAC), Subchapter 2D .0503, .0515, .0516, .0521, .0535, .0540, .1110 and .1806. This facility is not subject to the RMP requirements of the 12(r) program since they do not use or store any of the regulated pollutants in quantities greater than the thresholds listed in the rule. They are only subject to the General Duty requirements contained in the General Duty Clause. DISCUSSION: No emission sources or control devices were in operation at the time of the inspection. The process begins with aggregate and sand that is manually loaded into a small hopper, which then falls onto a conveyor and then to a compartmented storage bin outside of the main building before being fed to the weigh batcher. Generally, all material is usually loaded for the day in the first hour and a half of the work day. Cement Storage Silo ES-1 is used to auger feed a gray cement into the weigh batcher. The cement silo was not operating at the time of inspection. Upon being asked, Mr. Richardson stated that the facility did not have any operational issues with delivery of the cement feeding into the silo. From the weigh batcher cement and aggregate are mixed and water is added before the concrete mixture is sent to the block stamping machine. After being stamped, the blocks are placed onto trays and sent to one of the five kiln rooms. The kilning process generally takes place for 24 hours after which the blocks are moved to a storage area in the rear of the facility. Mr. Richardson started the kilns during the inspection. IES-2-PLT 1 is a No. 2 Fuel oil -fired boiler that is used to provide steam heat for the kilns. The boiler is subject to NESHAP Subpart JJJJJJ and this rule is discussed in more detail under the MACT/GACT section of this report. IES-3-PLT is the 2,000-gallon fuel oil storage tank that supplies the boiler. Mr. Richardson stated that the tank was double -walled and there did not appear to be any concerns with the tank at the time of inspection. The small screen crusher (part of [ES-4-PLT 1) was viewed on -site, but was not in operation. The crusher is used to recycle unusable blocks into aggregate for future block mixtures. The facility attempts to recycle some of the unused block material (after crushing), but many of the blocks are just stored in various piles throughout the premises. Two additional conveyors are also used at the facility (as part of [ES-4 PLT 1). APPLICABLE REGULATIONS: 15A NCAC 2D .0503 particulate control requirement. This regulation sets emissions limits for the No. 2 Fuel Oil -fired Boiler (IES-2-PLT I). According to this regulation, the emissions of particulate matter from the combustion of a fuel are limited to 0.60 lbs./MMBtu for boilers of maximum heat input less than or equal to 10 MMBtu/hr. The last permit review R6 calculated the total particulate matter emission factor for the boiler as .0235 lbs. PM/MMBtu. Compliance with 2D .0503 is indicated. 15A NCAC 2D .0515 particulate control requirement. The allowable PM emission rate for silo ES-1 and ES-2 are each 35.4 lbs. PM/hr. With the bagfilters having a control efficiency of 99%, calculated emissions (taken from Mr. Leo Governale's, WSRO-DAQ Permit Engineer, review dated December 9, 201 1) from ES-1 and ES-2 are each 0.025 lbs. PM/hr. Compliance with 2D .0515 is indicated. 1SA NCAC 2D .0516 sulfur dioxide control requirement. This regulation sets the maximum allowable sulfur dioxide emissions rate to be no more than 2.3 pounds per million Btu heat input. This regulation applies to the No. 2 Fuel Oil- fired Boiler (IES-2-PLT 1). The last permit review R6 found that the actual S02 emission rate for the boiler is less than the allowable emission rate. Compliance with .0516 is indicated. 15A NCAC 2D .0521 visible emissions control requirement. This regulation sets a 20% opacity limit for bagfilter CD-1. No visible emissions were observed during this compliance assurance visit. Compliance with .0521 is indicated. 15A NCAC 2D .0535 requires the facility to notify the DAQ of any source with excess emissions that last for more than four hours and that result from a malfunction, a breakdown of process, or any other abnormal conditions. No notifications have been received since the assurance visit, and Mr. Richardson stated that there had been no such incidents since the last compliance assurance visit. Compliance with .0535 is indicated. 15A NCAC 2D .0540 fugitive dust requirement. This regulation concerns the control of fugitive dust emissions. No fugitive dust was observed traveling beyond the property boundaries during the inspection. According to the DAQ [BEAM database, no complaints have been received regarding fugitive dust emissions at this facility. Compliance with .0540 is indicated. ' 0! On August 17, 2017, Charles Gerstell, Environmental Senior Specialist with the North Carolina Department of Environmental Quality — Division of Waste Management (NCDEQ — DWM) conducted a multimedia inspection at the facility located at 2390 Midway School Road in Winston-Salem. The inspection discovered several deficiencies regarding the facilities industrial stormwater permit. This facility holds General Stormwater Permit NCG070197 to discharge stormwater from industrial activity associated with Stone, Clay, Glass, and Concrete Products [SIC 321 under the National Pollutant Discharge Elimination System (NPDES). The permit became effective June 1, 2013 and expires on May 31, 2018. A copy of the current permit is required to be maintained• with the Stormwater Pollution Prevention Plan (SPPP) documents. The current permit was available for review and was included in the SPPP at the time of Mr. Gerstells inspection. This facility was required to develop and maintain a Stormwater Pollution Prevention Plan (SPPP). A Notice of Deficiency (NOD) was issued to this facility on September 2, 2016. It is noted that significant improvements have been made to the facilities records and to the facility condition itself since that date. At the time of the multimedia inspection on August 17, 2017, the facility had not provided required annual employee training, had not been providing bi-annual qualitative monitoring, had not updated some areas of the SPPP and had not been signing annual certification of the SPPP. NC 27103 336 776 9800 15A NCAC 2D .1806 requires the facility to control and prohibit odorous emissions. No objectionable odors were present during the inspection. There are no odor complaints recorded in ]BEAM . Compliance with .1806 is indicated. MACT/GACT: The facility's exempt No. 2 Fuel Oil Fired 1991 Hurst FB350-15-2 with a 140,OOOBTUIhr. heat input (IES-2-PLT 1) is subject to 40 CFR 63 Subpart JJJJJJ, also known as Boiler GACT. The required initial notification form and EPA's CEDRI/CDX submittal were noted as received in a prior July 28, 2014 DAQ compliance inspection and information can be found in the source file. This regulation requires for a boiler of this size (< 5 million Btu/hr. heat input) to conduct tune-ups every 5 years with an initial tune up date of March 21, 2014. The facility conducted their initial tune-up on July 16, 2014 (late and resulted in July 29, 2014 NOV) and the next tune-up will need to be conducted by July 30, 2019. The facility did submit their Notice of Compliance Status (NOCS) on July 17, 2014 via the Compliance and Emissions Data Reporting Interface (CEDRI). Compliance is indicated at this time. NSPSI NESHAP APPLICABILITY: Other than the Boiler subject to NESHAP 6J and referenced earlier in this report, the facility is subject to no other NSPS or NESHAP emission standards, as there are no RICE engines onsite used for any emergency purposes and there are no diesel -fired engines used for water suppression. FACLITY WIDE EMISSIONS: Facility wide actual emissions, as reported by this facility for CY 2010 and CY 2015 are located in the report header. COMPLIANCE HISTORY (last five years): On July 28, 2014, a compliance inspection performed by Mr. Ray Stewart, Jr., P.E., WSRO-DAQ Compliance Supervisor, and Mr. Michael Shepherd, former Environmental Specialist, resulted in a Notice of Deficiency issued on July 29, 2014 for a late initial boiler -tune up. The required tune-up date was March 21, 2014 and the facility records indicated that the tune-up was performed on July 16, 2014. The Notice of Deficiency was mailed to the facility on July 29, 2014, no response letter was requested since the tune up was completed prior to the inspection. On the March 27, 2013 compliance inspection performed by Mr. Davis Murphy, DAQ/WSRO Environmental Specialist (now Environmental Engineer), the facility received a Notice of Deficiency (NOD) for failing to record inspections for the cement silo bagfilter CD -I for over a year. The Notice of Deficiency was mailed to the facility on April 2, 2013. The facility's response letter was received on April 8, 2015, it stated that the facility would be redistributing job duties of its personnel in order to meet the requirements for bagfilter maintenance. Mr. Richardson, Plant Manager, recorded an inspection of the cement silo bagfilter into the logbook for March 27, 2013. CONCLUSION: Based on visual observations and records reviewed, the facility appeared to be operating in compliance with all applicable Air Quality standards and regulations at the time of this compliance assurance visit. The following observations and violations were noted during the NCDEQ multimedia inspection: Stormwater Pollution Prevention Plan: • NCG 070000, Part H Section A requires the permittee to develop and implement a Stormwater Pollution Prevention Plan that includes items 1 thru 9 of Section A. While an SPPP was provided, it was found to be deficient. The facility was deficient with annual certification of the SPPP as well as annual updating of other areas of the plan, and has not provided or documented annual employee training as required. Qualitative Monitoring (Bi-Annual) • NCG 070000, Part 11 Section C requires qualitative monitoring to be conducted twice annually at each outfall. The facility was deficient in that it did not monitor twice annually as required and has not documented or provided monitoring at each outfall. Representative Outfall Status has not been determined and it appears that this facility has outfalls that do qualify. Monitoring requires the person conducting monitoring to go to each outfall twice each year and document on the qualitative monitoring form what was observed. It is a visual inspection only. Fill out a separate form for each outfall. Please begin monitoring twice (2) annually as permit requires. Your Required Response: Accordingly, you are directed to respond to this letter in writing within 30 calendar days of receipt of this Notice. Your response should outline how the violations will be addressed and should be sent to this office at the letterhead address and include the following: • Stormwater Pollution Prevention Plan: In your response, tell us how you will address the deficiencies of each of the 9 items of Part II, Section A of the permit and let us know when you expect to have this issue corrected. • Qualitative Monitoring: Begin qualitative monitoring for each outfall. Monitoring should identify color, odor, clarity, floating solids, suspended solids, foam, oil sheen, erosion or deposition and any other obvious indication of pollution. A photo taken of the outfall at the time of inspection provides excellent documentation. Your response should include your expected compliance with bi-annual monitoring. For assistance with determining outfall locations and which outfalls will require monitoring as well as assistance with other questions related to industrial stormwater, contact Glen White at 336-776-9660. Failure to correct the violations may result in this office's recommendation for enforcement by the Director of the DEMLR (Division of Energy, Minerals, & Land Resources) if the issues and any future violations continue. This office requires that the violations, as detailed above, be abated immediately and properly resolved. Environmental damage and/or failure to secure proper authorizations have been documented on the subject tract as stated above. Your efforts to undertake activities to bring the subject site back into compliance is not an admission, rather it is an action that must be taken to begin to resolve ongoing environmental issues. It appeared that the facility may have as many as 3 - 4 outfalls on site. It appeared that material from the sand and gravel stockpile area located on the opposite side of Brushy Creek had deposited into the creek. In the previous inspection report, Mr. Glen White requested that an outfall be provided in this location and the old retaining wall be repaired or a berm constructed to minimize the erosive effect and for saftey. Since the last -inspection, five concrete butresses had been placed in this location to address Mr. White's concerns. Mr. Dwayne Richardson stated that he was needing assistance from DEMLR to inform him where the butresses should be placed to address Mr. White's conerns. Mr. Richardson's request for guidance will be forwarded to DEMLR via this inspection report. It appeared that aggregate from stockpiles were entering multiple stormdrains located directly adjacent to the production building on the west side. This condition may be considered a release of wastewater which could trigger requirements in the 140000 series permits to nclude analytical monitoring. This concern will be referred to DEMLR staff. This inspection report will also be referred to DEMLR staff to determine status of compliance in reference to the Notice of Defciency that was issued to Leonard Block Company on September 2, 2016. Facility still needs to document employee training as requested on the previous inspection report. This was discussed with Mr. Richardson and Ms. Miller. Responsible party information was updated since the last inspection to show Mr. C.H.B. Leonard as the sole responsible party. For annual updating, many portions of the document are still in need of updating. Annual updating of the SPPP was discussed with Mr. Richardson and Ms. Miller. It is recommended that DEMLR staff contact the facility to provide additional guidance. It appeared that only one outfall had been monitored in the past; but inspection appared to reveal that there may be as many as three to four separate outfall locations leading from multiple stormdrains on site. Outlets from stormdrains on the west side of the facility appeared to lead to Brushy Fork. However, outfalls leading from the storm drains on the west side of hte facility could not be observed due to safety concerns. Stormdrains observed on the east side of the facili!Y appeared to daylight at a concrete wall located adjacent to an unnamed tributary to Brush Fork. REGAN Secretary TRACY DAVIS Director On August 17, 2017, Charles Gerstell, Environmental Senior Specialist with the North Carolina Department of Environmental Quality — Division of Waste Management (NCDEQ — DWM) conducted a multimedia inspection at the facility located at 2390 Midway School Road in Winston-Salem. The inspection discovered several deficiencies regarding the facilities industrial stormwater permit. This facility holds General Stormwater Permit NCG070197 to discharge stonrnwater from industrial activity associated with Stone, Clay, Glass, and Concrete Products [SIC 321 under the National Pollutant Discharge Elimination System (NPDES). The permit became effective June 1, 2013 and expires on May 31, 2018. A copy of the current permit is required to be maintained with the Stormwater Pollution Prevention Plan (SPPP) documents. The current_ permit was available for review and was included in the SPPP at the time of Mr. Gerstells inspection. This facility was required to develop and maintain a Stormwater Pollution Prevention Plan (SPPP). A Notice of Deficiency (NOD) was issued to this facility on September 2, 2016. It is noted that significant improvements have been made to the facilities records and to the facility condition itself since that date. At the time of the multimedia inspection on August 17, 2017, the facility had not provided required annual employee training, had not been providing bi-annual qualitative monitoring, had not updated some areas of the SPPP and had not been signing annual certification of the SPPP. NC 27103 :i3b 776 9800 The following observations and violations were noted during the NCDEQ multimedia inspection: Stormwater Pollution Prevention Plan: NC_G 070000, Part II_Section A requires the permittee to develop and implement a Stormwater Pollution Prevention Plan that includes items 1 thru 9 of Section A. While an SPPP was provided, it was found to be deficient. The facility was deficient with annual certification of the SPPP as well as annual updating of other areas of the plan, and has not provided or documented annual employee training as required. Qualitative Monitoring (Bi-Annual) • NCG 070000, Part lI Section C requires qualitative monitoring to be conducted twice annually at each outfall. The facility was deficient in that it did not monitor twice annually as required and has not documented or provided monitoring at each outfall. Representative Outfall Status has not been determined and it appears that this facility has outfalls that do qualify. Monitoring requires the person conducting monitoring to go to each outfall twice each year and document on the qualitative monitoring form what was observed. It is a visual inspection only. Fill out a separate form for each outfall. Please begin monitoring twice (2) annually as permit requires. Your Required Response: Accordingly, you are directed to respond to this letter in writing within 30 calendar days of receipt of this Notice. Your response should outline how the violations will be addressed and should be sent to this office at the letterhead address and include the following: • Stormwater Pollution Prevention Plan: In your response, tell us how you will address the deficiencies of each of the 9 items of Part II, Section A of the permit and let us know when you expect to have this issue corrected. • Qualitative Monitoring: Begin qualitative monitoring for each outfall. Monitoring should identify color, odor, clarity, floating solids, suspended solids, foam, oil sheen, erosion or deposition and any other obvious indication of pollution. A photo taken of the outfall at the time of inspection provides excellent documentation. Your response should include your expected compliance with bi-annual monitoring. For assistance with determining outfall locations and which outfalls will require monitoring as well as assistance with other questions related to industrial stormwater, contact Glen White at 336-776-9660. It appeared that material from the sand and gravel stockpile area located on the opposite side of Brushy Creek had deposited into the creek. In the previous inspection report, Mr. Glen White requested that an outfall be provided in this location and the old retaining wall be repaired or a berm constructed to minimize the erosive effect and for saftey. Since the last inspection, five concrete butresses had been placed in this location to address Mr. White's concerns. Mr. Dwayne Richardson stated that he was needing assistance from DEMLR to inform him where the butresses should be placed to address Mr. White's conerns. Mr. Richardson's request for guidance will be forwarded to DEMLR via this inspection report. It appeared that aggregate from stockpiles were entering multiple stormdrains located directly adjacent to the production building on the west side. Site Activities The facility is a concrete block manufacturing plant which began operating in 1921. Activities on site include loading and unloading of raw materials, manufactured product and building supplies, manufacturing of concrete blocks, and storage of manufactured products. No processing or operations take place outside the main plant area. Outdoor areas are for storage and unloading/loading purposes only. Dust/particulate controls are in place for the cement silo. The baghouse filter in place on the cement silo controls dust emissions on unloading. The cement silo, screw and weigh hopper are part of a closed system to deliver the bulk cement to the mixer. No other manufacturing elements of the facility are involved in any form of dust generating process, including sawing or cutting of product beyond normal material handling. Waste products are recycled when possible including raw materials, manufacturing detritus, and inferior manufactured product. Motor oil and hydraulic fluids are stored in a waste oil tank. The waste oil is picked up by Noble Oil Services. Supporting documents related to waste oil pickup are available. As normal vehicle and equipment maintenance is performed on site, and machinery and vehicles are operating on a daily basis, the potential area of pollutants would be motor oil and hydraulic fluid lost during normal vehicle and machinery operation. 4 A Feasiblity Study Upon examining the existing methods of operations and/or storage practices, the following items have been considered. l . Aboveground diesel fuel tank — possibly add secondary containment concrete berm around edge of concrete pad containing tank and fuel pump. Motor oil and hyrdraulic fluid are kept in 55 gallon drums under shelter. There are 2 above -ground tanks and one underground tank. I am unaware of any changes which would increase the ability to control any runoff. B. Secondary Containment Records C� 1. Motor Oil: 1 55-gallon drum. 2. Hydraulic Oil: 1-2 55-gallon drum. 3. Waste Oil: 1 #00 gallon tank Fifty-five gallon drums are kept either under a shed or inside the plant buildings. The propane tank is inspected by Carolane Propane on a yearly basis. The fuel n oil tank is used for heating oil purposes, and is not regulated. The diesel fuel tank 4` is under a shelter on the side of truck storage building. The waste oil tank is also under'a shelter. An additional aboveground tank is on site, but not is use. C. BUT Summary No BMP have been implemented at this time. 3. Spill Prevention and Response Plan (SPRP) An assessment of potential pollutant sources seems limited to incidental presence of motor oil, hydraulic fluid, and fuel from normal vehicle and equipment usage. Fluids from maintenance operations are collected and put in the waste oil tank. Overall responsibility for the SPRP will lie with the Safety Coordinator. Preventive Maintenance and Inspections A. Maintenance Each plant area should be kept in a clean and presentable manner. The ideal of preventive maintenance is to keep each area clean and clear of debris and potential pollutants. Trash pickup will be performed as needed. A licensed waste management provider, North Davidson Garbage, shall pick up trash once a week. Drains shall be kept clear from litter and debris. B. Inspections Inspections of the facility and all stormwater systems shall occur at a minimum of once during each six month period from January to June, and July to December. The inspection shall be documented along with any changes in the maintenance or operation of the stormwater control system, equipment and machinery. A schedule of monthly inspections of stormwater outfalls, control systems, plant equipment, loading and unloading areas, and other facility locations that have a potential impact on stormwater runoff shall be implemented. The responsible person for performing the inspections shall be the Safety Coordinator. 5. Employee Training Employees shall be trained for proper spill response and cleanup procedures for any operations which have the potential to contaminate stormwater runoff. Training will be provided and documented by Erie Insurance on an annual basis. Erie Insurance Risk Control personnel will conduct the training sessions. 6. Responsible Party The Safety Coordinator,shall be responsible for the overall coordination, development, implementation, and revision to the plan. Basic cleanup services shall be performed by all members of the staff at Leonard Block Company. 7. Plan Amendment A. The Stormwater Pollution Prevention Plan shall be amended upon a change in design, construction, operation, or maintenance which has a significant effect on the potential for the discharge of pollutants to surface waters. The Stormwater Pollution Prevention Plan shall be reviewed and updated on an annual basis with attached lists of significant spills and recertification that the stormwater outfalls have been evaluated for the presence of non-stormwater discharges. 8. This facility is not subject to the RMP requirements of the I I2(r) program since they do not use or store any of the regulated pollutants in quantities greater than the thresholds listed in the rule. They are only subject to the General Duty requirements contained in'the General Duty Clause. INTRODUCTION: On August 22, 2016, Mr, Blair Palmer, WSRO-DAQ Environmental Specialist and WSRO Multimedia Inspector, contacted Ms. Tammy Miller, Accounts Payable, and Mr. Dwayne Richardson, both employees for Leonard Block Co., for an unannounced targeted compliance inspection at Leonard Block Company located in Davidson County and operating under Air Permit No. 05931R06. The facility manufactures concrete blocks for mainly construction purposes. This facility was last inspected on September 2, 2015 by Mr. Avery Waring, WSRO DAQ Environmental Specialist, and this permitted facility appeared to be operating in compliance with DAQ regulations at that time. This inspection was not part of the multimedia program. The National Pollutant Discharge Elimination System (NPDES), specifically the stormwater and wastewater program is part of the August 22, 2016 inspection, but that information is discussed in a different database and report known as the BasinWide Information Management System or "BIMS." The facility has a plant at 2390 Midway Road where all operations are conducted and a 133 Midway Road non -operational plant where ES- 2 and its control system CD -I are located. Facility personnel state that the 133 Midway Rd. operation will remain out of operation and there are no plans to build it in the foreseeable future. Mr. Richardson and Ms. Miller confirmed the contacts listed in the IBEAM database and no changes are needed in IBEAM. The hours of operation at this plant are Monday -Friday (7:00am — 4:30pm), and 50 weeks a year. DISCUSSION: The facility was idle prior to the start of compliance inspection and remained idle during the inspection. The process begins with aggregate that manually loaded into a small hopper, which then falls onto a conveyor and then to a compartmented storage bin outside of the main building before fed to the weigh batcher. Generally, all material is usually loaded for the day in the first hour and a half of the work day. Cement Storage Silo ES-1 is used to auger feeds a gray cement into the weigh batcher. The cement silo was not operating at the time of inspection and Mr. Richardson commented that the facility doesn't have any problems with delivery of the cement feeding into the silo. From the weigh batcher cement and aggregate are mixed and water is added before the concrete mixture is sent to the block stamping machine. After being stamped the blocks are place onto trays and sent to one of the five kiln rooms. The kilning process is generally 24 hours after which the blocks are moved to a storage area in the rear of the facility. At the time of the August 22, 2016 inspection, the kiln area and the exempt boiler were not in operation due to some maintenance. IES-2- PLTI is a No. 2 Fuel oil -fired boiler that is used to provide steam heat for the kilns. The boiler is subject to NESHAP Subpart JJJJJJ and this rule is discussed in more detail under the MACTIGACT section of this report. IES-3-PLT is the 2,000-gallon fuel oil storage tank that supplies the boiler, it appeared to be in ok condition at the time of inspection. Lastly the small screen crusher (part of [ES-4-PLT 1) was viewed on -site and it is being repaired according to Mr. Richardson. The crusher is used to recycle unusable blocks into aggregate for future block mixtures. The facility attempts to recycle some of the unused block material (after crushing), but many of the blocks are just stored in various piles throughout the premises. There are also two conveyors used at the facility (as part of IES-4 PLT 1), PERMIT CONDTIONS: Condition A.2 is the Permit Renewal and Emissions Inventory Requirement. The facility's permit will expire on November 30, 2016. A permit renewal request letter must be submitted 90 days prior to permit expiration or by September 1, 2106. Included with the renewal request, a 2015 Emissions Inventory (EI) report with a signed certification sheet needs to be included. Ms. Miller is working on both the renewal and EI. Compliance with 2Q.0304 and 213.0202 is anticipated and Ms. Miller is ware of the September 2, 2016 due date. Condition A.3 contains the 2D .0515 requirements associated with particulates from miscellaneous industrial processes. The allowable PM emission rate for silo ES-1 and ES-2 are each 35.4 lb PM/hr. With the bagfilters having a control efficiency of 99%, calculated emissions (taken from Mr. Leo Governale's, WSRO-DAQ Permit Engineer, review dated December 9, 2011) from ES-1 and ES-2 are each 0.025 lbs PM/hr. Compliance with 2D .0515 is indicated. Condition A.4 contains the "Visible Emissions Requirement," a sets a 20% opacity limit for bagfilter CD -I. Emission Source, ES-2, and associated control device, CD-2 is not installed at the 133 Midway School location at this time. A review of the maintenance records and discussion with Mr. Richardson suggests that the bagfilter is well maintained, and compliance with 2D .0521 is expected. Condition A.5 pertains to regulation 2D .0535 "Notification Requirement." The facility is to notify the DAQ director of any source with excess emissions that last for more than four hours and that result from a malfunction, a breakdown of process, or any other abnormal conditions. No notifications have been received since the last inspection and facility personnel were unaware of any issues. Compliance is indicated. Condition A.6 (21) .0540) concerns the control of fugitive dust emissions. Mr. Palmer observed no fugitive dust emissions near the facility boundary and no complaints have been received, indicating likely compliance. The facility will use a truck to water the haul roads during summer months or during abnormally dry conditions. The facility was not watering the aggregate stock piles during the inspection and no dust was observed. Compliance is indicated. Condition A.7.(2D .0611) concerns the control of particulate emissions. The facility is required to perform, at minimum, an annual internal inspection of the bagfilters and record all maintenance activities in a logbook to be made available to Division personnel. The last internal inspection for both bagfilters was performed on July 22, 2016 no problems were noted. The bags (a total of 13 thirteen) were changed on December 18, 2015. The facility keeps extra bags on -site. Compliance is indicated. Condition A.8 (21) .1806) concerns the control of odorous emissions. Mr. Palmer detected no objectionable odors during the inspection nor have any complaints been received, indicating likely compliance. Condition A.9 pertains to "Federal Rules Applicable to Sources Exempted from Air Permitting Requirements." This condition references 40 CFR Part 63, Subpart JJJJJJ(6J) and is applicable to iES-2-PLT 1. This is discussed in more detail under the MACT/GACT section of this report. Condition A.10 pertains to upon becoming aware of additional information (credible air emissions data) that has not previously been considered in the permit review, the DAQ Director may request a facility to submit additional documentation such as emissions estimates and air dispersion models. The permit may be modified by the Director with additional emissions controls and restrictions found necessary to meet the requirements of applicable regulations. It doesn't appear that any changes have been made at the facility that would require reopening. Compliance with N.C.G.S. 143-215.108(c) is expected. Condition A. I 1 pertains to "Toxic Air Pollutant Emissions Limitation Requirement," and sets limits on facility wide limits for toxic air pollutants (TAPS). In accordance with 2Q .0711 the facility must operate in a manner that TAPs including those from fugitive emissions do not exceed Toxic Permit Emission Rate (TPERs) limits. The facility is required to obtain a permit prior to exceeding any of the TPERs and to show compliance with 2D.1100 "Control of Toxic Air Pollutants." The required initial notification form and EPA's CF.DRI/CDX submittal were noted as received in a prior July 28, 2014 DAQ compliance inspection and information can be found in the source file. This regulation requires for a boiler of this size (< 5 million Btu/hr heat input) to conduct tune-ups every 5 years with an initial tune up date of March 21, 2014. The facility conducted their initial tune-up on July 16, 2014 (late and resulted in July 29, 2014 NOV) and the next tune-up will need to be conducted by July 30, 2019. NSPS/ NESHAP APPLICABILITY: Other than the Boiler subject to NESHAP 6J and referenced earlier in this report, the facility is not subject to no other NSPS or NESHAP emission standards, as there are no RICE engines onsite used for any emergency purposes and there are no diesel -fired engines used for water suppression. PERMIT DISCUSSION: At this time all information found in the current air permit appears to be accurate. The plan when updated should have at least two people as the responsible party (Tammy Miller and Dwayne Richardson), Mr. Leonard is seldom on -site anymore, The facility was unaware of the qualitative monitoring, including the use raingauge. The facility has a 300 diesel Pule tank (AST), but it is empty according to plant personnel. Any oils used at the facility are generally stored in 55 gallon drums. Mr. Palmer advised the facility to keep any oils not outside. Also. There is a painttwelding area, which facility personnel were unaware of (see file pictures). It is located near the aggregate area across the creek. This needs to be follow-up upon and WSRO-DEMLR have been informed. The next permit cycle should consider the fact that the aggregate piles would trigger wastewater requirements in the 140000 series permits, which would include anaytical monitoring. This facility may want to be considered a 140000 permit instead of a 070000. The best way to deal with it is to get them under the permit. That will give me the some extra teeth to address that and any other issues. Sometimes it has been things that have fallen from them, and other times it has been the footings and/or foundation of them that have caused erosion/sedimentation problems. I will look at everything when I visit the site after the permit is in place. We will address any concerns I have at that time. You will realize, once you start developing a Stormwater Pollution Prevention Plan, that one of, if not the main objective of the permit is to force the permittee to be proactive in preventing contamination of any streams/wetlands, whether directly or indirectly via ditches etc. It is much easier/cheaper to prevent a problem than to fix one. With the permit in mind, if you see anything on site that you think needs to be addressed, by all means, don't feel like you have to wait until I visit to do so.
