VAN DER VAART Secretary TRACY DAVIS Director On June 11, 2016 Glen White of the North Carolina Department of Environmental Quality; Department of Energy, Minerals, and Land Resources met with you at the facility located at104 Williams Street, Thomasville, to conduct a compliance inspection per the Industrial Stormwater Permit requirements. This inspection consists of review of the Stormwdter Pollution Prevention Plan, training, monitoring and spills records as well as observation of the general condition of the site, the outfalls and any possible exposures to downstream stormwater. Your assistance with the inspection was greatly appreciated. Permit: This facility holds General Stormwater Permit NCG070181 to discharge stormwater from industrial activity associated with Stone, Clay, Glass and Concrete Products [SIC 321 under the National Pollutant Discharge Elimination System (NPDES), The permit became effective June 01, 2013 and expires on May 31, 2018. A copy of the current permit is required to be maintained with the Stormwater Pollution Prevention Plan (SPPP) documents. The current permit was available for review and was included in the SPPP. Records/Reports: This facility was required to develop and maintain a Stormwater Pollution Prevention Plan (SPPP) in accordance with Part II, Section A of the permit. The SPPP has been updated annually as required and contained all components as required. No deficiencies noted. Facility Site Review: Finch Industries is a mirror manufacturer and glass fabricator. This facility makes 3500 SF of mirror per hour during a typical work day. A big part of the operation involves beveling the mirror edges for a more elegant finished look. The plant also has the ability to drill holes into the glass and can make other specialty mirrored products including antique mirrored styles, graphics additions and prints. The plant employs approximately 50 employees who all work a 40 hour week. Clients are furniture, home and mobile home industry. Suite 300 I Winston-Salem, NC 27103 336 776 9800 Effluent Receiving Waters: Effluent from this site drains to Hamby Creek, class "C" waters of the Yadkin Pee -Dee River Basin. Self -Monitoring Program: This plant has several outfalls. Each outfall was observed during the inspection. The site is surrounded by streets and other industry and therefore each outfall is subject to contamination by others. Only qualitative monitoring is required. Monitoring appears to be conducted as required. The facility was found to be compliant with requirements of its industrial stormwater permit.
