Mike,
Sorry but everyone does the modification different, For example, the regular program doesn't charge a fee, applicants
pay to use the express program. Besides the regular program doesn't have a Stormwater Management Plan Application
Form. I understand your concern but to go through the Express please fill out the Form and provide as much information
as possible. I just looked through the Application Form and there's not much required that cannot be produced. If you
have a specific question on the Form we can help address that. With respect to the Drainage Area Breakdown
Information Table, and the Submittal Requirements under Section VI, please complete as much as you can and mark
others N/A.
Please submit the design calculations, supplement form and 0 & M for the grassed swale, along with the full set of plans
for both the stormwater wetland and grass swale. Let me know if there is any other question. Thanks.

Joseph, in reading over the acceptance letter, I do have a couple of questions relating to Items B and C.
Item B: This is requesting a completed Express Review Stormwater Permit Application. Since this pond was reviewed
only in conjunction with the 401 Certification initially, it was never permitted by a full Stormwater Permit Application. This
Application requires extensive information which isn't relevant to the current review, since we are not requesting that the
functioning of the pond be modified. We are only requesting that to make certain repairs which would allow the pond to
function as initially designed. In the past we have been able to make this kind of modification with a detailed narrative and
transmittal letter that fully explained the request.
Item C: This item requires several different documents, including design calculations for the existing Stormwater
Wetlands, and current supplement forms for the Stormwater Wetlands. The problem we're going to have is that the pond
was designed and permitted under the old design standards. It will not meet the current design standards, which the BMP
supplement forms are set up for. Again, we are not asking to modify the function of the originally permitted pond, we are
only asking to make a modification to allow it to function as originally designed. Obviously we will be providing the BMP
Supplement Forms for the new grass swale. Regarding the signed/notarized Operation and Maintenance Agreement, I
am assuming that this would only need to cover the new grass swale, since the original pond system already has an O&M
agreement in place. Please let me know if this is not correct.
Please let me know your thoughts on these items so we can proceed with preparation of the necessary materials
accordingly. Thank you.

Subject: Initial Interest Form submittal package - detention system modification at Garner Retail Site
Joseph, per our discussions during our on-site meeting earlier this week, please find attached the Initial Interest Form and
supporting documentation. We would like to permit our proposed Stormwater Modification through the Express Review
Process.
The Initial Interest Form isn't set up very well for requesting a stormwater modification, so please review how I completed
the form and let me know any questions or comments.
