On behalf of Sampson County Disposal, LLC [SCD], Smith Gardner IS+G1 is pleased to submit
this Annual Summary Discharge Monitoring Report (DMR) for monitoring at the SCD Mine
Facility. The subject site is located in Roseboro, North Carolina. The SCD Mine Facility is
currently permitted to discharger stormwater from mine activities under Certificate of
Coverage (CDC] NCG020628, General Permit NC6020000_
We are providing you with two copies of the report, each including:
• Signed and dated DMR Form; (Attached)
• Site Map prepared by S+G; (Appendix 1)
• Flow calculations prepared by S+G; and, (Appendix 21
• Laboratory Reports. (Appendix 3, only in the version kept on -site, not
submitted)

All mines must monitor WW discharges for TSS. Only industrial sand mine discharges are subject to TSS limits.
'" If an effluent limit is exceeded, the permittee is required to institute monthly monitoring for that parameter for the remaining permit term.
`The discharge shall not cause an instantaneous measurement of the turbidity of the receiving water to exceed: 10 NCu for freshwater streams, lakes, and reservoirs
designated as trout waters; 25 NTU for all lakes and reservoirs, and all salt waters; 50 NTU for all other streams and surface waters. Turbidity may be monitored at the
Stormwater Discharge Outfall. Alternatively, the permittee may choose to monitor turbidity in the receiving water, directly upstream and downstream of the discharge

this Annual Summary Discharge Monitoring Report (DMR1 for monitoring at the SCD Mine
Facility. The subject site is located in Roseboro, North Carolina. The SCD Mine Facility is
currently permitted to discharger stormwater from mine activities under Certificate of
Coverage (CDC) NCG020628, General Permit NCG020000.
We are providing you with two copies of the report, each including:

Report (DMR) for monitoring at the SCD Mine Facility. The subject site is located in Roseboro,
Sampson County, North Carolina. The SCD Mine Facility is currently permitted to discharge water from
mine dewatering activities under Certificate of Coverage (COC) NCG020628, General Permit
NCG020000. This report is also submitted regarding correspondence regarding pH limits dated June 22,
2004 from Mr. Alan W. Klimek, P.E., Director of Water Quality, to Mr. Matthew S. Lamb, Project
Scientist with G.N. Richardson & Associates, Inc. (GNRA).
We are providing you with two copies of the report, each including:

The Division has reviewed your request for a variance to the pH limit for the Subject Stormwater/Wastewater Permit due
to the naturally occurring low pH of the waters at the SCD Mine. These low pH waters are the result of waters in the mine
pit being in contact with the high sulfur content soils that occur in the Black Creek geologic formation in this area of
Sampson County. Your analytical work has shown that while pH of the discharges from the mine are lower than the
General Permit requirement of between 6.0 and 9.0 (with designated swamp waters being allowed a pH as low as 4.3 if
due to natural conditions) that stream water quality standards are not being contravened in the downstream waters. The
discharge from the mine dewatering is regularly in the 3.0 to 3.5 range. Stream measurements have indicated that the
downstream waters are not being caused to fall below the water quality standard of 4.3.
Based upon your request and supplied stream data, we have determined that your request for variance is granted. The
new limit for the permit (specifically for this facility) is that the pH of the upstream and downstream waters be
measured on the same date that the effluent samples are taken with the pH in the downstream waters not being
caused to fall below 4.3 and the pH limit on the effluent is suspended. This variance will continue for the future
versions of NCG020000 without having to be renewed even though the general permit will have to be reissued every five
years (the current NCG020000 will expire on November 30 this year).

On behalf of Sampson County Disposal, LLC (SCD), G.N. Richardson & Associates,
Inc. is submitting the quarterly Discharge Monitoring Report (DMR) for monitoring at
the SCD Mine Facility, located in Sampson County, North Carolina during the quarter
beginning December 1, 2003 and ending February 29, 2004. The SCD Mine is
currently permitted to discharge water associated mine dewatering activities under
certificate of coverage NCG020628 and general permit NCG020000. This permit is
active, although the SCD Mine is currently inactive, is not anticipated to be activated
in the near future, and there are no exposed significant materials at the Mine related
to past mining activities.
Discharge Monitoring Report
Table 6 of Section CA of the general permit contains monitoring requirements for
mine dewatering. Parameters required to be analyzed include pH, settleable solids,
turbidity, and total flow. Total suspended solids (TSS) are not required to be
analyzed, given that no mining of industrial sand occurs at the SCD Mine.
All discharges are the result of natural conditions, including groundwater seepage
and surface runoff. Water flows over the ground surface and through a series of
ditches into the settling basin, which in turn discharges through a spillway,
designated as outfall 001. Below the spillway, the water disperses through several
small channels until it reaches the receiving stream, Little Coharie Creek, which is
designated as swamp waters. No pumping of groundwater occurred during the
monitoring period. The DMR form is included in Attachment 1. Analytical results as
reported by the North Carolina certified laboratory are included in Attachment 2. A
map depicting the Mine area and outfall 001 is included in Attachment 3. This map
also depicts outfalls 002 and 003, which were permitted in the Notice of Intent (NOI),
but do not currently discharge effluent from the SCD Mine site.
Daily Flow Approximation
Construction of a weir has recently been completed which will enable flow
measurements to be taken. However, this weir was not in place during the sampled

event on February 16, 2004. Therefore, average flow estimates from the previous
quarterly monitoring period continue to be used during this monitoring period. Actual
flow measurements will be used during the next quarterly monitoring period to
determine sampled daily flow.

t� pH Effluent Limitation Exclusion Request
The SCD Mine Facility is also requesting exclusion from pH effluent limitations. The
entire site, and much of North Carolina's tidewater and coastal plain regions, is

underlain by the naturally occurring Black Creek geologic formation. This formation
is characterized by soils with high sulfur content. When these soils are exposed, an
oxidation reaction takes place that causes the pH of the soils to drop. Portions of the
Black Creek formation are exposed at the SCD Mine. As a result, effluent pH is
affected.
pH measurements have been taken at the outfall 001, as well as upstream and
downstream of the point where the effluent enters the Little Coharie Creek. pH of
effluent discharging through outfall 001 measured 3.17 standard units (s.u). A pH
measurement of 4.80 s.u. was taken upstream from the outfall, at a bridge where
Highway 24 crosses Little Coharie Creek. A downstream pH measurement of 5.25
was taken from the Little Coharie Creek approximately 1.5 miles south of the SCD
Mine site. Upstream and downstream sample locations are based on information
provided by SCD personnel.
According to effluent limitations contained in the general permit, designated swamp
waters can have a pH as low as 4.3 s.u. if due to natural conditions. The pH
measurement data does not appear to indicate that the effluent discharge is
impacting the receiving stream in such a way as to cause pH levels to drop.
Based on the fact, that the low pH of effluent discharged from the SCD Mine Facility
is due to naturally occurring conditions, and that the effluent does not appear to
negatively impact the quality of the receiving stream, the .SCD Mine Facility is
requesting exclusion from pH effluent limitations'
Daily Flow Approximation
During the sampled event on October 31, 2003, no flow measurement data were
collected, due to the fact that a flow measurement device was not available at the
site. Instead, two (2) methods are used to approximate flows over a range of
discharge events. In the first method, Darcy's Law was utilized to approximate flow
through the rip -rap cross-section of outfall 001. In the second method, Manning's
Law was used to calculate flow. Flow depth was assumed to vary between two (2)
inches and twelve (12) inches. Flow gradient was also assumed to be essentially
flat. Average flow is therefore estimated to be approximately 3 million gallons per
day (MGD). Calculations for these two (2) methods are included as Attachment 4.

Parts 1, II, 111, IV, V, VI and VII of General Permit No. NCG020000 as attached.
This certificate of coverage shall become effective September 19, 2003.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
