Ms. Aana Taylor -Smith of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ)
conducted a compliance evaluation inspection at the subject facility on 21 June 2013. The assistance and
cooperation of Mr. Marvin Loftis were greatly appreciated. An inspection checklist is attached for your records

This facility is located at 548 Shelton Road in Stokesdale, Rockingham County, North Carolina. Stormwater
related to industrial activity is discharged from the site to an unnamed tributary to Belews Creek (Kernersville
Lake), which is currently classified as Class WS-IV (water supply), CA (critical area) waters, in the Roanoke River
Basin.
Documentation & Monitoring Review
The Stormwater Pollution Prevention Plan (SPPP) was reviewed and found to be mostly complete, with one
exception. Annual employee training has been conducted, but has not been documented as required by Part II,
Section A-6 of the permit. Stormwater training is required at least annually and training must be documented
with the date and the signature of each employee in attendance.
Problems with the SPPP were described in the previous inspection, conducted 9 December 2009, with the
resulting recommendation that an engineering or consulting firm be hired to develop an effective SPPP. We

In addition to the above problem, analytical monitoring (AM) has not been conducted as required. Mr. Loftis
indicated that he was not aware that AM must be conducted semi-annually and had instead been conducting

AM only once per year. Please be aware that AM must be conducted twice per year as described in Part 11,
Section B of the permit. Carolina Campin Incorporated must correct this deficiency by beginning monitoring as
required in the permit at the next possible opportunity. Qualitative monitoring (QM) has been conducted and
recorded as required..
Site Review
The facility is orderly and well -maintained. All necessary secondary containment is implemented as required. All
Stormwater Discharge Outfalls (SDOs) were examined during the inspection. The SDOs were clean and well -
maintained, with,no visible foam or floating solids. No evidence of erosion or instability at the SDOs was
observed.
Thank you for your attention to this matter. Remedial actions should be taken to correct these deficiencies.
Unresolved deficiencies may lead to the issuance of a Notice of Violation and/or assessments of civil penalties by
the Division of Water Quality.

On 18 June 2010 your application requesting Representative Outfall Status was received in the Winston-Salem
Regional Office of the North Carolina Division of Water Quality. The application was prepared on your behalf by Mark
Talbert of Dc4v bel iy! Gild Davit ill c., for the lal.11lty located MU at J-tU Shel iVn Rd, In Li le TVWII of JLVkejdale, Ili i-\UUr1II lg halll
County, North Carolina. Stormwater related to industrial activity at this facility discharges to Belews Creek (Kernersville
Lake).
Mr, Talbert identified twenty-five stormwater outfalls at this facility as depicted in a drawing dated 6 July 2010. On
15 July 2010 Mr. Ron Boone and Mr. Mike Thomas, both of this office, conducted a site visit along with Mr. Talbert and
Mr. Loftin the Maintenance Supervisor -to evaluate these outfalls. As a result, outfalls 007 through 017 and outfalls 020
through 025 were determined to be coming from residential sources and do not need to be monitored for industrial
Stormwater.
On 26 July 2010 Mr. Talbert provided an updated drawing depicting the nine remaining outfalls that will require
monitoring to be in compliance with Permit No. NCG190024. Outfalls 001,002, 004, and 006 will require both Qualitative
Monitoring and Analytical Monitoring as outlined in the existing permit conditions. Outfalls 003,005, 007, 008, and 009
require only Qualitative Monitoring. In the event that flow is insufficient to allow sampling at 004, samples may be
collected at 007 to satisfy permit requirements as long as the location is noted at the time the sample is collected.
Please append this letter to your permit to document that representative outfall status has been approved (along
with the current site diagram dated 26 July 2010) If changes in drainage area, structures, processes, storage practices, or
other activities occur that significantly alter the basis of this approval, representative outfall status may no longer be valid.
