is hereby authorized to discharge stormwater from a facility located at:

to receiving waters designated as a UT of Richland Creek, in the Cape Fear River Basin, in
accordance with the effluent limitations, monitoring requirements, and other conditions set forth

This certificate of coverage shall become effective July 20, 2015.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

The Division of Water Quality (DWQ) has reviewed your request for an exemption from the monitoring requirements
in your NPDES Industrial Stormwater permit. Based on a site visit by Winston-Salem Regional Office staff on
March 5, 2013, DWQ is unable to concur with your request for exemption from monitoring at this time.
Observations at facility the found that industrial materials at the site are exposed or have.the potential to be
exposed to stormwater. DWQ cannot grant the requested exemption from monitoring based on the following:
e Stockpiles of gravel, sand, recycled concrete and recycled asphalt pavement were present. The
dormant status guidance prevents the storage of raw materials on -site. In addition, the benchmark
for TSS was exceeded in November 2011 and May 2012.
Chemicals were present in several totes and tanks plus the secondary containment area. The
dormant status guidance prevents the storage of chemical additives on site.
Please continue to operate under the full conditions of General Permit NCG160041. If you have any questions or need

As a result of the weak economy, we are proposing to temporarily suspend operations at
our asphalt facility located off Old Jackson Lake Road in High Point for the upcoming year; as
such the facility will be locked and unmanned. Since the site will not have industrial stormwater
discharges, we respectfully request our permit be placed in "dormant" status and that a
monitoring exemption be granted.
It is understood that the conditions of the permit will still be in effect requiring us to
maintain the Stormwater Pollution Prevention Plan, pay any associated fees, notify the regional
office when the operation resumes, and resume sampling.

Gabe — I stopped by the High Point asphalt plant on Tuesday (3/5). Quick observations revealed stockpiles of gravel,
sand, recycled concrete and asphalt pavement as well as chemical storage involving several totes and tanks plus the

chemical additives) are stored on the site". The current condition of the asphalt plant is not going to meet the dormant

Your facility is currently covered for stormwater discharge under General Permit NCG 160000. This permit will
expire on July 31, 2004- North Carolina General Statute 143.215.1(c) requires that an application for permit
coverage renewal be filed in a timely manner. A prior notice of renewal was sent for this site on December 1,
2003. To date, no response has been received by the Division of Water Quality (DWQ). In order to assure your
continued coverage under the general permit, you must apply to DWQ for renewal of your permit coverage.
Enclosed you will find a general permit renewal application form. The application must be cornpleted and returned
by April 19, 2004 in order to assure continued coverage under the general permit.
Failure to request renewal by April 9, 2004 can result in a civil assessment of at least $250.00, Larger penalties
may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility without
coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could
result in assessments of civil penalties of up to $10,000 per day.

Comment: A site visit was conducted in response to LARCO's 2/26/13 letter
requesting dormant status. Observations revealed that DWQ could not grant the
requested exemption from monitoring based on the following:
*Stockpiles of gravel, sand, recycled concrete and recycled asphalt pavement were
present. The dormant status guidance prevents the storage of raw materials on site. In
addition, the benchmark level for TSS was exceeded in November 2011 and May 2012.
*Chemicals were present in several totes and tanks plus the secondary containment
area. The dormant status guidance prevents the storage of chemical additives on site.
A letter dated 3/11 /13 was sent to LARCO advising that the request was denied and
that they should continue to operate under the full conditions of General Permit
NCG 160041.

On May 16, 2008, .fenny Graznak of this office performed a General Stormwater Permit
Inspection at the High Point Asphalt Plant located at 1330 Old Jackson Lake Road in High
Point, North Carolina. This facility holds General Stormwater Permit No. NCG160041 under the
National Pollutant Discharge Elimination System (NPDES) to discharge Stormwater from
activities associated with Asphalt Paving Mixtures and Blocks. You were present for the
inspection. This type of inspection consists of two basic parts: a review of facility files and self -
monitoring data and an inspection of the facility's discharge points. This particular inspection
evaluated five (5) areas, and observations from each area are addressed below:
1. Permit
The permit became effective August 1, 2004 and expires July 31, 2009.
2. Records/Reports
The facility has a Stormwater Pollution Prevention Plan (SPPP) that includes a location map,
site plan, storage practices, and a list of Best Management Practices on the site. Stormwater
Services Group, LLC updated the SPPP in January 2008. Employee training in stormwater
pollution prevention occurs on an annual basis.
3. Facility Site Review
The new site includes an asphalt production area and an aggregate storage area (sand, gravel,
debris, etc.). The asphalt production area houses liquid asphalt and fuel oils protected with
secondary containment. The foreman is required to observe the standing water in the
containment area for signs of contamination (oil sheen, color, foam, etc.) prior to its release.
The water is restrained by manually operated and locked valves. Records of these secondary
containment observations and releases are maintained with the SPPP. Noble Oil is typically
called upon to pick the Stormwater in the containment area if there are any signs of
contamination.
4. Effluent/Receiving Waters
Stormwater from this site drains to an unnamed tributary to Richland Creek (WS-III; NSW) in the
Cape Fear River Basin. There are two stormwater oufalls on the site, each draining from a
Stormwater retention pond. Outfall/Pond 001 drains area containing the new plant site on top of
the hill. Outfall 002 drains the lower part of the property, including the old plant site. There was
a third outfall that was eliminated during re -grading for the new site.

5_Self-Monitoring Program
The stormwater permit requires semi-annual qualitative monitoring at each stormwater
discharge outfall. An on -site review of the qualitative monitoring data during the inspection
showed that all parameters were evaluated per the specifications of the permit. The
environmental consultant made the most recent visual observation on May 13, 2008. Also, the
plant performs a monthly review of site -specific BMPs and maintains records of these
inspections with the SPPP.
Analytical monitoring is also required on an annual basis. TriTest Laboratories performed the
last analytical monitoring event on dune 24, 2007, during a rain event with a total rainfall of 0.42
inches and lasting for 3 hours and 15 minutes. The results were as follows:
