HomeMy WebLinkAboutNCG140442_COMPLETE FILE - HISTORICAL_20160801STORMWATER DIVISION CODING SHEET NCG PERMITS PERMIT NO /v (�� �`•-~(} y�I DOC TYPE HISTORICAL FILE ❑ MONITORING REPORTS DOC DATE ❑ 4 O) b Q YYYYM M DD Energy Mineral and Land Resources ENVIRONMENTAL QUALITY August 1, 2016 Ntr 1 ed Chandler Chandler Concrete 1006 S Church Street Burlington, NC 27215 PAT MCCRORY Governor DONALD R VAN DER VAART Secretary TRACY DAVIS Director Subject NOTIFICATION OF MONITORING REQUIREMENTS RELATED TO NPDES PERMIT NCG140000 FOR STORMWATER, WASTEWATER AND COMINGLED DISCHARGES Dear Mr Chandler Recently various personnel from Department of Environmental Quality have inspected Chandler Concrete facilities in the Winston-Salem Region which includes Alamance County to Watauga County and the northern NC state line to Randolph, Davie and Davidson Counties You've either received, or will receive the Inspection Reports for each individual facility For all of these facilities, there are 3 kinds of discharges to the waters of the state covered under the NCG14000 permit Stormwater discharge, wastewater discharge and comingled discharge (which is a mixture of stormwater and wastewater and is treated as wastewater) Stormwater is monitored on a semi-annual basis at all outfalls January 1-June 30 is the first time frame and July 1-December 31 is the second During each six-month period, following a significant storm event, a sample of the stormwater discharge must be obtained and analyzed by a qualified laboratory for the parameters outlined in the NCG140000 permit Part IV, Section A, Table 1 Wastewater and/or comingled water are monitored on a quarterly basis either from the wastewater basins or comingled water from all the outfalls January 1-March 31, April l -June 30, July 1-September 30 and October 1-December 31 are the dates that signify the quarters During each three-month period, during discharge from a basin or following a significant storm event, a sample of the wastewater/comingled discharge must be obtained and analyzed by a qualified laboratory for the parameters outlined in the NCG140000 permit Part IV, Section D, Table 7 During times of unusually low rainfall, or in the case of a recycle system where there is no discharge from a basin, it is acceptable to record a finding of "NO FLOW" on the Discharge Monitoring Report State of North Carolina I Environmental Quality J Energy Ntneral and Land Resources 450 West Hanes NO Road Siute 300 ` Winston Salem NC 27105 336 776 9800 Phone/336 776 9797 FAX The analytical results must be submitted to the Division on Discharge Monitoring Report (DMR) forms no later than 30 days from the date that the facility receives the results Also, a copies of all DMR's must be kept on site for a period of 3 years and available for review during inspections During the recent inspections of the Chandler Concrete facilities in this region it was noticed that for the last several recording periods, all of the DMR's (Stormwater and Wastewater) have "No Flow" wntten on them That indicates that for those recording periods, NO significant ram events have occurred at any of the locations in the 15 county Winston Salem region Photos taken at the various facility sites indicate differently showing incised areas around outfalls from high velocity discharges and at two locations photos show where concrete slurry has made it to water of the states which is a violation of the Clean Water Act and could be cause for a Notice of Violations and assessment of associated fines This office recommends that Stormwater/Wastewater Permit training be implemented for all of the Chandler Concrete facilities so that possibility of non-compliance be nummalized Also, please understand that these facilities will be monitored by NCDEQ Inspectors on a more frequent basis to ensure compliance Should you have any questions regarding these matters, please contact Sue White at (336) 776- 9661 Sincerely, Matthew E Gantt, P E Regional Engineer Land Quality Section Winston-Salem Regional Office cc DEMLR - Stormwater Pollution Prevention Plan (SPPP) As of Friday, June 10", we had developed, implemented and trained the site manager on the minimum requirements of the SPPP, which included: 1. Site Plan 2. Stormwater Management Plan 3. Spill Prevention Ft Response Plan 4_ Preventative Maintenance and Housekeeping Program 5, Employee Training 6. Responsible Party 7. Plan Amendment 8. Response to Directed Plan Changes 9. Facility Inspections 10. Implementation Additionally we reviewed: • Emergency Contact list • Emergency response protocols • Stormwater Permit Requirements • Routine inspections - significant rain events, quarterly It annual housekeeping • Precipitation monitoring - historical data was populated from local resource. Site precipitation data will be maintained using the installed rain gauge (6/7/16). • Erosion Control inspections - which will be performed following the next rainfall event and each rain event going forward. • Erosion Control maintenance - will be performed and documented as identified during the Erosion Control Inspections. • And other plan elements. POST OFFICE BOX 131 • BURL INGTON, NORTH CAROLINA 27216-0131 TELEPHONE: 336.226.1181 / Fax: 336.570.0557 Following the SPPP review we completed: • QHIR - Quarterly Housekeeping Inspection Report for the 2"" quarter. • IDC - Individual Demonstration of Competency for pH meter calibration and use. • Training on stormwater sampling techniques, how to obtain samples, pH testing, completion of the COC and packaging for lab analysis. In response to the erosion control maintenance records: • Erosion control records will be maintained onsite going forward. • 1 assisted the plant manager in clearing basins and baffles, noted on 6/ 10/ 16 records. • Additional silt fencing and stone were installed on adjacent hillside to reduce basin loading, noted on 6 / 13 / 16. In response to the skimmer basin not functioning: • Upon closer inspection, it was found that the skimmer was properly functioning. A storm event prior to the inspection had washed/blown a section of corrugated PVC piping from the adjacent jobsite and went into the basin. The piping rested alongside the existing pipe giving the appearance that it had broken loose. This finding does affirm your statement regarding routine maintenance. This should have been cleaned or removed from the pond previously. 2. Qualitative Monitoring In accordance with our permit, we will perform the Qualitative and Analytical monitoring at the time of the next qualifying discharge, weather and conditions permitting. These records will be maintained and available for review. Should a qualifying sample not be obtainable prior to June 30" we will perform qualitative and analytical monitoring at least twice during the second half of 2016, as we are able. 3. Analytical Monitoring: As stated above. Sue, I am personally and professionally embarrassed that I failed to complete these required elements after our previous discussion. Site management has now been instructed and provided the tools to maintain compliance with the site's stormwater permit. We do take our compliance requirements seriously and I apologize on behalf of myself and Chandler Concrete for our failure to uphold that standard. I would like to meet you at the site to show you what we have done and get your input on any additional measures we may implement to improve the site and water quality.
