benchmarks and provisions of Part IV Section A including the Tiered Response Action
'Flow rate can be measured continuously or calculated Flow limits for wastewater discharges to HQW waters shall beset to 50% of the Summer 7Q10 Flow as per 15A NCAC 02B 0224 Permittees
who discharge wastewater to HQW waters shall obtain a Summer 70.10 flow and report this information to DEMLR If the permittee cannot obtain a Summer 7Q10 flow for the receiving waters at the
discharge location the permittee shall notify DEMLR and the DEMLR Regional Office may require an annual flow report on a case by case basis

STORMWATER POLLUTION PREVENTION PLAN
(SPPP)
I SITE INFORMATION
A Site Location This site is located in the City Limits of Greensboro on the West side of
town The address is 114 North Chimney Rock Road This property is located in an
industrial section Trees surround the property on the North and East Beyond the trees is
an industrial area Adjoining the property on the South is an industrial business Chimney
Rock Road is to the West and across the street is a nightclub Water discharged from this
site will enter Horse -pen Creek, a class WS41, NSW water body in the Cape Fear River
Basin
B Site Plan A copy of the site plan is located behind tab marked Plan/Maps
C General Location Map A copy of a portion of the USGS map identifying the
surrounding area can be found behind the Plan/Maps tab
D Narrative Description of Storage Practices The sand and stone are delivered by a
contract hauler and stored in silos and on concrete slabs with tall concrete walls These
materials are sprinkled in order to maintain consistent moisture to prevent fugitive dust
The cement and fly ash are stored in closed silos The cement and the fly ash are
delivered via company owned tanker trucks This material is blown into the silos by
means of pneumatic pressure supplied by the tanker truck Potential dusting is controlled
by dust collectors The chemical admixtures are stored in tanks located in a contained
area There are no fuel tanks on the property All vehicles are fueled off -site The mixer
trucks are loaded under the cement silo with dust collection system controlling any
potential dusting The majority of the traffic area is paved and is cleaned on a regular
basis to help control any fugitive dust and sediment entering the stormwater discharge
The remaining traffic area is treated to control dusting during dry weather At the
completion of deliveries the mixers are rinsed off into a series of settling basins These
basins are cleaned out on an as needed basis and stockpiled for drying to be hauled off site
and used as fill material There is one commingled storm/process water discharge outfall
at this location A second stormwater outfall was added to address potential stormwater
discharges emanating from the East side driveway loop and aggregate storage areas
Discharges from these outfalls will be monitored for compliance in accordance with
permit and discharge requirements

E List of Significant Spills There have been no significant spills reported at this location
up through the date of this revision Should a leak or spill occur it will be duly noted on
the proper form located behind tab labeled Spills
F Certification of Stormwater Outfalls There are now two recognized outfalls for this
site
OF1 At this outfall the stormwater comingles with process water This outfall is
monitored according to the permit specifications
OF2 is identified as a stormwater outfall, maps and permit modifications acre to be
submitted on/before August 5, 2016 to NCDENR to incorporate these changes
From the time of installation (February 2016) to the submittal period, there lhav been no
observation or evidence of discharge
II. STORMWATER MANAGEMENT PLAN
A Feasibility Study This plant does not operate on a regular basis and his no assigned
personnel It is used as a back-up facility for the other two Greensboro locations
Routine inspections/observations are performed on a weekly basis to verify security,
safety and environmental concerns
Operational 1 Management practices have been adapted to minimize the potential for
stormwater / process water discharges and/or discharges that could potentially exceed
permit limits These are further described below in section C Best Mgmt Practices
This site plan has been modified in order to better control the discharge of water Even at
this expense we were not able to separate the storm and process water at OF due to the
layout of the plant and accompanying buildings OF2 was added to reduce the potential
for overloading of OF1
This retrofit was designed to eliminate possible contamination of storm water as much as
practicable within economic constraints Other areas that have been modified are the
truck wash area along with additional settling basins and concrete paving to direct this
process water into the basins
The admixture tanks are located inside secondary containment The aggregate storage
area is contained on three sides to prevent spillage of material One side was left open in
order to receive and to have excess to the material This aggregate is transferred to the
in -plant storage bins via a front-end loader and conveyor system Cement and fly ash is
being unloaded, stored, and discharged via'a closed system
Care is taken to separate the process water from as much storm water as possible by the
addition of swales and berms This process water goes through a series of settling basins
in order to settle out sediment and a CO2 treatment is used to lower the pH Some of this
water is used for truck washdown and/or reused at the plant for dust control and truck
drum washing

B Secondary Containment Schedule The secondary containment vessel for the liquid
admixtures is presently in place These vessels are equipped with a manually control
dram valve that is secured with a locking mechanism When this area is cleaned it is
recorded on the Secondary Containment Release Report (SCRR) form found behmd the
same named tab Prior to release the water will be visually observed and noted on the
"Secondary Containment Release Report" for color, foam, stain, visible sheen, etc If
uncontaminated it will be released If contaminated it will then be properly contained
and disposed the truck wash liquid is labeled biodegradable and phosphate free
solution, with a pH adjusted to within 6 to 9 standard units All other possible
contaminates are stored inside and not exposed to storm water Records of the releases
from the containment vessel are kept at this site for a period of five years
C Best Management Practices Summary (BMP) Best management practices are
physical, structural or management practices that prevent or reduce pollution of waters of
the state The following is a descrtption of the BMP and storm water controls that are in
place at the Chimney Rock Road facility
a) All plant personnel are assigned specific responsibilities relating to duties in
maintaining the plant site in accordance with permit requirements
b) Process water is channeled from the plant and truck wash areas into a series of
settling basins to allow sediments to settle out This water is being used for truck
washing and to control the dust on the haul roads and other traffic areas
c) Drivers must report any leaks of fuel, hydraulic or other fluids to the plant
manager and fleet maintenance for proper repairs Spills are to be cleaned up
immediately and properly disposed of
d) Rinsing out of the truck mixer drums is kept to a minimum and is to be performed
primarily at the Swing Rd plant or other assigned location When necessary at the
end of the day a maximum of 100 gallons of recycled water is used This water is
discharged into the settling basins
e) A log is maintained in the plant manager's office for inspections, service and
maintenance performed on the dust control system
f) There is very little water that drains from the stockpiles and silos Since this is
clean water that is used to keep the aggregate moist and it flows over washed stone
there should be little if any concern for contamination In fact a lot of this water
will evaporate before it has the potential to leave the site
g) The settling basins are cleaned on a regular scheduled basis The final basin is
maintained so that there is at least a 12-inch freeboard to capture a regular rain
event
h) The pH controller is maintained on a regular basis
i) Sediment removed from the basins is stockpiled so that any drainage will flow
back into the basins After the sediment dries it will be disposed offsite in a proper
manner
j) A rain gauge is available at this facility However, due to the reduced frequency of
operations, precipitation levels will be maintained and available at our Greensboro
Swing Road facility

III SPILL PREVENTION AND RESPONSE PLAN (SPRP)
A Potential Pollutant Sources This site does not meet the EPA requirements for a Spill
Prevention Control and Countermeasure Plan (SPCC) Plan The actions to be taken for spill
prevention and response are based on a proper assessment of potential pollutant sources of the
materials in inventory Liquid admixtures, cement, and fly ash and the mixer trucks pose the
greatest potential for a large spill either by rupture of one of the tanks, a severed supply line,
or by overfilling Truck wash detergent is Mortarman CR650 and Safe Soap II, both
biodegradable and phosphate free There are no polymers, brighteners or solvents used on
site Plant personnel have been trained on the proper way to contain any spill and properly
dispose of this material

such to their supervisor
IV. Preventative Maintenance and Good Housekeeping
A Storm Water Outfalls There is only one point source storm/process water outfall at this
site It is located in the northwest corner of the property next to the tree line and is so
indicated on the site plan Water from this outfall will flow into Horse -pen Creek, a class
WS-III, NSW water body in the Cape Fear River Basin
B Preventative Maintenance An effective Preventative Maintenance Program (PM) is an
important aspect of any BMP control system Well -maintained equipment is less likely to
cause problems that result in accidents and spills the Chimney Rock Road Plant has a
preventative maintenance program that satisfies the overall needs of the SPPP The
purpose of the PM is to locate and correct equipment problems before they occur or cause
major problems PM will reduce truck and plant breakdowns in addition to reducing the
risk of pollution of storm water
C Good Housekeeping Proper housekeeping is maintaining an orderly work environment
in order to contribute to the overall facility pollution control effort Good housekeeping is
achieved through a combination of periodic employee training, motivation, and significant
management to the details of making sure storage and work areas are clean and orderly

Good housekeeping contributes to prevention of accidents, but also supports worker
health and safety programs, eliminates waste and generally prevents deterioration of
company property and equipment The Chimney Rock Road facility shall adopt a
standard of good housekeeping in all of its work and storage areas The following steps
will help minimize any potential for storm water pollution in addition to promoting safety
and a sense of well being at work
a) All basins and drains are to be checked daily and cleaned on a regular scheduled basis
b) The chemical admixture containment basin will be checked daily and all trash and
debris will be removed In the event of a rain/snow or spill from any of the tanks the
residual liquid will be analyzed and disposed of in a proper manner
c) All material storage areas will be kept in a manner to prevent this material from
entering the drainage system
d) All paved areas will be cleaned in order to prevent tracking on to public streets and
entering the drainage system All unpaved haul roads will be sprinkledor treated
during dry periods to prevent dusting
e) The settling basins will be maintained so as to reduce the chances of any overflow
The objective is to maintain a minimum of at least a 12 inch freeboard in the last clear
water basin at all times in order to maintain a regular rain event Solid material
removed from these basins will be stockpiled next to the basins so that any water will
flow back into the basms This material will be removed from the plant site regularly
to an approved location
fj The forms used for casting barrier block will be treated with a biodegradable from
release This material is to be kept inside the storage building when not in use
g) Truck wash detergent is biodegradable and has an adjusted pH between 6 and 9
h) Trash and other waste will be picked up regularly A covered trash receptacle will be
strategically located inside and outside and emptied on a regular basis
i) All liquid containers that could be exposed to the weather will be kept in a secondary
containment vessel or inside the shop or storage building
r) All containers stored inside will be placed on pallets or shelves to prevent corrosion or
deterioration from exposure to moisture on the floor
k) Empty drums and used pallets will be properly disposed
1) All inspections must be documented with prmted name, date and signature of the
individual doing the inspection and maintained in the SPPP
m) The Good Housekeeping Checklist will be completed as part of the Quarterly
Housekeeping Inspection Report and retained in this book behind tab labeled QHIR
V. Em to ee Training
Training of employees is essential to the success of any program The following training will
be conducted
A Initial and annual training for all employees will include
a) An introduction to Chandler Concrete Company s SPPP, the Stormwater Discharge
permit, and what their responsibilities are
b) Spill response and cleanup procedures

this manual behind the Training tab

Ultimately every employee assigned to this plant has a responsibility to maintain this site in
accordance to the Storm Water Permit The Environmental Manager shall be responsible for
the overall coordination, development, implementation and revision if this plan
The Plant Manager and Production Manager are responsible for maintaining the proper
operational procedures according to this Plan and the DENR Permit NCG 140083 They are
responsible for sampling, reporting, and maintaining all necessary forms according to this
Plan and Permit They also will assign special housekeeping and preventative maintenance
tasks to the employees that are assigned to this plant Employees are instructed to follow the

VII Plan Amendment
his plan will be amended whenever there is a change in design, construction, operation, or
maintenance that has significant effect on the potential for the discharge of pollutants to
surface waters At a minimum the plan will be reviewed and updated on an annual basis
VII Facility inspections
Inspections of the facility and all stormwater systems shall occur as part of the Preventative
Maintenance and Good Housekeeping Program at a minimum on a semi-annual schedule
according the permit requirements Any inspections shall have at least 30 daysl separating
inspection dates unless inspections are required more frequently by DWQ as part of the
Tiered response
VIM Facility Inspection Program
The plant manager or his designee will conduct visual inspections and monitoring of the
outfall and other areas as per the permit requirements The forms can be found behind the
tabs as indicated below After the forms have been completed they are to be filed behind the
properly marked tab and kept for at least five years
A Stormwater Discharge Outfall Monitoring Form (SDOMR) Phis is the form that is to
be used when you actually take a sample of the storm water This is required to be done
two times a year (January — June and July — December) Once the sample is collected it is
sent to the designated lab to be analyzed along with the proper Chain -of -Custody form
Upon completion of initial data the SDO form is to be sent to the Environmental Manager
(EM) to be completed When the data is returned from the lab the EM will then submit to
the State and also send a copy back to the plant to be filed in this book
B Process Wastewater Monitoring Report This form (PWMR) is used for a discharge of
process water regardless of a storm event This sample is required to be done quarterly
(Jan -Mar, Apr -Jun, Jul -Sept, and Oct -Dec) Once the sample is collected it is sent to the
designated lab to be analyzed along with the proper Chain -of -Custody form Upon
completion of initial data the SDO form is to be sent to the Environmental Manager (EM)
to be completed When the data is returned from the lab the EM will then submit to the
State and also send a copy back to the plant to be filed in this book
C Stormwater Discharge Outfall Qualitative Monitoring Report (SDO) Phis form is to
be completed out when the SDO is performed semi-annually You do not need to send
this form to the EM or the State it is to be filed in this book in the proper location
D Quarterly Housekeeping Inspection Report (QHIR) Phis form is primarily a good
housekeeping checklist that is to be completed during the quarterly inspection and kept in
this book This form does not need to be sent to anyone unless requested

E Secondary Containment Release Report (SCRR) This form is to be completed every
time the water is released from the secondary containment vessel
F Spills and Releases Anytime there is a significant spill or release it will be documented
