On November 29, 2018 this facility was inspected for compliance. I met with Steve Barnes on site. During the inspection, the
following items were noted:
1) The last Analytical result from Outfall 1 was showing a high PH. The permittee is directed to evaluate the PH treatment
system on site and confirm that it is functional.
2) Due to the combined storage volume, the cleaner storage area observed of site should be in secondary containment. Mr.
Barnes said that the largest of the tanks, and several of the smaller drums would be hauled off soon.
3) There was some confusion about the sampling location for outfall 2. The permittee is directed to contact the individual that
does the sampling and confirm the sampling method and location.

Inspection Summary:
The Multi -Media team met with Johnnie Alexander, Environmental Manager, to conduct the inspection. The SPPP was
up-to-date and monitoring is being conducted as required. We discussed the need for secondary containment at the facility
due to the liquid bulk storage containers exceeding 1320 gallons. Johnnie indicated that the collection pit in the front could
-be used -for secondary -containment.. This collection_ pit -can -not be used as a secondary containm_e_nt for the liquid storage
_ tanks on site. The facility needs to review the options for secondary containment.
During review of sampling records. We noticed that TSS levels were exceeded on 4/18/15,-sample from Qutfall 001. We
discussed the exceedance with Johnnie. He indicated that the facility had reviewed what the issue was and had taken care .
of the issue.

On this date Starr Silvis and Roy Davis met with Henry Batten and Steve Barnes to discuss a project which
would provide adequate management of the stormwater and truck washwater across Withrow Road from
the office. Henry will send the ARO a letter proposing a time schedule as a alternate to the schedule
contained in our NOV. Additionally, Tri-City will temporarily improve upon the basin across the street, start
using captured truck washwater to wash drums and to the extent possible wash more trucks on the plant
side of the road, again using that water to wash drums.

In accordance with your application for a discharge permit received on June 10, 2008 and
additional information received August 11, 2008, we are forwarding herewith the subject
certificate of coverage (COC) to discharge under the subject state — NPDES general permit. This
permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and
the Memorandum of Agreement between North Carolina and the US Environmental Protection
Agency dated October 15, 2007 (or as subsequently amended).
Please note that this COC does not allow discharge from the irrigation of raw
material stockpiles, which is defined as a wastewater, from this site. This stipulation is a
result of the information presented in the application and gathered during technical review of
your request for an authorization to construct wastewater treatment system(s) at this plant.
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act or any other
federal or local governmental permit that may be required.
An Authorization to Construct treatment facilities has been issued concurrently with
this COC. The Asheville Regional Offir_e tele.nhnnP ntimhPr (SORT 7Qt;_4SQQ shall be notified
at least forty-eight (48) hours in advance of operation of the installed facilities so that an in -place
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.

Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the normitted
facility has been installed in accordance with the NPDES Permit, the associated
Authorization to Construct, and the approved plans and specifications. Please mail the
certification (attached) to the Stormwater Permitting Unit, 1617 Mail Service Center, Raleigh;
NC 27699-1617.
Please note that any future construction, installation, or modification of wastewater
treatment facilities (including process wastewater recycle systems) will require an Authorization
to Construct (ATC) prior to construction per 15A NCAC 21-1.0138 & .0139. You must submit,
in triplicate, plans/specifications and design calculations, stamped and sealed by a professional
engineer, with a letter requesting an ATC to the Stormwater Permitting Unit, 1617 Mail Service
Center, Raleigh, NC 27699-1617.]
One (1) set of approved plans and specifications is being forwarded to you. The
Permittee shall maintain a copy of the approved plans and specifications on file for the life of the
