In accordance with your application for discharge permit received on March 4, 2003, we are forwarding herewith
the subject certificate of coverage to discharge under the subject state - NPDES general permit. This permit is issued
pursuant to the requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between
North Carolina and the US Environmental Protection agency dated December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you,
you have the right to request an individual permit by submitting an individual permit application. Unless such demand is
made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of Water
Quality. The Division of Water Quality may require modification or revocation and reissuance of the certificate of
coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the Division
of Water Quality or permits required by the Division of Land Resources, Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.

Your facility holds a General Permit from the North Carolina Division of Water Quality to
discharge stormwater associated with industrial activities performed at your facility. Please be
advised that the Winston-Salem Regional Office will be performing NPDES Stormwater
Inspections sometime in the near future.
If this office has not previously inspected your facility, you should be prepared to demonstrate
compliance with all terms and conditions included in the Permit. Specifically, we will be
evaluating the following: Stormwater Pollution Prevention Plan, Stormwater Outfall Locations,
Qualitative and Analytical Monitoring Data and any other activities required by your permit.

SUBJ: Compliance Storm Water Evaluation Inspection
Dear Mr. Warraich:
On January 14, 2003 the U.S. Environmental Protection Agency (EPA) and the North
Carolina Department of Environment and Natural Resources (NCDENR), Winston-Salem District
Office performed a Compliance Storm Water Evaluation Inspection (CSWEI) of Stamper and
Haynes Salvage (Facility). EPA's participation in this inspection was to evaluate the Facility's
compliance with the treatment and disposal of storm water in accordance with the requirements of
Section 402(p) of the Clean Water Act (CWA), 33 U.S.C. § 1342(p), the regulations promulgated
thereunder at 40 Code of Federal Regulations (C.F.R.) Part 122,26, and the NCDENR General
Permit to Discharge Stormwater Under the National Pollutant Discharge Elimination System
(NPDES), General Permit No. NCG100000 (the Permit) issued in September 2002. The results
of EPA's inspection are summarized in the enclosed report.
During the inspection it was discovered that your facility had not submitted a Notice of
Intent (N01) for coverage under the Permit. A copy of the NOI and the Permit were provided to
your representative during the inspection. EPA requested that you submit the NOI to the
NCDENR not later than February 17, 2003, with a copy to EPA. If you have not submitted the
NOI, please do so as soon as possible and send a copy to EPA at the address on the letterhead to
avoid enforcement action. Enforcement action could result in the issuance of an Administrative
Order and/or the imposition of civil penalties of up to $27,500 per day. If you determine that the
storm water leaving your property does not enter a receiving stream of a Municipal Separate
Storm Water Sewer System, then you should notify the NCDENR and EPA.
Also enclosed is a document entitled "U.S. EPA Small Business Resources -Information
Sheet" for your use and to assist you in understanding the compliance assistance resources and
tools available to you. However, any decision to seek compliance assistance at this time does not
relieve you of your obligation to EPA, does not create any new rights or defenses, and will not
affect EPA's decision to pursue enforcement action. In addition, the Securities and Exchange

Introduction
On January 14, 2003, personnel from the United States Environmental Protection Agency (EPA),
Region 4 and personnel from the North Carolina Department of Environment and Natural
Resources, Division of Water Quality, Winston-Salem Regional Office, conducted a Compliance
Storm Water Evaluation Inspection (CSWEI) at Triad Auto Salvage located at 7224 Old
Lexington Road, Winston-Salem, North Carolina. .The purpose of the CSWEI was to evaluate the
facility's treatment and disposal of storm water in accordance with 40 Code of Federal
Regulations (C.F.R.) Part 126 and the State of North Carolina's General Permit No. NCG100000
to Discharge Stormwater Under the National Pollutant Discharge Elimination System (the Permit)
covering storm water discharges from'establishments primarily engaged in activities classified as
Used Motor Vehicle Parts (Standard Industrial Classification Code [SIC] 5015) and Automobile
Wrecking for Scrap (a portion of SIC Code 5093). Section 402(p) of the Clean Water Act
(CWA) states that storm water discharges associated with industrial activities to waters of the
United States or into a Municipal Separate Storm Water Sewer System (MS4) must be authorized
by a National Pollutant Discharge Elimination System (NPDES) permit; in this case the North
Carolina Permit.

General Permit No. NCG100000 to Discharge Stormwater Under the National Pollutant
Discharge Elimination System for establishments primarily engaged in activities classified as
Used Motor Vehicle Parts (Standard Industrial Classification [SIC] 5015) and Automobile
Wrecking for Scrap (a portion of SIC 5093).
Permit Requirements: A permit is required for all storm water discharges to'waters of the
United States whether to a receiving stream or into a MS4. General Permit
No. NCG100000 authorizes the discharge of storm water only. An .
individual NPDES permit is required if waters are co -mingled with process
wastewater.
Triad Auto Salvage is covered under Standard Industrial Classification (SIC) Code 7538, general
automotive repair and SIC Code 5015, dismantling or wrecking used motor vehicles for parts
recycling or resale and for scrap. A review of this site indicates that storm water is discharged
from the property and into a drainage area at the rear of the property which drains into an
unnamed creek. It was determined that this facility should seek coverage under General NPDES
Permit No:' NCG10000.'- A package was left at the site which'included a copy of the NOI and the
Permit.

Liquid Storage in Above Ground Storage Tanks; Illicit Connection to
Storm Sewer -
This facility consists of a dirt and gravel yard covering approximately 10 acres; there are
approximately 700-800 vehicles currently at this site. The owner purchased this business about
one year ago and plans many improvements at the site.
Incoming vehicles are placed in the yard without having fluids removed; fluid removal is
performed, if necessary,. when parts are removed. Cars are stored containing all parts; many of the
vehicles were stored with hoods up. Parts are pulled by an employee upon customer request. Gas
tanks are not removed from stored vehicles. Freon is not captured and recycled.
Fluids are stored in separate used oil/anti-freeze tanks. However, the tanks were not labeled and
were not under cover. In addition, the tanks were not stored on a concrete or elevated pad.
Batteries are stored on the ground and not under cover or on a raised platform. There are several
barrels throughout the site that had no labels. These barrels should be labeled and disposed or
properly. Empty barrels or drums should be stored on their side or disposed of properly. Metal
for a planned warehouse is stored in the open. Also, around the site there are several areas with
assorted metal and car parts that are not stored under cover.
Storm water discharges to the rear of the property and into a drainage area. The drainage area
discharges into an unnamed tributary.
Conclusions and Recommendations
Due to the nature of the storm water discharge, a NOI should be submitted to the NCDENR not
later than February 26, 2003, with a copy to EPA. The facility must develop a Pollution
Prevention Plan within the first year of the permit and perform monitoring. If the facility
determines that permit coverage is not necessary because the drainage area does not discharge
into a stream, then notification should be made to both NCDENR and EPA. It is recommended -
that best management practices be implemented immediately; these include the labeling of all
drums and barrels, spent oil and antifreeze drums should be kept undercover and on a concrete or

elevated pad, and batteries should be stored off the ground and under cover. Once the crusher is
put into service, the facility should ensure that all fluids are drained from the vehicles prior to
crushing and disposed of in a proper manner.
