reissued pursuant to the requirements of North Carolina General Statute 143-215 1 and the Memorandum of
Agreement between the state of North Carolina and the U S Environmental Protection Agency, dated October
15, 2007 (or as subsequently amended)
The following Information is included with your permit package

• Two copies of the Discharge Monitoring Report (DMR) Form
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater, and it specifies your obligations for discharge
controls, management, monitoring, and record keeping Please review the new permit to familiarize yourself
with all the changes in the reissued permit Your facility has six (6) months from the time of receipt of the
permit to update your current SPPP to reflect all new permit requirements
The first sample period of your permit begins January 1, 2013 Your facility must sample a "measureable
storm event' beginning during the periods beginning January 1 and July 1 of every year (or, if applicable,
report "No Flow," as outlined in Part III Section E) Also, please note that Tier 3 Actions in Part II of your
permit are triggered by benchmark exceedances on four occasions beginning on the effective date of this
permit and do not count prior exceedances

Inspection Summary
DWQ staff was met on site by Blake Kehoe and Mike Puttick A tour was conducted of the exterior of the facility,
observing the stormwater conveyances and outfalls and site conditions During the visit the SPPP manual was reviewed
operational activities and existing stormwater exposures were discussed Permit requirements were also reviewed as
pertaining to the most recent permit version dated November 1 2007 At the time of the inspection the site was found to
be in compliance with the permit The inspector noted that the facility appears to maintain good housekeeping practices
and records were found to be in good order
The two streams located within the southeast portion of the property were inspected A variety of algae were noted in both
reaches The stream located adjacent to the wastewater treatment facility was also heavily populated by algae Further
investigation into the origin of the algae was considered warranted DWQ will be visiting the site to collect algae and
stream samples

BACKGROUND
On Match 19, 2009, Clement Pappas contacted Altamont about a wallet seep located near the
southeastern cornet of the building and adjacent to stounwater outfall SDO-0001 The water seeping out
of the ground contained a strong odor and had a white, milky color In compai ison, the water discharging
fi am neai by outfall SDO-000I %sas cleat and flowing at a constant i ate of approximately one gallon per
minute (gpin)
SUMMARY OF FINDINGS
On Match 19, 2009 Altamont collected a grab seep sample to determine if fecal eolifoim at
Ti ihalomcthanes (TI IM) weie present in the watet seep The seep sample was collected in laboiatoiy
supplied sample containers and placed in a cooler on ice to await delivery The sample was analyzed fot
fecal colifoirn by CPA Method MBIC 92222D and THM by EPA Method 8260 by Pace Analytical
Set vices, Inc (Pace) in Asheville, North Carolina
Dui ing the THM analyses, Pace observed the presence of three additional volatile organic compounds
acetone, 2-butanone (MEK), and methyl-tert butyl ether (MTBE) Pace contacted Altamont and

additional results ate summarized on the attached table The laboratory analytical report and chain of
custody for m are also attached
According to the attached table and analytical report, the results of the seep sample had no detections of
fecal colifotm, TI-IMs, or tepoited volatile organic compound% above the North Carolina Administrative
Code, Title 15A, Subchapter 2L Standards (21, Standard) for groundwater Based on the fecal colifoim
and THNI results, the watet seep is likely not associated with sanitary wastewater of ehloitnated di inking
water dischaignng from the site The low detection of Methyl -tent -butyl ether (MTBE) is often associated
with incomplete fuel combustion in automobile exhaust and could be associated with the pioxrtnity of the
seep to the facility shipping and receiving aica MEK and acetone aie often associated with the type of
cleaner and glue that aie used for the assembly of polyvinyl chloride (PVC) pipe Additionally, acetone
is a common laboratory artifact, which may have been caused by inadvertent contamination dut ing
sample handling at the lab

Division is granting your certification as provided for under 40 CFR 126 22(g) which is incorporated by
reference in North Carolina regulations
Please note that by our acceptance of your no exposure certification, you are obligated to maintain no
exposure conditions at your facility If conditions change such that your facility can no longer qualify for a no -
exposure exclusion you are obligated to immediately obtain NPDES permit coverage for your stormwater

NPDES Storm Water Permitting
Submission of this No Exposure Certification constitutes notice that the entity identified in Section A does not require permit authorization for its storm water
discharges associated with industrial activity in the State identified in Section B under EPAs Storm Water Multi -Sector General Permit due to the existence
of a condition of no exposure
A condition of no exposure exists at an industrial facility when all industrial materials and activities are protected by a storm resistant shelter to prevent
exposure to rain snow snowmelt and/or runoff Industrial materials or activities include but are not limited to material handling equipment or activities
industrial machinery raw materials intermediate products by products final products or waste products Material handling activities include the storage
loading and unloading transportation or conveyance of any raw material intermediate product final product or waste product A storm resistant shelter is
not required for the following industrial materials and activities
- drums barrels tanks and similar containers that are tightly sealed provided those containers are not deteriorated and do not leak Sealed"
means banded or otherwise secured and without operational taps or valves
- adequately maintained vehicles used in material handling and
- final products other than products that would be mobilized in storm water discharges (e g rock salt)
A No Exposure Certification must be provided for each facility qualifying for the no exposure exclusion In addition the exclusion from NPDES permitting is
available on a facility wide basis only not for individual outfalls If any industrial activities or materials are or will be exposed to precipitation the facility is
not eligible for the no exposure exclusion
By signing and submitting this No Exposure Certification form the entity in Section A is certifying that a condition of no exposure exists at its facility or site
and is obligated to comply with the terms and conditions of 40 CFR 122 26(g)
ALL INFORMATION MUST BE PROVIDED ON THIS FORM
