familiarize yourself with all changes in the reissued pcnnit. Significant changes to the General Permit are
outlined in the attached Technical Bulletin.
Your coverage under the General Pennit is transferable only through the specific action of DWQ.

Summary
This facility submitted a Notice of Intent to be covered under General Permit NCG050000 (ind..
Rubber and Miscellaneous Products). The stormwater discharge drains to an Unnamed Tributary to
the West Fork of the Deep River (Class WS-IV), in the Cape Fear River Basin. This facility is
located in Guilford County, handled by the Winston-Salem Regional Office.
Application review
a) The permit application has been signed and sufficiently completed. Yes.
b) The SIC code accurately describes this facility's activities and is covered by this general
permit. Yes. SIC: 3084 (Plastics Pipe)
Facility produces PVC pipe from blended PVC compound
c) Does this facility have other permits? Yes. NC0036366.
d) Hazardous Waste Activities? Yes. Facility is a small quantity generator and stores
material inside covered warehouse. See information below.
e) Regional Office input requested: 12/14/04.
RO Approval received: 12/14/04 (via e-mail from Corey Basinger.)
Site applied for coverage on Corey's recommendation after a site inspection several
weeks ago.
g) Application approved? Yes. COC drafted 12/14/04.
Phone logs
12/14/2004: Followed -up with contact Qeff Cortez, 336-996-2711, ext. 13) regarding haz. waste
storage and MAN. The facility stores all waste (maybe 4 drums or so) inside the warehouse against
the back wall, adjacent to the manufacturing area and hydraulic fluid storage (the aisle is approx. 8' x
50'). The material is mainly used oil, parts cleaner, used ink/solvents, and oil adsorbent. There ate
no drains in the building —if material spills, it is cleaned up immediately using adsorbents, etc.
There have been no spills or releases in the past few years, and no NOVs associated with the HW.
To prevent stormwater contamination, the facility currently implements BMPs such as storing used
equipment in covered areas and brushing/cleaning off used chemical pallets (facility uses a lot of
waxes) before storing outside.
