The Systerns Logistics-Vertique ("Systems Logistics") facility located in Arden, North Carolina is
submitting the following information to the Division of Water Quality ("Division") in regards to the
facility general industrial permit NCG030636. Please note, Systems Logistics was formerly named
1TW Warehouse Automation.
The current monitoring period is recognized as July 1, 2018 through December 31, 2018. The sample
analysis attached was collected from outfall 002. The results were reported within 30 days of the date
of collection. For 2018, facility personnel are scheduled to be trained to the SPPP and best
management practices. Also, specific personnel were identified to properly collect samples and
report out as needed.
Attached is the discharge monitoring report required for monitoring period year 1-period 2. The
sampling requirements per Section D of the General Permit are below:
a. September 11, 2018/Systems Logistics location outfall 002/1.30 pm

The benchmark level for pollutants has not been exceeded this monitoring period. A pH meter will
be purchased and kept onsite for testing the storm water once it is collected to verify the result is in
compliance with permit NCG030636.
Appalachian Environmental Resources, Inc. has been contracted to handle the water quality program
at the facility. The Storm water Pollution Prevention Plan (SPPP) has been developed and all
information pertaining to the NPDES permit for Metal Fabrication has been completed.

Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may so certify, and the requirement for TFO monitoring
may he waived. The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General
Permit. For those facilities electing to employ the Tf0 monitoring waiver, the discharger shall sign the following certification statement:
"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics
(TTO), I certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing the all the provisions of the
solvent management plan included in the Stormwater Pollution Prevention Plan."

The Systems Logistics-Vertique ("Systems Logistics") facility located in Arden, North Carolina is
submitting the following information to the Division of Water Quality ("Division") in regards to the
facility general industrial permit NCG030636. Please note, Systems Logistics was formerly named
ITW Warehouse Automation.
The current monitoring period is recognized as July 1, 2017 through December 31, 2017. In 2017,
facility personnel were trained to the SPPP and best management practices. Also, specific personnel
were identified to properly collect samples and report out as needed.
The sampling requirements per Section D of the General Permit are below (please note, there was no
discharge for monitoring period year 5-period 2):
