ADDITIONAL REGIONAL STAFF REVIEW ITEMS On November 4, 2019, WSRO staff conducted site visits to the proposed land application fields included in the application for permit renewal. The residuals generating facility was NOT visited. The following items require additional information to be requested: 1. Site RC-11 Field #2 has an area with unsuitable slopes present in the application area (see attached site map sketch). The site map needs to be revised to buffer this area from land application. Applicable forms will need revised acreages provided as result. 2. Site RC -I I Field #4 has two drainage ditches that are located within the application area (see attached site map sketch). The site map needs to be revised to show these two features and buffer these areas from land application. Applicable forms will need revised acreages provided as result. 3. Site RC-11 Field #7 has a cabin and a well adjacent to the field which is not shown on the site map (see attached site map sketch). The site map needs to be revised to show these two features and the applicable buffers. Applicable forms will need revised acreages provided as result. 4. Site RC-25 Field #8 has a well adjacent to the field which is not shown on the site map (see attached site map sketch). The site map needs to be revised to show the well and the applicable buffer. Applicable forms will need revised acreages provided as result. 5. Site RC-28 Field #1 has a drainage ditch that is located within the application area (see attached site map sketch). The site map needs to be revised to show this feature and buffer. Applicable forms will need revised acreages provided as result. 6. Site RC-45 Field #1B had no changes shown on the GIS produced site map. However, the application area has reportedly increased in size by 0.6 acres. Request an explanation be provided as to how the area increased with no changes to the application area site map.
