Below is some general information regarding our renewals. I have attached your permit contact information that our system currently has on file. If you'd like to be the owner affiliation contact, please fill out the owner affiliation form and mail back to me. See other information below and let me know if you have any questions. NCG03 RENEWAL INFORMATION: You may go to this website (https://deq.nc.gov/about/divisions/enerev-mineral-land-resources/enerjzv-mineral-land- permits/stormwater-program).and click on the "Check your Permit Contact Information" link. There, you can type in your Certificate of Coverage and review the PDF report of contacts. Unless the Owner Affiliation needs to be revised, you can simply send an email to the Stormwater Permitting Program staff (please send any necessary updates to Laura Alexander, who is copied on this e-mail). The Stormwater Permitting Program plans to send out letters to all permittees later this year to explain that there is nothing to do to request renewal, as long as all annual fees are paid up. There's no further action needed at this time to renew coverage under your permit. The standard conditions of the NCG030000 General Permit do refer to the permittee's obligation to submit an application to the Division no later than 180 days before expiration to assure compliance and continued coverage, but that is not the case for General Permits; it's language that really only applies to individual permits. Per NC Administrative Code (15A NCAC 02h .0127(e)) : "Dischargers covered by general permits need not submit new Notices of Intent or renewal requests unless so directed by the Division." Corrections to the standard conditions for General Permits will likely be made as those permits are renewed this year. Please keep an eye out for the permit renewal explanation letter. You can also check our website or contact me to check on the status of permit renewal. The Stormwater Permitting Program staff first have to revise the NCG030000 General Permit itself, send it to EPA for comments, and then put it out to Public Notice for 30 days. Once that's been completed, they will renew permittees covered under the permit. The answer to number 11 is in error. The BMP's for Stormwater Control include "animal liquids and blood drain to City Sanitary Sewer", not City Storm Sewer. The error will be corrected by Larry Graham and a new copy of the application will be sent as replacement. I visited this facility with the EPA and we walked the grounds and observed all outfak locations. There would have been evidence of blood, body fluids and insect activity if waste was draining to stormsewer. None of that exist. According to Bradley, `the site is one that EPA inspected earlier and they are working on an enforcement case with them". Also, in their NOI application, Randolph Packing said "animal liquids and blood go to the city storm drain". If you could have the inspector check on this please. First of all, this is one of the ones that EPA inspected earlier this year and they are actively working on an enforcement case with them. I have copies of the EPA letters if you want to look at them. Also, in the NOI I noticed that one area they noted drainage of some materials that we would consider wastewater to the municipal storm sewer. Maybe they just got that wrong and it was really going to the sanitary sewer, but we need to make sure of that. Results are shared with area management with corrective actions identified. RESULTS: overall satisfactory results obtained throughout the permit cycle. Areas of opportunity continue to be centered around storage of municipal solid waste dumpsters in parking lot without covers. Dumpster contents include scrap steel, plastic and wood. Summary of BMP's (three identified for the site): 1. Sediment and Erosion Control - majority of ground surface of the building and activity area including paved driveways, parking lots and concrete pad, Erosion is reduced on unpaved ground Surfaces by well -maintained grass or gravel cover. Z. inspection schedules -semi-annual at a minimum, once Q1/2 - second Q3/4 by qualified plant personnel. Tracking results to ensure appropriate corrective actions, maintaining records of inspections. Visual includes; parking, driveways, gates for signs of oil/grease, spills; areas around equipment; outdoor processing areas, material storage and handling areas; waste generation and storage areas; storm water management devices and spill kits. 3. Bulk fuel Loading/Unloading - an SPCC plan is in place and is implemented at the facility and activities meet the requirements of DOT 49 CFR 177 Subpart B. A loading/unloading checklist is completed each time and the record is maintained in the SPCC plan. The SPCC plan was rewritten in Q1 2017 as a Tier I document due to the fact that no spills occurred during the plan cycle. NCG030519 Significant changes in Industrial Activity Summary: 2.2 Site Plan - Initial plan includes a bulk propane tank which was removed from the site in 2016 and replaced with 12 433 lb. cylinders. 2.2 Site Plan - Sampling points for outfalls 001-003 were changed in Q3 2016 from off site collection which typically included standing water to last representative storm grate serving each outfall . This change in sampling also reduced the potential for injury and/or drowning to obtain sample. Change in sampling outfall location completed between NC-DEQ and Draka by consultant. 2.3 On site Storage and Industrial Activities - Initial plan does not include metal forming operations which were in place at permit issuance. 4.1 Oil Spill -site contact information changed to reflect current management responsibilities. Persons named are trained in the SW3P plan. Emergency response contractor agreement in place to address timely responses to spill volumes outside of personnel/equipment capability. 4.5 PVC pellet spill - updated site contact list to reflect current management responsibilities. 7.1 Monitoring Frequency - Exceedences of Cu and Zn benchmark values require the site to follow Tier III monitoring plan. Adverse weather is defined in the "Definitions" section of the permit. • Sections B, C. The term "Representative Storm Event" has been replaced by "Measurable Storm Event." A measurable storm event is defined in the permit. • Section D: If the permittee fails to respond effectively to problems identified by qualitative monitoring, DWQ may require the permittee to perform corrective action. Please review Parts III and IV to understand the Standard Conditions of your new NPDES General Permit, including Compliance and Liability, Reporting, Monitoring and Records requirements; Operation and Maintenance obligations; and Definitions. Please note that all samples analyzed in accordance with the terms of this permit must be submitted to the Division on Discharge Monitoring Report (DMR) forms available on the Stormwater Permitting Unit's website above.
