REGAN Secretary TRACY DAVIS Director On August 15, 2017 Glen White of the North Carolina Department of Environmental Quality — Department of Energy, Minerals & Land Resources met with you and James Frei (Consulting Environmental Engineer) of SwSG, LLC at the subject facility, located at 7537 Hwy 87 North, Reidsville, to conduct a compliance inspection for the facilities Industrial Stormwater Permit. Your assistance with inspection was greatly appreciated. Permit: This facility holds General Stormwater Permit NCG030034 to discharge stormwater from industrial activity classified as Heat Treating of Metal (SIC 3398), Fabricating of Metal Products (SIC 3398), Manufacturing of Measuring and Analyzing Instruments (SIC 38) under the National Pollutant Discharge Elimination System (NPDES). The permit became effective November 1, 2012 and expires on October 31, 2017. If a permit revision is not issued prior to the expiration date, the facility will continue to operate under the expired permit until a new permit is issued. A copy of the current permit is required to be maintained with the Stormwater Pollution Prevention Plan (SPPP) documents. A copy of the current permit was available for review at the time of inspection. Records/Reports: This facility was required to develop and maintain a Stormwater Pollution Prevention Plan (SPPP) in accordance with Part II, Section A of the permit. Inspector reviewed the SPPP and each of its components. The SPPP has been updated annually as required. A training workshop that includes emergency response training is conducted annually by Mr. Frei. All employees sign off on training log with pen and ink signatures. Nt_ 27103 336 776 9,300 Facili Site Review: This facility manufactures tank trucks and specialty trucks for a range of uses that vary from carrying drinking water to hauling various types of fuels including jet fuel. The facility has both private and government contract clients with.large multi -year contracts for military equipment. Vehicles and tanks are built to customer specifications and tested using the government and customer required methods. Renovation and expansion that was underway at this facility, during the 2015 inspection, appear to be complete. The facility has also added a covered area for metal stock. Rust Proofing and Sand Blasting are performed in metal building located away from the main plant. The storage yards are nearing full capacity with completed equipment waiting to be picked up by transport contracted by the customer. Site appears to be well maintained and everything is stored neatly. Production areas are all under roof. Effluent Rec_eivin_1 Waters: Effluent from this facility drains to from an unnamed tributary to Wolfe Island Creek, a part of the Roanoke River Basin. Self -Monitoring Program - Part II, Sections B & D of the permit require this facility to perform qualitative and analytical monitoring on a semi-annual basis unless a Tiered Response is triggered. The last monitoring samples were taken on August 10, 2017. This facility appears to be well maintained. The production area expansion appears to be complete. New covered production areas are complete and there is a new covered area for metal stock. All raw materials to be neatly stored in their respective place. The SPPP and siteplan will need to be revised to include all site renovations, additions, and current records once the revised/new general permit has been issued. Mr. James Frei of SwSG, LLC ad Mr. Mike Leffew of Isometrics Engineering, assisted the inspector with review of the SP3 (Stormwater Pollution Prevention Plan) documents and with site review. The facility was found to be compliant with its stormwater permit requirements. An inspection report is attached for your records and the inspection findings are summarized below. 1. Vehicles and tanks are built to customer specifications and tested using the government and customer required methods. This plant is currently undergoing upgrades and expansion. There have been building additions and changes to the location of storm outfalls. More additions are scheduled to begin soon. Much of the raw steel material is stored in the yard now and is exposed to the elements. New covered areas are to be constructed and raw materials will be stored under cover. The SP3 and siteplan for this site will need to be revised to show new facilities, storage and outfall locations. 4. Effluent/Receiving Waters Stormwater from this facility drains from an unnamed tributary to Wolfe Island Creek, a part of the Roanoke River Basin. S. Self -Monitoring Program Analytical Monitoring: This site is required to provide analytical monitoring under current conditions of the permit. Monitoring has been performed twice annually as required. Mr. Frei, SwSG, LLC, maintains the SP3 plan and conducts the required monitoring. Overall, the condition if the site is well maintained. The SP3 plan and monitoring records are currently compliant but need to be updated soon to include facility expansion and new outfall locations. This permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 1S, 2007 (or as subsequently amended). Additional conditions for specific industry sectors have been added to the SPPP requirements in some cases. • Sections B, C: Failure to perform analytical stormwater monitoring may result in the Division requiring that the permittee begin a monthly sampling scheme. • Sections 8, C: A lower TSS benchmark of SO mg/I for HQW, ORW, PNA and Tr Waters applies to these more sensitive waters. • Sections 8, C: The monitoring parameter Oil & Grease (O&G) has been replaced by the parameter Total Petroleum Hydrocarbons (TPH) for vehicle maintenance areas, and in some cases, other analytical monitoring requirements. • Sections B, C, D: Inability to sample due to adverse weather must be recorded in the SPPP, or in separate on -site records if your General Permit does not require an SPPP. A measurable storm event is defined in the permit. • Section D: If the permittee fails to respond effectively to problems identified by qualitative monitoring, DWQ may require the permittee to perform corrective action. The assistance and cooperation of Mr. Mike Leffew of Isometrics and James Frei of Stormwater Services Group, were greatly appreciated. Inspection findings for both facilities are summarized below and inspection reports are attached for your records. Isometrics, Inc. Plant #1 NCGO30033 Siaht_Review This facility is located at 1266 N. Scales St., in Reidsville, Rockingham County. Stormwater related to industrial activity is discharged from the site to an unnamed tributary to Wolf Island Creek, which is currently classified as Class C waters in the Roanoke River basin. The facility was clean and in good shape at the time of inspection. Many of the housekeeping issues noted in the previous inspection have been addressed. During the inspection, Mr. Thomas pointed out that the used oil drum still had no secondary protection. Mr. Leffew suggested relocating the tank to the former hazardous waste site that has existing secondary containment. Relocating the tank will bring the tank into compliance with permit conditions. Mr. Thomas also noted that there were two smaller tanks of diesel fuel that are used in pressure testing the tanks that do not have secondary containment. In addition, all above ground storage tanks at the facility are required to have secondary containment with manually controlled valves, which shall be secured closed with a locking mechanism. Documentation Review for Plant #1 and Plant #2 Both facilities have nearly identical Stormwater Pollution Prevention Plans and are well implemented. Mr. Thomas did observe that there were some minor deficiencies in tracking training events and updating the SP3. There is a greater variety of exposed industrial activity occurring within its drainage area. b. Scrap debris is often stored within its drainage area. c. Finished metal parts awaiting assembly are stored exposed within its drainage area. d. A portion of the paint booth and the undercoating spray pit is located within its drainage area. e. Drums containing hazardous wastes, which are placed inside a plastic clam -shell storage device, are located within its drainage area. f. Roof drainage from the Tank Fabrication building accounts for a large portionof the impervious area. SDO-003, while having a larger drainage area than SDO-002, has fewer potential pollutant sources and less various exposed industrial activities. The industrial activities within the SDO-003 drainage area consist of: a. Parking for truck chassis awaiting tank mounting. b.. Finished tanker truck parking_. c. Finished modular tank assemblies storage. d. Employee parking lot. e. Roof drainage from the Tank Fabrication building. Prior to 2008, SDO-003 was not considered an industrial outfall as only employee parking and finished tanker truck parking occurred within its drainage area. Therefore no analytical monitoring data exists for SDO-003. When the Permittee's production increased and additional industrial activity sometimes occurred within the SDO-003 drainage area, it was decided that qualitative monitoring was prudent. Qualitative monitoring reports for SDO-002 and SDO-003 are enclosed. After NCG030000 was re -issued in 2007, the Permittee concluded that SDO-001 was not a point source discharge, and was thus eliminated as an outfall.. SDO-001 has always consisted of a short section of 12" HDPE pipe passing under the rear fence with the pipe discharging as sheet flow for a distance of at least 200 feet across an open field overgrown with kudzu. The following are our responses to your findings. 6. Housekeeping will be improved. The owner has committed to removing 50% of the accumulated abandoned equipment and scrap debris from the facility no later than December 31, 2009, and the remainder to be removed by March 31, 2010. Please note that metal stock will continue to be stored on racks outdoors. 6_a. All open top containers will either be closed or poured into a larger drum. Containers of unknown liquids will be removed from the site by Safety-Kleen by June 30, 2009. 6.b. The vehicle undercoating pit does accumulate rainwater. Effective immediately, the accumulated rainwater will be visually inspected, documented, and pumped onto the ground for infiltration only if the inspection shows no evidence of contamination. A checklist is provided in the SWPPP for recording these pre-release inspections. Further, we will lower the walls and cover the ends of the structure to preclude the entry of rainwater. Our experience with this accumulated rainwater is that it does not show an oil sheen — the undercoating material solidifies and settles to the bottom of the put — nor is the water discolored or cloudy. 6.c Sandblasting operations have been moved indoors effective April 10, 2009. We will test the sand for toxic and hazardous characteristics by May 31, 2009. The sand will be removed and disposed into an appropriate facility only if it is found to be toxic or hazardous; otherwise we will stabilize it in place with topsoil, seeding, and mulch. 6.d. The hazmat drum storage shed has a secondary containment volume of 603.5 gallons. The largest tank within this shed has a volume of 180 gallons. The volume of the secondary containment meets the regulatory requirements. This storage shed will be removed and replaced with commercially purchased hazmat storage devices with secondary containment We also will purchase an above ground diesel fuel tank with secondary containment. The Permit does not specifically require secondary containment inspections. There have been no controlled releases of accumulated rainwater — the hazmat shed has a roof, and does not accumulate significant rainfall — rainwater is allowed to evaporate, thus there are no records documenting such releases. The hazmat shed is inspected twice per year; the Permittee is directed to clean all drips and residuals immediately upon discovery. All drums and containers holding used or unknown materials currently sitting in the yard will be removed by Safety-Kleen no later than June 30, 2009. Employees have been instructed to store all containers indoors, within the designated hazmat shed, or within secondary containment pallets and to maintain labels and other identifying marks on the containers. This facility is a Conditionally Exempt Small Quantity Generator; applicable rules regarding the storage of hazardous wastes are being followed. 6.e. An employee has been tasked to walk the facility grounds and to remove with a shovel all soil with visible oil stains. This soil will be placed into a drum and removed along with the drums identified in 3.d. This task commenced as of April 15. 2009 and will be completed by May 15 2009. 6.f..1. By modifying this outfall into an 17 infiltration trench and level spreader, there will be no continuous conveyance that creates a point source discharge off the Permittee's property or into waters of the State. From this new level spreader, discharge will consist only of sheet flow for a distance of about 220 feet across a meadow and 50 feet through a wooded buffer into an intermittent stream. The Permit does not require monitoring of sheet flow. SDO-001 will be eliminated as a point source discharge before June 30, 2009. 6.f.2. Semi-annual analytical and qualitative monitoring is conducted at SDO.002 in accordance with the Permit. The monitoring point is at metal weir installed in an earthen ditch downgradient from two pairs of HDPE pipes that collect runoff from the northern portion of the facility. 613. Originally, SDO-003 was not considered an industrial outfall because only employee parking, the parking of truck chassis awaiting tanker assembly, and finished tanker trucks were parked within its drainage area. We thought this practice exempted the outfall as having no exposed industrial activity. In the last year or so, other industrial activities such as scrap debris and abandoned equipment storage have occurred within the SDO-003 drainage area. Upon reviewing our files, we have determined that a letter requesting ROS was never submitted. When our consultant was first hired in 2002, it was determined that samples would be collected from both industrial outfalls SDO-001 and SDO-002 and that SDO-003 was non- industrial. The ROS form now provided on the NCDWQ website will be submitted by May 1, 2009. We will make every attempt to collect a valid sample during a representative rainfall event from SDO-003 before June 30, 2009 unless ROS is granted sooner. We will cease collecting samples at SDO-001 as this outfall will no longer be considered a point source discharge. 7. We disagree with the second and third sentences in this paragraph. All of the semi-annual inspections as required by the Permit have been performed by our consultant since the Permit was issued November 1, 2007. Semi-annual inspections in compliance with the previous Permit were also performed; these reports have been archived. Employee training has not occurred every year, but a Permit & SWPPP compliance workshop will be held on May 5, 2009. The SWPP Team has been assigned and the organizational chart completed. All site inspections, analytical monitoring, and qualitative monitoring have been documented. There have been no significant spills or secondary containment releases requiring documentation. The following are our responses to your findings. 3. Housekeeping will be improved. The owner has committed to removing 50% of the accumulated abandoned equipment and scrap debris from the facility no later than December 31, 2009, and the remainder to be removed by March 31, 2010. Please note that metal stock will continue to be stored on racks outdoors. 3.a. The 55-gallon drum containing spent absorbent has been removed. The absorbent is being recycled and is currently in a new drum, inside the test facility. The old, rusted out drum has been destroyed. 3.b. The floor of the tank test building will be cleaned and all spent absorbents will be removed by May 31, 2009. The broken CMU-block containment wall, which provides tertiary containment, will be repaired by June 30, 2009. The Permit does not require containment for mobile tankers. This CMU- block wall is provided as an extra spill containment measure. The Permit does not specifically require secondary containment inspections. There have been no controlled releases of accumulated rainwater — the tank test building has three walls and a roof, and does not accumulate rainfall — thus there are no records documenting such releases. The tank test building is inspected twice per year; there has never been evidence that fuel leaks or spills exit this building. 7 3.c Sandblasting operations have ceased_ We will test the sand for toxic and hazardous characteristics by May 31, 2009. The sand will be removed and disposed into an appropriate facility only if it is found to be toxic or hazardous; otherwise we will stabilize it in place with topsoil, seeding, and mulch. 3.d. All drums and containers currently sitting in the yard will be removed by Safety-Kleen no later than .tune 30, 2009. Employees have been instructed to store all containers indoors or on the designated hazmat containment pallet, and to maintain labels and other identifying marks on the containers. This facility is a Conditionally Exempt Small Quantity Generator, applicable rules regarding the storage of hazardous wastes are being followed. 3.e. An employee has been tasked to walk the facility grounds and to remove with a shovel all soil with visible oil stains. This soil will be placed into a drum and removed along with the drams identified in 3.d. This task commenced as of April 15, 2009 and will be completed by May 15, 2009. V.,1. We disagree that the test structure be considered a significant pollution hazard: There is no evidence of fuel spill or leak, no requirement exists for containment of mobile tanker and the storage tank in the test structure is in secondary containment, see 3b. above. As to the outfall SDO-001, we request that Mr. Boone identify the "mildly defined conveyance" to which he refers, either by marking it on a site plan or returning to the facility and showing us the conveyance. 312. Monthly monitoring in accordance with Tier Two commenced at SDO.002 on March 26, 2009. The DMRs have been submitted to Central Files. The next monthly monitoring will occur before April 30, 2009, should a representative rainfall event occur before then. V-3. Only since 2006 has SDO-003 been considered an industrial outfall. Originally, this outfall only collected runoff from the employee parking lot and the administrative wing of the main manufacturing building. In early 2006, our consultant observed that water tank testing, truck rinsing, finished truck parking was occurring within the SDO-003 drainage area. The submittal of a letter requesting representative outfall status was overlooked. However, every ❑MR submitted to NCDWQ since the Permit was re -issued in 2007 has indicated that SDO-003 is represented by SDO-002. This Permittee has selected SDO-002 as its representative outfall for the following reasons: o It has the larger drainage area o It has the more various industrial activities occurring within its drainage area. o It has more exposed industrial activities within its drainage area. SDO-003 drainage area is dominated by an employee parking lot and a portion of the administrative offices from which runoff is not required to be monitored. The two industrial activities occurring within the SDO-003 drainage area are: o Pressure testing of water tanks mounted on trucks resulting in a discharge of non - contaminated water. o Parking of finished tanker trucks — a final product intended for outdoor use. This item alone would not prevent the "No Exposure" exemption. Under Item 3, we have indicated that these two drainage areas DO NOT contain same or similar materials because SDO-003 contains no materials. In our opinion, no pollutants at a level that would negatively impact waters of the state would be detected in the SDO-003 discharge. Prior to 2002, this Permittee had considered an area northwest of Bldg 4 as a potential monitoring point. This Permittee installed a metal weir and designated it as SDO-001. When their current consultant was hired in early 2002, it was decided that the weir location was not in an area of concentrated flow and thus was eliminated. There is no `.point source discharge" of stormwater northwest or west of Bldg. 4. Sometime in 2005, the Permittee began the aforementioned industrial activities within SDO-003. Prior to this date, this outfall. only discharged runoff from the employee parking lot and a portion of the admin building, and was not considered an industrial outfall. An ROS form was not submitted because no such form existed on the NCDENR/DWQ website until September 2008. Since January 2008, this Permittee has indicated to the NCDENR/DWQ that SDO-003 is represented by SDO-002 by notation on the submitted DMRs; no notice of rejection has been forthcoming from the DWQ Stormwater Permitting Unit. Rockingham County Dear Mr. Hancock: Mr. Ron Boone of the Winston-Salem Regional Office (WSRO) of the NC Division of Water Quality (DWQ or the Division) conducted compliance evaluation inspections (CEI) of Isometric's Reidsville plants #1 and #2 on 17 March 2009. The assistance and cooperation of Mike Leffew of Isometrics and James Frei of Stormwater Services Group, were greatly appreciated. , Inspection findings for both facilities are summarized below and inspection reports are attached for your records. It is noted that James Frei is a consultant commissioned by Isometrics to develop their Stormwater Pollution Prevention Plan (SP3). Isometrics, Inc. Reidsville Plant #1, 1266 North Scales Street, NCG030033 Site & Monitoring Review 2. This, facility is located at 1266 North Scales Street, in Reidsville, Rockingham County, North Carolina. Stormwater related to industrial activity is discharged from the site to an unnamed tributary to Wolf Island Creek, which is currently classified as Class C waters in the Roanoke River basin. The facility uses aluminum and stainless steel to manufacture tanks for fuel and water tanker trucks and then assembles and paints the trucks on site. 3. General housekeeping at this facility is very poor. The entire yard, as well as some of the outlying areas, which according to Mr. Leffew don't directly relate to company operations, but rather to the secondary activities of some of the company's employees, are littered with everything from truck parts, equipment, general solid waste and the like, to 55-gallon drums, cardboard boxes, pallets, spilled fluids, etc. The list below reflects some of the specific items noted by Mr. Boone during the inspection: There is an open -top 55-gallon drum located just outside the tank test structure. The drum is used to store spent absorbent material that is used inside the tank test structure. The bottom of the drum is rusted out and the absorbent, along with its absorbed constituents, is leaking onto the ground. Absorbed material is undoubtedly leeching into the soil from the drum. b. The tank test structure itself is a significant pollution hazard. This is a covered structure with two sidewal[s and a rear wall. There is a large fuel tank under the shelter and within a secondary containment. A spider web of pumps, hoses and piping is used to pump the fuel into the tanker trucks. There was a significant amount of spilled fuel in the northwest corner of the containment at the time of the inspection and a significant amount of fuel covered a large amount of the floor outside the containment. Handling of the fuel(s) appears haphazard and there are no records of secondary containment inspections as required by Part II, Section A, Paragraph 2b, of the permit. Furthermore, although the structure itself has a containment berm, the berm does not surround the entire structure and there are many voids between the walls and floor of the structure, providing an easy way for a spillage to escape the containment devices of the structure. . c. Sandblasting operations were or are being performed outside, on the west side of the plant. According to Mr. Leffew, all sandblasting operations were transferred to the Hwy 87 plant (plant #2). However, the spent sand was left on the ground at the site and has actually been pushed up and combined with riprap to form a stormwater diversion/filter, according to Mr. Frei. All sand previously used for sandblasting operations needs to be recovered and disposed of properly. The sand may be significantly contaminated with metals and paint residue, which over time will leech out into the ground, groundwater and surface water. The NC Division of Waste Management and Division of Air Quality have both been notified of this finding. d. There are a large number of 55-g0on drums, 5-gallon buckets and other miscellaneous size containers scattered throughout the entire plant and yard. Many of the containers are not properly labeled, sealed, etc. According to the SP3, the site does have a hazardous storage facility. All such containers must be identified, their contents identified, the containers properly labeled and sealed, and the containers and their contents stored, used or disposed of properly. e. There were several areas throughout the yard where materials had been spilled at some time and not properly cleaned up. The exact'type of materials is unknown but they are believed to be petroleum derivatives. The SP3 does include a spill plan. Formerly, the site had three Stormwater Discharge Outfalls (SDO), as specified below: 1. 001 was at the northwest corner of the site. Mr_ Frei stated during the inspection that he eliminated 001 as a point source discharge (PSD) from the site in 2001 or 2002 because he determined that discharge from the drainage area is sheetflow. He therefore discontinued qualitative and analytical monitoring at SDO 001. Upon examination, Mr. Boone determined that the stormwater draining from this area does have a mildly defined conveyance that begins near the rear northwest corner of the tank test structure and then dissipates out onto a dirt road and into a sloped forested area before coming to the receiving stream. The distance form the tank test structure to the stream is approximately 250 feet. Because the tank test structure is located within this drainage area and in its current condition is considered a significant pollution hazard, we require that SDO 001 be monitored both analytically and quantitatively at or near the northwest corner of the tank test structure until the pollution hazards in this area are remedied. Please begin analytical and qualitative monitoring immediately at this SDO in accordance with permit conditions. 002 is located near the southwest comer of the site. Analytical monitoring during late 2008 generated two consecutive exceedences of the Total Suspended Solids (TSS) benchmark value, requiring a tier two response in accordance with Part 11, Section 6, of the permit. However, analytical monitoring is not being conducted monthly and not all requirements and deadlines of the tier two response have been met. This must be addressed immediately. Mr. Frei and Mr. Leffew stated that, as part of the tier two response, a sediment basin is being constructed at SDO 002, in hopes of reducing TSS concentrations in the discharge. The basin is still not yet complete. Mr. Frei stated that monthly analytical monitoring was placed on hold until the basin was completed. You must immediately institute monthly monitoring at SOO 002 until three consecutive samples yield results below the benchmark value, regardless of other tier two response efforts, such as the sediment basin. Please review the tiered requirements in Isometrics, Inc. Attention: Mr. Lee Hancock, President Permit: NCG030033, NOV-2009-PC-0251 8 Permit: NCG030034, NOV-2009-PC-0252 25 March 2009 Page 3 of 5 Part II, Section 13, of the permit in detail. Failure to conduct the required tiered response measures is a violation of the permit. Additionally, Mr. Boone's initial assessment of the basin being constructed is that the basin is not large enough for the area being served. Isometrics may need to have the basin professionally designed and built by an engineer if TSS concentrations are not. sufficiently reduced. 3. 003 is at the northeast corner of the site. No issues were noted with this SDO, except for the fact that analytical monitoring is not being conducted at this SDO, as discussed further below. 4. There is some stormwater runoff from the southeast corner of the site but Mr. Boone agrees with Mr. Frei that this is sheetflow. Mr. Frei also stated during the inspection that Isometrics had been granted Representative Outfall Status (ROS) at some point in the past, although neither he nor Mr. Leffew had any documentation from the Division showing this. Furthermore, there is no documentation in our files showing that Isometrics has ROS for this facility. According to Mr. Frei, he submitted correspondence to the Division's Central Office in Raleigh, requesting ROS for this Isometrics facility. Although a reply to that correspondence was apparently never received, it was assumed that Isometrics was thereby granted ROS status. As a result, qualitative monitoring is currently being performed at SDOs 002 and 003 and analytical monitoring is being conducted at only SDO 002. This must be corrected immediately. You must begin the following monitoring regime immediately, and until further notice from this office: SDO 001: Immediately begin analytical and qualitative monitoring at.the location specified above in paragraph 3(f)(1) in accordance with the frequency requirements of the permit. SDO 002: Immediately begin monthly analytical monitoring,at this.SDO due to the exceedences of permit benchmarks for TSS, and continue qualitative monitoring at this SDO. Note again that monthly analytical monitoring must continue until the facility has three consecutive samples below the TSS benchmark. SDO 003: Immediately begin analytical and qualitative monitoring in accordance with the frequency requirements of the permit. Please note that Isometrics has not been granted ROS status for this site. DOCUMENTATION REVIEW 4. Review of the SP3 shows it to be acceptable and that it includes all the required components. However, the SP3 has not been implemented. Very few of the required inspections, training, documentations, updates, etc. have been accomplished as required. It is noted that Mr. Frei has been conducting biannual inspections of the site. Results of his inspections have been given to Isometrics, but very few of the things that he has noted in his reports have been taken care of. Isometrics, Inc. Reidsville Plant #2, 7537 NC Hwy 87, NCG030034 Site & Monitoring Review This facility is located at 7537 NC Hwy 87 North, in Reidsville, Rockingham County, North Carolina. Stormwater related to industrial activity is discharged from the site to an unnamed tributary to Wolf Island Creek, which is currently classified as Class C waters in the Roanoke River basin. The entire yard is littered with everything from truck parts, equipment, general solid waste and the like, to 55-gallon drums, cardboard boxes, pallets, spilled fluids, etc. The list below reflects some of the specific items noted by Mr. Boone during the inspection: There were several open -top containers located throughout the site at the time of the inspection, the exact contents of which were unknown. All such containers must be identified, their contents identified, the containers properly labeled and sealed, and the containers and their contents stored, used or disposed of properly. b. The vehicle undercoating structure has a roof but no sidewalls. It has a recessed pit that the painters enter so they can undercoat the bottom of the finished vehicles. There was significant staining of the concrete both inside and outside the pit. There is no drain in the pit and stormwater does get into it at times. The pit is simply pumped out onto the ground as needed but no inspections are done to determine if the water is contaminated prior to being pumped out. Although this is not technically a secondary containment (SC), Isometrics should inspect the water in this pit for contaminants before pumping it out, just as they would an SC for an above ground storage tank, if the water is found to be contaminated it should be retained and disposed of properly. Failure to do so may result in benchmark exceedences at the SDOs and/or surface and/or groundwater contamination. C. Sandblasting operations are being conducted in the open field on the east side of the plant. The spent sand is left on the ground at the site. It is unknown how large/long an accumulation of the sand exists at the site. In addition, the area around the sandblasting operation was littered with all kinds of miscellaneous equipment, solid waste and spilled fluids of a petroleum nature. All spent sand needs to be recovered and disposed of properly. The sand may be significantly contaminated with metals as well as paint residue, which over time will leech out into the ground, groundwater and surface water. The NC Divisions of Waste Management and Air Quality have both been notified of this finding. Additionally, all spilled fluids must be cleaned up and disposed of properly in accordance with the permit and the facility's SP3. All solid waste should also be cleaned up and disposed of properly. d. The drum storage area is covered and does have SC. There were approximately fifteen 55-gallon drums, several miscellaneous size containers and a fuel storage tank, which is approximately 250 gallons in capacity, within the containment at the time of the inspection. SC inspections are not being conducted and documented as required. Also, whether the volume of the SC meets permit conditions is questionable and should be reviewed. This area, also needs significant housekeeping. There were several open containers in and around the area that need to be identified, stored, used and/or disposed of properly. e. There were several other areas throughout the yard where other unidentified materials had been spilled at some time and not properly cleaned up. The exact type of materials is unknown. All such spills should be cleaned up and disposed of properly in accordance with the permit and the SP3. Isometrics and Mr. Frei are currently monitoring three SDOs at this site, as specified below: 1. 001 is 'at the northeast corner of the site and both analytical and qualitative monitoring are being conducted at this SDO. I No immediate problems were noted at the SDO but some drainage from the sandblasting operations and areas that require significant housekeeping are draining to this SDO. 2. 002 is located on the north side of the site. It drains the northern part of the -site, which includes the drum storage area and the undercoating facility. Both analytical and qualitative monitoring are being conducted at this SDO. No immediate problems were noted at the SDO. 3. 003 is 4ocated on the west side of the site, nearest the southwest corner. Only qualitative monitoring is currently being conducted at this SDO. No immediate problems were noted at the SDO. Isometrics, Inc.
