Storm water Prevention Plan
1 Otway sand mine Maintenance on Equipment- there is no maintenance performed on equipment on site of
the mine Equipment maintenance is performed at my shop in New Bern NC
2 There are no fuel or oil storages on site Fuel is brought in on an as need basis in my truck. I do this for many
reasons This Is to reduce run off and spillage potential
3 In the event that there is a fuel or oil spill, there are absorbers located in a storage trailer on site The fuel or
oil will be cleaned up and properly disposed
4 In the event there is a oil or fuel spill in or around excavaton area, absorbers will be placed, then call will be
made to field representative of NCDENR water quality for further instruction of how to assess cleanup
5 Monthly and quarterly reports are to be done in compliance with NCDENR so that test maybe done on
water to assure no potential hazardous runoff is leaving the property
6 Equipment that is not being used is parked away from excavation area to assure no type of runoff can make
it into the water
T Water is never pumped off property while it is raining, this method is used so that sediment will not leave
property It is my practice not to dewater at least 2 days after heavy rain
S There are 3 retention ponds on the property, all of which have check dams of riprap, this also insures better
management of runoff during heavy rainstorms
9 All roads used for trucks are well maintained, There are berms around hall roads, this is to prevent any type
of erosion or potential spills from trucks
10 Equipment is inspected daily to assure there are no potential leaks or hazards that may spill in work area
11 1 am on site at all times during mining operations If anything should happen I am thereto deal with any
potential problems
12 In this mine I have utilized 3 different ponds within the pit using 2 water dikes l never dewater from the
excavation pond off the property; I utilize the other ponds as sediment ponds before I dewater off the
property This ensures that the best possible water Is being pumped off the property Also by having these

ponds it limits the amount of water being pumped off the property
13 1 have included a color map of the site to show that the water is pumped off the property into a stream,
which ulbmately ends up in wards creek.
14 I hope this is a good storm water prevention plan, if there are any suggestions feel free to help better
revise this to ensure the best BMPs

A site inspection was conducted at the Otway Sand Mine on February 3, 2011 by Division Representatives, Linda
Willis and Joanne Steenhuis The inspection was conducted to determine the compliance status with permit
requirements specified in the NPDES. General Stormwater Permit NCG020000
Enclosed with this correspondence are
1 A copy of the NPDES General Permit (renewed January 1 2010)
2 A copy of the Compliance Evaluation Inspection report
3 A list of certified laboratories in the State
4 A copy of the required monitoring reporting forms (3 monitoring reporting forms)
Should you require any additional copies of the permit or monitoring reports, you may find them at the Division of
Water Quality s website located at http //portal ncdenr org/web/wq/ws/su/npdessw As a result of the inspection
and a file review, the following violations are noted
Item 1 Failure to monitor discharge from mine pit dewatering activities
Part III Section D and E , requires semi-annual qualitative and quarterly analytical monitoring of the discharges
from the mine pit When quarterly monitoring has not been conducted, monthly monitoring is required for the
remainder of the permit cycle You should begin conducting the monthly monitoring until further (written) notice
froh this office You should document the analytical data received from the lab (monthly) on the quarterly
monitoring report provided There is an annual reporting requirement that is due in March of every year Please
refer to Part IV , Section E , of your permit for details concerning reporting requirements
Item 2 Failure to prepare a written Stormwater Pollution Prevention Plan (SWP3) I
Part III , Section A, of the NPDES General Permit requires a written Stormwater Pollution Prevention Plan (due
by January 1, 2010)
