You are receiving this correspondence because you are currently listed as the owner of a
home/property covered under the subject General Permit for the discharge of domestic wastewater.
Please be aware this permit is not transferable to any person without prior written notice to and
approval from the Director in accordance with 40 CFR 122.61. A NPDES Certificate of Coverage
(CoC) NCG55000- Ownership Change Form is enclosed for your convenience. Please complete
the enclosed form as soon as possible or request permit rescission.
The State requires the Division to perform a Compliance Inspection at least, once every 5 years.
This office has been unsuccessful in several recent attempts to contact you to schedule a
Compliance Inspection for the subject wastewater system. Please call Mr. Justin Henderson at 336-
776-9701 at your earliest convenience to schedule this inspection.
