This COC was not renewed because there are still overdue fees. Please see the attached letter we sent out earlier this
year. Below is a screen shot of the annual fee that has not been paid (from 2017). When this has been resolved, please

The General Permit under which you are covered expires this year. Our records indicate there are past
due annual fees owed on this permit. Coverage under this permit for this site cannot be renewed until
these fees are paid. The new General Permit is scheduled to be issued effective August 1, 2019.
Sometimes fees lapse because billing contact or other information is out of date. Please review the
permit contact information enclosed. Ensure that all contact information is accurate and complete, and
that e-mail addresses are provided wherever possible. If you no longer need this permit, please submit
a Rescission Request Form. That form is available from our website:
