The attached inspection form notes the areas that were evaluated for the inspection, with notable issues summarized as follows: 1. Please ensure that disinfectant is added to keep a continual supply, and that chlorine tablets used for disinfection are properly labeled for wastewater (generally, tablets that utilize at least 65% calcium hypochlorite as the primary ingredient, see Part 1, Section D.1(a)). 2. The outlet pipe could not be located. Please note that the Permit requires a visual review of the outfall location to be executed at least twice per year to verify that there are no visible signs of malfunction (Part 1, Section D.6). Therefore, please ensure that the outfall is located and accessible for inspection. 3. Please be reminded that Part 1, Section C requires the discharged effluent to be analyzed at least once per year to verify its compliance with effluent limits. The analysis must be conducted by a certified laboratory. A list of labs in this area is enclosed. She said her septic tank was pumped about a year ago, but that's all that's been done for upkeep. She knows where the other tanks are but she doesn't maintain them. Does the facility analyze process control parameters, for ex: MLSS, MCRT, Settleable Solids, pH, DO, Sludge Judge, and other that are applicable? Comment: Permit Yes No NA NE (If the present permit expires in 6 months or less). Has the permittee submitted a new application? Is the facility as described in the permit? # Are there any special conditions for the permit? Is access to the plant site restricted to the general public? Is the inspector granted access to all areas for inspection? Comment: Effluent Pipe Yes No NA NE Is right of way to the outfall properly maintained? Are the receiving water free of foam other than trace amounts and other debris? If effluent (diffuser pipes are required) are they operating properly? According to records the effluent pipe is actually buried below the ground surface and hasn't been seen in decades. I tried to go to the area where the pipe is supposed to be but it was thickly overgrown. Comment: Septic Tank Yes No NA NE (If pumps are used) Is an audible and visual alarm operational? Is septic tank pumped on a schedule? Are pumps or syphons operating properly? Are high and low water alarms operating properly? Mrs. Suggs said the septic tank outside her house was pumped last year. She was not sure about the other septic tanks as they're managed by their individual residents. Comment: Disinfection-Tablet Yes No NA NE Are tablet chlorinators operational? Is there chlorine residual prior to de-chlorination? Chlorinator is enclosed in a ~3 foot diameter concrete riser with metal lid on it. There was vegetation growing on top of the riser lid. It obviously has not been checked in some time. Comment: Effluent Sampling Yes No NA NE Is composite sampling flow proportional? Is sample collected below all treatment units? Is proper volume collected? Is the tubing clean? # Is proper temperature set for sample storage (kept at less than or equal to 6.0 degrees Celsius)? Is the facility sampling performed as required by the permit (frequency, sampling type representative)? Effluent sampling is not conducting, and according to permit files there is no discharge to surface waters because the effluent pipe is buried. Comment: Page#4
