Good Afternoon,
We mailed the attached letter in March and it was returned to our office. Your permit expires on May 31, 2018. The
attached letter explains the instructions for submitting outfalls and checking permit contact information.

I noticed that there is no one listed in the owner affiliation – person responsible for permit. Please fill out attached
form and scan signed copy back to me. It is important that we have email addresses as well.
