Matt,
The Division of Energy, Mineral, and Land Resources (DELMR), received a complete Stormwater Management Permit
Application for the subject project on August 01, 2017. Staff review of the plans and specifications has determined that
the project, as proposed, will comply with the Stormwater Regulations set forth in Title 15A NCAC 2H.1000 rules. We
are forwarding Permit No. SW3170801, dated September 12, 2017, for the construction, operation and maintenance of the
subject project and the stormwater system. This permit shall be effective from the date of issuance and shall be subject to
the conditions and limitations as specified therein, and does not supersede any other agency permit that may be required.
The original signed permit package and approved plans are being mailed to Robert Langdon at:

This project will be kept on file after 30 days at the Mooresville Regional Office. If you have any questions, or need

Hope you had a good long relaxing weekend. I checked the files and although you did send the supplement form to
Laserfiche, it is not the current form. Please visit our website to obtain the proper forms for the following:
Completed Supplemental EZ form
Completed O&M EZ form (we need and original signed & notarized hardcopy).
I will start the review today, but will need those forms to continue with the review.
