We have reviewed the transfer application you submitted for the referenced mine site. In order for this office to
complete its review of the referenced project in accordance with G.S. §74-50 and §74-51 of the Mining Act of 1971,
please provide the additional or revised information in accordance with the following comments:
1. Please complete the first three pages of the current application posted on the mining program website.
The application was recently revised.
Vl/Please clarify the name of the permittee. The application lists D Logan as the applicant, however the
bond that was provided lists Blueberry LLC as the applicant. The name own the application must match

you? Can you share so I can revise — otherwise I will look into after I resubmit the documents
