in which you requested a 90-day extension of time to respond to the Department's request for supplemental
information regarding the issuance of the referenced permit.
The request for an extension has been granted. Should the Department not receive a complete response to its August
4, 2021 and August 11, 2021 request for additional information by May 13, 2022, The Director will make a decision
on the issuance based on the information currently on file.
