There is minor sediment loss at all cut outj some of which outlet into creeks and a pond not owned by the mine operator. Please design and implement some type of settling devise at all road cut outs. There is no berm along the edge of the road over the culvert area and sediment loss aId bank erosion was noted. Please provide an outlet structure at the culvert that will allow the road to drain without causing erosion issues and armor the banks around the culvert. Dust is a problem along the haul road and a water truck is needed on site at all times that the mine is operating. Please provide the new designs and implement them in the field and initiate dust control as it is a required operating procedure noted in your permit. Update the mine map showing these additional measures. Rodgers, Ashley From: Rodgers, Ashley Sent: Friday, March 25, 2011 2:36 PM To: Holley, John Subject: Matthew Pit 1 and 2 John, Could you please have someone go inspect Matthew Pit land 2 (51-41) in Johnston County? The permit was issued in 2004 and I don't see any record that it has ever been Inspected. There is nothing in the file or in IBEAM. Looking at the Johnston Co. GIS and Google Earth it seems that there is some disturbance outside of the permit boundary, but I can't tell if it is mine related or not. Just based on the aerials, it seems like it very well could be. Either way, it's time we inspected it! B Thanks! Wehner@ncmail.net> CC: Tracy Davis <Tracy.Davis@ncmail.net>, Floyd Williams <Floyd.Williams@ncmail.net>, Wyatt.Brown@ncmail.net Sorry for not meeting the suspense date; we were in a bit of a time crunch, and the snow/ice kept me out of the office until late yesterday. Y'all processed the package out on the 8th, and I picked it up on a swing through the central office late the next week. That only left us just over a week to review the package. In any event, it was reviewed last week, but I. needed to get feedback from DWQ/DAQ and discuss it with Wyatt Brown before sending final comments. Our comments are as follows: (1) Basic S&E appears satisfactory for this site considering the flat topography and buffers to any waters. The basin design appears adequate for efficiency considering the soil texture (will.settle easily) and dead storage that usually is available in such structures. The basins will essentially be initial pit openings that will grow as the mining activity continues. The silt fences proposed should be OK for initial site openings. zn that the access road is existing and flat, it should not be -problematic. I think it will be OK without further design data provided that they are willing to install stone check dams at any ditch outlets if the road ends up having to be regraded/adjusted at all, and if they are willing to use a temporary liner for stabilization of any, disturbed ditchlines. Otherwise, a typical cross-section. nd supporting design data (for ditch stabilization) must be require . e ound cover specs. on the drawings appear acceptable assuming reasonable seedbed preparation, but the revegetation plan approval sheet in the application was not completed. The construction sequence should be 1 of 2 1/29/2004 1:37 PM Re: Matthews Sand and Gravel • adjusted to eliminate the "certificate of compliance" s "certificate of completion" steps. (2) Proposed buffers appear adequate.` (3) DAQ indicates that a processing permit may be required and the T- applicant should contact them to discuss. (4) DWQ has no problems as long as we are. satisfied with the S&E since the proposed work does not encroach on waters. With adjustments or conditions to address the shortcomings identified in #1 above, the application should be adequate for permitting. If there are any questions, please advise.
