I plan to get the inspection report and photos into iBeam on Monday. The site was in compliance with the mining permit. We just had two comments from our site visit: 1) The skimmer frame should be flipped on Basin 1. 2) Continue to establish ground cover wherever possible. Comments from the application review: 1. May need encroachment agreements from DOT and Duke energy 2. It looks like there are wetland/stream impacts. May need 401/404. Per cover letter, they are still awaiting buffer determinations from DWR and wetland determinations from USACE. They state that updated maps will be submitted/application will be submitted once those determinations have been finalized. Those areas are currently not shown on the maps provided. We really cannot complete our review until those are finalized, correct? 3. May need pipe calculations for 36" RCP and driveway pipes. Pipe design calcs are needed if they are new pipes. 4. Provide proof that new neighbors (northwest of Rains Mill Rd) have bene notified of blasting. 5. There is no icon for silt fence in the legend. Please provide one. 6. No basin is shown in the area north of the pit for the transition period. 7. May need to ID the contours. 8. Label any SW outfalls on the map. 9. Silt fence is shown through a crossing location at access roads. Please correct and provide larger scale details showing how the silt fence will be installed in relation to the construction entrances. 10. Provide a construction sequence for installing the road crossing. 11. Provide maintenance requirements for the temporary crossing and wetland details. 12. Need erosion control measures in the northeast corner. Silt fence alone is inadequate protection for this area. 13. May need to provide deeds because other land owners are shown on the map. 14. Provide a baffle detail for sediment basins. 15. The buffer determination provided with the application is expired. See note 2 above. 16. Endangered species report provided with the application was only valid for 90 days and was dated November 2018 (expired). 17. No diversion ditch calculations were provided. Provide design calcs and procedures for stabilization of all diversion ditches.
