See Diane's comments in the attachment. She comments on the E&SC aspects of the submittal. It would be nice to get
those issues addressed on the current mine map, but I'm not sure if it is appropriate for this type of application. It may
be better to deal with those in the future. Whichever way you want to go with it is fine with me.

Did the regional office have any comments for the Little River Quarry / Luck Stone transfer? I believe this is a
greenfield, so I don't know if there is much to comment on anyway.

I have reviewed the permit application package and offer the following comments:
1. On the sediment and erosion control plan sheet, all skimmer locations and spillways should be clearly
shown (sheet 7 of 16).
2. Plan detail shows a rock dam/sediment basin. Please clarify where such basins are located and
drainage area of those basins.
3. Clean water diversion detail on page 13 of 16 needs to be modified to show matting of the
diversion. The construction sequence should state that the clean water diversion will be matted and
stabilized immediately.
Seeding specification should be included in sediment and erosion control plan portion of the package
�rosion control plan should include current stormwater requirements language to include:
a. "The contractor shall conduct self -inspections of the erosion and sedimentation control measures
and compete the following combined self -inspection form found on the DEMLR website
(http://deq.nc. gov/about/divisions/energy-mineral-land-resources/erosion-sediment-
control/formsl. Complete inspection forms shall be kept on -site and available for inspection at
all times. It is recommended a copy be kept in a permits box." (GS 113A-54.1 (e), 15A NCAC
0413.0131, NCG01 Section IIB (3)) . A copy of the form should be i'�ncluded.
b "Self -inspections for erosion and sedimentation control measures are to be performed at least
wl�
once every seven calendar days and within 24 hours of every rain event of greater than 1 inch.
Any needed repairs shall be made immediately to maintain measures as designed. All ESC
measures shall be maintained as specified in the construction details on this plan. A rain gauge
shall be installed at the project site for monitoring."

Y Detail the decommissioning of the skimmer basins in the construction sequence. Include the planned
\ use of the silt bag during this process. (GS 1�57(3), 15A NCAC 04B .0113)
I have included two pictures taken on the 21 st of August and my inspection report for that day.
