U Yes V No U N/A If no, explain: Operating Condition 8 G, Fly Rock Prevention - Fly rock was thrown beyond the areas where access is temporary or permanently guarded by the operator. Fly rock was found on the property next to the mine. Operating Condition 11 - Plan Modification - road access to the mine is not on the permit. Submit a report to the State Mining Engineer in the Raleigh office concening the fly rock incident. 20.
