Is the mine in compliance with the Operating Conditions of the Permit? ❑ Yes ® No ❑ N/A If no, explain: OC # 11 A -"No on -site disposal of refuse or other solid waste that is generated outside of the miningLpermit area shall be allowed.....The method of disposal shall be consistent with the approved reclamation plan" OC #11 B-"No petroleum products, acids, solvents or their storage containers or any other material that may be considered hazardous shall be disposed of within the permitted area." 17. Is the mine in compliance with the Reclamation Conditions of the Permit? U Yes N No U N/A If no, explain: RC minimum standard # 1-"The final slopes in all excavations in soil, sand, gravel and other unconsolidated material shall be at such an angle as to minimize the possibility of slides and be consistent with the future use of the land." RC #2B- "The side slopes to the lake excavation shall be graded to a 3 horizontal to 1 vertical or flatter to the water line and 2 horizontal to 1 vertical or flatter below the water line." RC #2C- "Any settling ponds or sediment basins shall be backfilled and stabilized. RC # 2D- "The processing, stockpile, and other disturbed areas neighboring the mine excavation shall be leveled and smoothed. RC #217 — "No contaminants shall be permanently disposed of at the mine site. On -site disposal of waste shall be in accordance with operation condition 11A-D." RC #2G- "The affected land shall be graded to prevent the collection of noxious or foul water. 18. Is there any off -site damage? U Yes N No Explain: While erosion is present at the site, all erosion is being contained within on site basins. 19. Corrective measures needed and/or taken: Uneven surfaces and ditches are present across the site. The stockpile should be leveled. Smaller basins which are acting as sediment ponds should be properly closed. Water should be pumped through a silt bag and the pond back filled and permanent vegetation established. Side slopes of basins are not in accordance with the reclamation plan. Waste debris, oil drums, pipes, rebar, buckets and a portable above ground tank was observed in wooded areas of the site. 20. Other recommendations and comments: Undisturbed area to northwest of the mine (86.6 acres) is suitable for a release. There is an additional 12.1 acres in the center of the mine site that has been undisturbed and is suitable for release. 21.
