Dear Mr. Hill:
On May 31, 2012, an inspection of the subject mine was performed by Ryan Kormanik of
this office. As discussed with you on the site, one deficiency wa, observed that requires your prompt
attention.
The deficiency discovered and the necessary corrective a,.tion is as follows:
OC #4C: An erosion and sediment control plan(s) shall be submitted to the Department for
approval prior to any land disturbing activities not indi(vcw on the revised erosion control
plan or mine maps submitted with the approved appli(ation for a mining permit and any
approved revisions to it. Such areas include, but are not limit,,-d to, expansion outside of the
approved pit area, creek crossings, or expansion of overrarden or waste disposal areas.
An area of approximately 1.5 acres which is not included within the active mining area per
the mine maps, MP-1 dated July 26, 2011, MP-2 and M1)-3 dated September 1, 2011
received in the Land Quality Section has been disturbed. operating Condition No. 4C of
your renewal permit dated February 2, 2012 addresses this concern. This area will either
need to be reclaimed back to original conditions or included within the active mining
boundary.
