Describe in detail the sequence of events for the development and operation of the mine and reference the sequence to the mine map(s).Attach additional sheets as needed. Addendum-1 long narrative attached. 2. Describe specific erosion control measures to be installed prior to land disturbing activities and during mining to prevent offsite sedimentation(include specific plans for sediment and erosion control for mine excavation(s),waste piles,access/mine roads,and process areas),and give a detailed sequence of installation and schedule for maintenance of the measures.Locate and label all sediment and erosion control measures on the mine map(s)and provide typical cross-sections/construction details of each measure.Engineering designs and calculations are required to justify the adequacy of any proposed measures. 1 -Sedimentation-control berm ( 2' T x 8' W, foliated) to be constructed inside unexcavated buffer around perimeter of excavation area. 2 - Diversion ditches constructed inside unexcavated buffer to divert stormwater internally to excavation. 3 - 35' W x 30' L gravel construction entrance to be constructed at intersection of mine access road and SR 1111 Woodley Road. 4 - Sediment pond outfall ditch to be lined with riprap for 20' length from discharge point for sedimentation control. 3. A)Will the operation involve wash in the material mined,recycling process water,or other wastewater handling? FIYES X NO If yes,briefly describe all such processes including any chemicals to be used. Not applicable. B)Will the operation involve discharging fresh or wastewater from the mine or plant as a point discharge to the waters of the State? YES �NO If yes,briefly describe the nature of the discharge and locate all proposed discharge points(along with their method of stabilization)on the mine map(s). Above-ground sediment pond overflow discharge through ditch to adjacent blue- line stream draining to wetlands and Scuppernong River through one discharge point (SDO-1). Discharge ditch will have 20-ft long riprap lining to prevent erosion of ditch bank at SDO-1. Revised September 21, 2021 P a g e 16 Addendum-1 Page 6-Mine Permit Application—Pearce Woodley Mine C. Protection of Natural Resources 1.Sequence of Events for Development and Operation: a. Operator will obtain all required mining permits from NCDEQ. b. Operator will construct one gravel construction entrance at intersection of mine access road and Woodley Road. c. Operator will remove topsoil down to 2.5 feet below natural ground level,and stockpile overburden for use in erosion control berms. d. Operator will construct 2-ft tall x 8-ft wide(2:1)vegetated berm for erosion control along the perimeter of the affected areas. e. Operator will maintain existing foliage, groundcover, and cultivated field visual screen along perimeter of the affected area. f. Operator will construct 0.26-acre above-ground sediment pond at southwest corner of excavation area. Sediment pond berm will be minimum 20 feet wide, 4 feet tall, with exterior slope at 3:1 grade,and interior slope at 2:1 grade. Two 6-inch PVC pipes will be installed in southwest sediment pond berm for water height control, discharging into riprap protected ditch (SDO-1) leading to blue-line stream ditch flowing to Scuppernong River. g. Operator will install dewatering pump between northeast corner of sediment pond and excavation area. Dewatering pump will be two 3-in transfer pumps running at low rpm with 6,000 gallon per hour flow into sediment pond. Pump will operate intermittently for average 12 hours per day upon need. h. Operator will begin excavation of pond (3.85 acres) approximately 25 feet east of sediment pond by digging with excavator and removing sand&topsoil to maximum depth feasible without dewatering the excavation. i. Operator will dewater main excavation into sediment pond, discharging overflow water from sediment pond through water height control pipes into blue-line stream ditches leading to Scuppernong River. Operator will dewater main excavation down approximately 20 feet below normal groundwater level. j. 3:1 cut slopes will be constructed around excavation edge down to normal groundwater level. As water is removed from excavation, 2:1 cut slopes will be constructed below normal groundwater level around excavation edge. k. Pond will be excavated to average 25 feet and maximum 30 feet below natural ground level while dewatering is maintained. Excavated material (sand) will be stockpiled in unexcavated portion of proposed excavation area. Sand will be loaded onto trucks and removed from area via construction entrance to SR 1111 Woodley Road. I. When final excavation of pond is complete, dewatering will be discontinued, and pump shut down and removed. Groundwater will refill the excavation to normal seasonal high groundwater level. m. After all excavation is complete,operator will establish groundcover on 3:1 above-water slopes of excavation and will construct 8ft wide 4:1 berm around excavation edge to prevent erosion of pond slopes, with outside edge of berm graded down to natural ground level. n. Aboveground sediment pond will be fully drained into excavated pond. Operator will dismantle and remove sediment pond berms and overflow discharge pipes will be removed. o. Operator will excavate former sediment pond area without dewatering,incorporating the area into main excavation,forming a full 3.85-acre pond. p. Operator will construct 8-ft wide by 1-ft tall berm around excavation area for stormwater erosion control. q. Former stockpile/ berm areas and processing areas outside of main excavations will be graded, leveled, and seeded with groundcover. Areas will be allowed to return to previous non-erosive field state. r. Internal roadways and haul roads will be graded, leveled, seeded with groundcover and allowed to return to natural field state. A field path will remain around the perimeter of the excavated pond. s. Operator will grade and level access road to Woodley Road back to original non-erosive state. t. Remaining affected areas inside main permit area will be fertilized and returned to natural field state. u. Remaining pond will be utilized by landowner for scenic&recreational purposes,and for wildlife habitat. RIM NO Estimated withdrawal rate in gallons per day: 72,000.00 If yes,what impact,if any,will mine dewatering have on neighboring wells?Locate all existing wells on the mine maps)that lie within 500 fleet of the proposed excavation area.Provide data to support any conclusions or statement made,including any monitoring well data,well construction data,and current water withdrawal rates.Indicate whether the proposed mine locale is served by a public water system or private wells. No impact expected, one active groundwater well(non-human consumption only)located at residence 225 ft from excavation,one shelter outbuilding owned by applicant with well located 168 ft from excavation. No other wells within 500 feet of excavation. The estimated depth of the mine will be 30 feet+/-with the water table depth to be estimated at 4-5 feet below the natural ground surface. The material will be removed and in the event the pit needs to be dewatered,the water will be pumped into the sediment pond which has emergency outlet in the event its volume is exceeded with rain or high water table. D)If you answered yes to any of the above questions,provide evidence that you have'applied for or obtained the appropriate water quality permit(s)(i.e.,non4scharge,NPDES,Stormwater,etc.)from the Stormwater Program.In addition,the applicant is required to register water use with the Division of Water Resources,Ground Water Management Branch,if the operation withdraws more than 10,000 gallons per day and needs a capacity use permit from the Division of Water Resources,Ground Water Management Branch,if the operation lies in a capacity use area and withdraws more than 100,000 gallons per day. Copy of NPDES NCG02 general permit application and pumping operation and maintenance plan for mine dewatering attached. Registration application for water usage has been submitted to DWR, copy attached.. 4. A)Will the operation involve crushing or any other air contaminant emissions? FTES ©NO If yes,indicate evidence that you have applied for or obtained an air quality permit issued by the Division of Air Quality or local governing body. Not applicable. B)How will dust from stockpiles,haul roads,etc.,be controlled? The access road, stockpiles, and internal haul roads will be sprayed with water from the mine excavation several times a day, as needed, to control fugitive dust. A)A buffer will be required between any mining activity and any mining permit boundary or right-of-way.It may be an unexcavated buffer(no excavation,but roadways,berms,and erosion&sedimentation control measures may be installed within it),an undisturbed buffer(no disturbance within the buffer whatsoever),or a combination of the two, depending upon the site conditions.Note that all buffers must be located within the mining permit boundaries. How wide a buffer will be maintained between any mining activity and any mining permit boundary or right-of-way at this site?A minimum buffer of 25 feet is recommended,although a wider buffer may be needed depending on site conditions.Show all buffer locations and widths on the mine map(s). 30'wide unexcavated buffers will be maintained between the permit boundary and mining activity on north, south, &west sides of permit area. 50'wide unexcavated buffer will be maintained between the permit boundary/ Woodley Road right-of-way and mining activity on east side of permit area. B)A minimum 50 foot wide undisturbed buffer will be required between any land disturbing activities within the mining permit boundaries and any natural watercourses and wetlands unless smaller undisturbed buffers can be justified. Depending on site conditions,a buffer wider than 50 feet may be needed. How wide an undisturbed buffer will be maintained between any land disturbing activities within the mining permit boundaries and any natural watercourses and wetlands at this site?Show all buffer locations and widths on the mine map(s). No wetlands on site, no undisturbed buffers. Blue-line stream/ditch located in 30' wide unexcavated buffer on south side of permit area, and blue-line stream roadside ditch located in 50'wide unexcavated buffer on east side of permit area. Both streams will be protected by silt fence, erosion control measures maintenance corridors, and 2' tall x 8' wide sedimentation control berms with groundcover. 6. A)Describe methods to prevent landslide or slope instability adjacent to adjoining permit boundaries during mining. Minimum 2 horizontal to 1 vertical slopes or flatter for clayey material and minimum 3 horizontal to 1 vertical slopes or flatter for sandy material are generally required unless technical justification can be provided to allow steeper slopes. Excavation will be surrounded by minimum 30-ft wide unexcavated buffer with 2- ft tall containment berm and diversion drains to prevent stormwater damage to lands outside of permit area. Cut slopes of excavation will have 3:1 slope above pond water level, and 2:1 slope below water level. B)Provide a cross-section on the mine map(s)for all fill slopes(berms,waste piles,overburden disposal areas,etc.), clearly indicating the intended side slope gradient,installation of any benches and/or slope drains(with supporting design information)if needed,and the method of final stabilization. The cut slopes of all excavated diversion ditches and walls of the sediment basin will be maintained at a minimum of 2 horizontal to 1 vertical or flatter.The cut slopes around the perimeter of the mine excavation will be maintained at a minimum of 3:1 slope or flatter above normal pond water level and 2:1 or flatter below normal pond water level.All cut slopes and other areas affected by the mining operation will be stabilized with vegetation as indicated on attached sheets to protect their integrity and promote slope stability. D)In hardrock excavations,specify proposed bench widths and heights in feet.Provide cross-sections of the mine excavation clearly noting the angles of the cut slopes,widths of all safety benches and mine benches,and the expected maximum depth of the excavation. Not applicable - no hard rock excavation. 7. Describe other methods to be taken during mining to prevent physical hazard to any neighboring dwelling,house, public road,or public,commercial or industrial building from any mine excavation.Locate all such structures on the mine map if they are within 300 feet of any proposed excavation. Operator's shelter outbuilding located 168 feet adjacent to excavation area and one residence located across Woodley Road and north 225 feet from excavation, no other buildings located within 300 feet of excavation. Lightly travelled SR 1111 Woodley Road located 68 ft from mine activity area, separated by 50-ft unexcavated buffer containing berms and silt fence. Gravel construction entrance will be constructed at entrance to mine area. Gated access road to mine activity area maintained by operator. 8. Describe what kind of barricade will be used to prevent inadvertent public access along any high wall area and when it will be implemented.Vegetated earthen berms,appropriate fencing and adequate boulder barriers may be acceptable high wall barricades.A construction detaillcross section and location of each type of barricade to be used must be indicated on the mine map(s). A lockable gate will be maintained at the site' s entrance to prevent access by others outside of normal business hours. Vegetated earthen berms are also provided around the entire perimeter of the mining operation to prevent inadvertent public entry. 9. Are acid roducing minerals or soils present? YES NO If yes,how will acid water pollution from the excavation,stockpiles,and waste areas be controlled? Not applicable. Permit area will be surrounded by foliated berms with mining activity located 50 feet from Woodley Road. B)Could the operation have a significantly adverse effect on the purposes of a publicly owned park,forest,or recreation area?If so,how will such effects(i.e.,noise,visibility,etc.)be mitigated? Ken Elliott,consulting October 17,2024 Berm Construction Details Pearce Woodley Mine Columbia, NC Tyrrell County Narrow-based berm The narrow-based berm is the most common berm having a top of 2 feet(0.6 m)and side slopes of two horizontal: 1 vertical.This berm is the most economical to build as it requires the least material.The steep side slopes of this type of berm eliminate the possibility of working on it with farm machinery. 10, a z a 2 it �I 2' NEW BERM EXISTING GRADE Berm Construction The berm construction comprises of the scraping back of the topsoil from the area where the berm and storage basin is to be built.A subsoil material,free of stones, rubble,stumps etc.and with at least 10% clay should be used to construct the berm.The material for the berm may be obtained from the area behind or in front of the berm location or moved from a borrow pit located elsewhere on the farm.An earth mover will be required to move material from a pit located a distance away while a bulldozer would be used if material is located in the berm area. The berm material is to be placed in layers and compacted as each layer is added. After the berm has a final shape the sides of the berm should also be compacted. Riprap Apron Size:Overflow pipe discharges directly into well-defined channel. The apron will extend across the channel bottom and up the channel banks to the top of the bank. length of apron will be 20 feet downstream to maintain velocity control of maximum 2.0 feet per second. Grade: Apron will have zero grade and no overfill at end of apron. Alignment: Apron will be straight throughout entire length if possible. If curve is necessary to align the apron with the receiving channel,the curve will be in the upstream section of the riprap. Materials: Riprap will consist of a well graded mixture of NCDOT Class A(2"to 6")stone or slag. Thickness:Minimum thickness of riprap will be 1.5 times the maximum stone diameter. Filter:Operator plans to install a layer of heavy-duty silt fence filter cloth between the riprap and the underlying soil to prevent soil movement into or through the riprap. Filter cloth will extend to the top of both channel banks, and edges will overlap 12 inches. Upstream end will be buried 12 inches and downstream end will be buried if necessary. Maintenance: Inspect riprap outlet structures weekly and after significant rainfall events to see if any erosion around or below the riprap has taken place,or if stones have become dislodged. Repair all damage immediately. Maintain all vegetation adjacent to the channel in healthy condition to prevent scour or erosion during out-of-bank flow. Control weeds and brush growing in the riprap by cutting. i� c cc Ken Elliott,consulting October 17,2024 Sediment Fence (Silt Fence) Construction Details: Practice Standards and Specifications 6.62 Pearce Woodley Mine Columbia, NC Tyrrell County A temporary permeable barrier erected on small,disturbed areas to capture sediment from sheet flow. It is made of filter fabric buried at the bottom, stretched and supported by steel posts. Planning:Silt fence will be installed along the east and south permit boundaries,between an erosion control berm and adjacent blue-line-stream ditch. Narrow width of fenced area will allow only shallow pools to form behind fence while promoting deposition of sediment and preventing sedimentation into adjacent wetlands. Ends of fence will be curled uphill in"J" pattern to prevent end flow. Stabilized outlets will be provided to protect the fence system and release storm flow that exceeds the design. Design considerations:Silt fence will be constructed of extra-strength synthetic filter fabric. Height of fence will not exceed 2 feet above natural ground level and will be constructed from a continuous roll to minimize joints. All joints will be fastened at a support post with 4 feet minimum of overlap. Extra-strength filter fabric will be fastened to road side of steel posts(installed 2 feet deep)every 6 feet(every 8 feet if using standard strength filter fabric with wire mesh backing)and fastened with wire ties. Trench will be excavated along inside of fence approximately 4 inches wide by 8 inches deep, into which 12 inches of fabric will be placed along the bottom and side of the trench. Trench will be backfilled and compacted. Maintenance: Inspect sediment fence every week and after each rainfall. Make any required repairs immediately. Replace any collapsed,damaged,or decomposed fence promptly. Remove all fencing materials and unstable sediment deposits and bring the area to grade and stabilize it after the contributing drainage area has been properly stabilized.
