Is the mine in compliance with the Operating Conditions of the Permit? ❑ Yes ® No ❑ N/A If no, explain: OC 10A: Additional LCID material from areas not covered by the permit have been disposed of on site without an authorization in place. OC 12: The operator shall notify the Department in writing of the desire to delete,modify,or otherwise change any part of the mining,reclamation,or erosion/sediment control plan contained in the approved application for a mining permit or any approved revision to it. Approval to implement such changes must be obtained from the Department prior to on-site implementation of the revisions 17. Is the mine in compliance with the Reclamation Conditions of the Permit? ❑ Yes ® No ❑ N/A If no, explain: RC:2B The side slopes of the mining area have not been sufficiently stabilized nor at such an angle as to restrain erosion. 18. If B or C,is there potential for offsite damage? ❑ Yes ® No Explain: 19. Corrective measures needed and/or taken: As of this inspection the site is nearly in compliance. The slopes along the boundaries of the mining pit area are not at the required slope angle,not stabilized and have significant erosion occurring. 20. Other recommendations and comments: Permit boundary stakes have been placed on site. Maintain these permit lines throughout the life of the permit. No reclamation report has been received by this office.Reclamation of the mine is approaching the final areas.Ensure that the reclamation reverts the pit to the original boundaries and designations listed for the buffer zones. Material has been documented being dumped on site that would require an LCID permit, however NO PERMIT OR NOTIFCATION HAS BEEN ISSUED. 21.
