repurposed to a residential development as indicated in the application,maps received by the
Department April 21, 2022, and the approved Erosion Control Permit#BRUNS-2022-083. The permitted
area is 53.10 acres and the area approved to disturb is 44.78 acres.

and permit 10-62 was suitable for release.
Release letter Drafted: 12/11/2023
Missing Bond
When searching the safe for the original ISLOC for permit 10-62 in order to return with the 12/11/2023
release letter,the original ISLOC could not be located. The read file coversheet stated that this ISLOC#
1088 had already been released on 1/15/2020.
A release letter for permit 10-38 for the same operator,Wayne's Backhoe Service Inc.,was also dated
1/15/2020.
In 2022 during a file review, it was discovered that the bond for permit 10-38, ISLOC#40630, in the
amount of$6,400 was never returned for the 1/15/2020 release. On 3/24/2023,a letter was sent to the
operator letting them know of this error and returned ISLOC#40630.
It is likely that ISLOC#1088 in the amount of$28,400.00(for permit 10-62)was sent in error in lieu of
the ISLOC#40630 in the amount of$6,400.00. The read file cover date matches the date on the 10-38
release letter.
For the release of 10-62,we sent a copy of ISLOC#1088 with the release letter dated 12/11/2023.
The safe is now accurate with both 10-38 and 10-62 permits released and both ISLOC's returned.
