Request.On April 18,2023,Central Office received a signed released request from Steve Simmons,for
Release of Simmons Mine, Mine Permit, Number 10-35.
On May 11,2023, Garrett Elmo and Austin Turbitt, Environmental Specialists,of the Wilmington
Regional Office inspected the Simmons Mine and found it was releasable.
Area to be released is 7.0 acres.
Inspection Results. On March 2, 2023, Garrett Elmo and Austin Turbitt, Environmental Specialists, of the
Wilmington Regional Office inspected the Simmons Mine and found it was releasable.
Decision. The site visited and the inspection report confirms that subject site is satisfactory reclaimed.
As documented in this memorandum,the Mining Program recommends the release of 7.0 acres.The
amount of security was$13,600.00.The security was an Assignment of Saving Account from BB&T. The
security is to be returned to Steve Simmons.

An inspection was made of the above mine on May 12, 2023, in accordance with N.C.G.S. §74-56 of the Mining
Act of 1971.
The mine was found to be satisfactorily reclaimed. Therefore, you are released from further reclamation
responsibility for this site with respect to the Mining Act of 1971. 1 am returning your Assignment of Savings
Account,No. 0005419170544 issued by BB&T in the sum of$13,600.00 to you for your disposal.
