A)Will the operation involve washing the material mined, recycling process water,or other wastewater handling? FIYES IN NO If yes, briefly describe all such processes including any chemicals to be used. B)Will the operation involve discharging fresh or wastewater from the mine or plant as a point discharge to the waters of the State? YES NO If yes, briefly describe the nature of the discharge and locate all proposed discharge points(along with their method of stabilization)on the mine map(s). This site will have 2 de-watering basins installed independently as each phase of the mine is started. They will be located on the south of the mine and will discharge storm water/wastewater into an existing farm ditch that currently flows into existing wetlands. The outlets will be stabilized with rip rap aprons. (See calculations enclosed for the de-watering basins and rip rap aprons.) 6 Are you applying for a discharge permit in the same location as a previously permitted mine?
