On behalf of Mr. Bobby J. Tripp, president of Tripp Bros, Inc., Elliott Consulting is requesting review of an
Application for a Mining Permit for a new mine operation slated for development in Grimesland Township, in
Pitt County, North Carolina. The operation is referred to as the Alvin Road Mine, based on ownership
preference, and includes a total permit area of 41.80 acres.

• $5,000 permit Processing Fee
• Mine Map and Reclamation Map
• Adjoinder Notification Map
• USGS Quad Site Map
Pitt County Stream Determination for applicability to Tar-Pamlico Riparian Area Protection
• Copy of Application for NCG020000 coverage with Pumping Operation and Maintenance Plan
• Copy of NCDEQ DWR Registration Application for Water Usage
