Describe in detail the sequence of events for the development and operation of the mine and reference the sequence to the mine map(s).Attach additional sheets as needed. Addendum-1 long narrative attached. 2. Describe specific erosion control measures to be installed prior to land disturbing activities and during mining to prevent offsite sedimentation(include specifrc plans for sediment and erosion control for mine excavation(s),waste piles,access/mine roads,and process areas),and give a detailed sequence of installation and schedule for maintenance of the measures.Locate and label all sediment and erosion control measures on the mine map(s)and provide typical cross-sections/construction details of each measure.Engineering designs and calculations are required to justify the adequacy of any proposed measures. 1 - Containment berm (6' T x 26' W, foliated) to be constructed inside unexcavated buffer around perimeter of excavation area. 2 - 30'W x 200' L paved construction entrance to be constructed at intersection of mine access road and SR 1562 Alvin Road. 3- Sediment pond outfall ditch to be lined with riprap for 20' length from discharge point for sedimentation control. 4 - Operator will install erosion and sediment control fencing adjacent to each side of access road at two blue-line-stream crossings. 3. A)Will the operation involve wash in the material mined,recycling process water,or other wastewater handling? RYES NO If yes,briefly describe all such processes including any chemicals to be used. Not applicable. B)Will the operation involve discharging fresh or wastewater from the mine or plant as a point discharge to the waters of the State? MYES RNO If yes,briefly describe the nature of the discharge and locate all proposed discharge points(along with their method of stabilization)on the mine map(s). Excavated 1.73-acre sediment pond overflow discharge through ditch to adjacent wetland through one discharge point (SDO-1). Discharge ditch will have 20-ft long riprap lining to prevent erosion of ditch bank. Revised September 21,2021 Page 16 Addendum-1 Page 6-Mine Permit Application—The Sandlot(Excavation) C. Protection of Natural Resources 1.Sequence of Events for Development and Operation: a. Operator will obtain all required mining permits from NCDEQ. b. Operator will construct one 30'wide paved construction entrance joining intersection of mine access road and SR 1562 Alvin Road. Paved entrance will be 200 feet long. c. Operator will construct a locking gate to be located across the mine access road entrance 80 feet from intersection with the Alvin Road right-of-way. d. Operator will install sediment and erosion control fencing adjacent to each side of access road at two blue-line-stream crossings. e. Operator will remove topsoil down to 2.5 feet below natural ground level,and stockpile overburden for use in erosion control berms. f. Operator will construct a 2-ft tall x 14-ft wide (3:1) vegetated berm for erosion control along the perimeter of processing areas. g. Operator will construct a 6-ft tall x 26-ft wide (2:1)vegetated berm for visual screening around perimeter of excavation area. The berm will be located approximately 700 feet from Alvin Road right-of-way and will be approximately 4000 feet long with openings for mining equipment and haul truck usage,and floodway stormwater drainage. h. Operator will maintain existing foliage and cultivated field visual screen along perimeter of the affected area. i. Operator will excavate a 20-ft deep 1.73-acre pond at the north end of the permit area for usage as a sedimentation pond for dewatering the main excavation. Operator will construct a sand berm around the sediment pond 4' tall x 22' wide, sown with groundcover to fully contain dewatering discharge. Operator will install a half-pipe flash- board riser overflow at the southwest corner of the sediment pond, connected to a minimum 18-in PVC pipe discharging to a single discharge point to a blue-line stream draining to Chicod Creek and Tar River. Discharge point will have a 20-ft long riprap stone apron for outfall protection to prevent erosion. j. Operator will install a Godwin Dri-Prime or similar model dewatering pump at the northeast corner of the main excavation. The pump will remove approximately 96,000 gallons of water per day from the main excavation and discharge it into the southeast end of the sediment pond. Water will then overflow at the southwest end of the sediment pond. The dewatering pump will operate at 1200 rpm intermittently for average 12 hours per day upon need. k. Operator will begin excavation of the main pond (14.88 acres) approximately 200 feet south of sediment pond by digging with excavator and removing sand & topsoil to maximum depth feasible without dewatering the excavation. I. Operator will dewater main excavation into sediment pond, discharging overflow water from sediment pond through PVC discharge pipe into a blue-line-stream leading to Chicod Creek and Tar River. Operator will dewater main excavation down approximately 20 feet below normal groundwater level. m. 3:1 cut slopes will be constructed around excavation edge down to normal groundwater level. As water is removed from excavation, 2:1 cut slopes will be constructed below normal groundwater level around excavation edge. n. Pond will be excavated to average 30 feet and maximum 35 feet below normal groundwater level while dewatering is maintained. Excavated material (sand) will be stockpiled in unexcavated portion of proposed excavation area. Sand will be loaded onto trucks and removed from area via paved construction entrance to SR 1562 Alvin Road. o. When final excavation of pond is complete, dewatering will be discontinued. Pump will be shut down and removed,and discharge pipe will be removed. Groundwater will refill the excavation to normal seasonal high groundwater level. p. After all excavation is complete,operator will establish groundcover on 3:1 above-water slopes of excavation and will construct a minimum 8-ft wide 4:1 berm around excavation edge to prevent erosion of pond slopes, with outside edge of berm graded down to natural ground level. q. Sediment pond will be drained through the overflow riser down to normal water level, and then the riser and discharge pipe will be removed. Operator will remove the discharge pipe and other equipment from main excavation and remove sediment pond berms. Sediment pond disturbed area will be graded, leveled, and sown with groundcover. Area will be allowed to return to previous field state. r. Former stockpile/ berm areas and processing areas outside of main excavations will be graded, leveled, and seeded with groundcover. Areas will be allowed to return to previous non-erosive field state. s. Internal roadways and haul roads will be graded, leveled, seeded with groundcover,and allowed to return to natural field state. A field path will remain around the perimeter of the excavated pond. t. Remaining affected areas inside main permit area will be fertilized and returned to natural field state. u. Remaining pond will be utilized by landowner for scenic&recreational purposes,and for wildlife habitat. No impact expected. No residences or active groundwater wells located within 500 feet of proposed excavation. Mine locale is served by Pitt County public water system. The estimated depth of the mine will be 30 feet+/-with the water table depth to be estimated at 6-8 feet below the natural ground surface. The material will be removed wet and in the event the pit needs to be dewatered, the water will be pumped into the sediment pond which has emergency outlet in the event its volume is exceeded with rain or high water table. D)If you answered yes to any of the above questions,provide evidence that you have applied for or obtained the appropriate water quality permit(s)(i.e.,non-discharge,NPDES,Stormwater,etc.)from the Stormwater Program.In addition,the applicant is required to register water use with the Division of Water Resources,Ground Water Management Branch,if the operation withdraws more than 10,000 gallons per day and needs a capacity use permit from the Division of Water Resources,Ground Water Management Branch,if the operation lies in a capacity use area and withdraws more than 100,000 gallons per day. Copy of NPDES NCG02 general permit application and pumping operation and maintenance plan for mine dewatering attached. CCPCUA registration application has been submitted online to DWR, copy attached.. 4. A)Will the operation involve crushing or any other air contaminant emissions? FIYES ®NO If yes,indicate evidence that you have applied for or obtained an air quality permit issued by the Division of Air Quality or local governing body. Not applicable. B)How will dust from stockpiles,haul roads,etc.,be controlled? The access road, stockpiles, and internal haul roads will be sprayed with water from the mine excavation several times a day, as needed, to control fugitive dust. A)A buffer will be required between any mining activity and any mining permit boundary or right-of-way.It may be an unexcavated buffer(no excavation,but roadways,berms,and erosion&sedimentation control measures may be installed within it),an undisturbed buffer(no disturbance within the buffer whatsoever),or a combination of the two, depending upon the site conditions.Note that all buffers must be located within the mining permit boundaries. How wide a buffer will be maintained between any mining activity and any mining permit boundary or right-of-way at this site?A minimum buffer of 25 feet is recommended,although a wider buffer may be needed depending on site conditions.Show all buffer locations and widths on the mine map(s). 1) 125-ft wide unexcavated buffer between excavation &west property line. 2) 125-ft wide unexcavated buffer between excavation & east property line. 3) 50-ft wide undisturbed buffer& 25-ft wide unexcavated buffer between excavation/sediment pond &wetlands adjacent north permit boundary. 4) 50-ft wide undisturbed riparian buffer between mine activity area and each bank of blue-line stream south of excavation. 5) 300-ft wide undisturbed buffer between processing area and south property line /Alvin Road right-of-way. B)A minimum 50 foot We undisturbed buffer will be required between any land disturbing activities within the mining permit boundaries and any natural watercourses and wetlands unless smaller undisturbed buffers can be justified. Depending on site conditions,a buffer wider than 50 feet may be needed. A)Describe methods to prevent landslide or slope instability adjacent to adjoining permit boundaries during mining. Minimum 2 horizontal to 1 vertical slopes or flatter for clayey material and minimum 3 horizontal to 1 vertical slopes or flatter for sandy material are generally required unless technical justification can be provided to allow steeper slopes. Excavation will be surrounded by 25-ft wide unexcavated buffer with 6-ft tall containment berm and diversion drains to prevent stormwater damage to lands outside of permit area. Cut slopes of excavation will have 3:1 slope above pond water level, and 2:1 slope below water level. B)Provide a cross-section on the mine maps)for all fill slopes(berms,waste piles,overburden disposal areas,etc.), clearly indicating the intended side slope gradient,installation of any benches and/or slope drains(with supporting design information)if needed,and the method of final stabilization. The cut slopes of all excavated diversion ditches and walls of the sediment basin will be maintained at a minimum of 2 horizontal to 1 vertical or flatter.The cut slopes around the perimeter of the mine excavation below water level will be maintained at a minimum of 2 horizontal to 1 vertical or flatter, and 3 horizontal to 1 vertical above water level.All cut slopes and other areas affected by the mining operation will be stabilized with vegetation as indicated on attached sheets to protect their integrity and promote slope stability. D)In hardrock excavations,specify proposed bench widths and heights in feet.Provide cross-sections of the mine excavation clearly noting the angles of the cut slopes,widths of all safety benches and mine benches,and the expected maximum depth of the excavation. Not applicable - no hard rock excavation. 7. Describe other methods to be taken during mining to prevent physical hazard to any neighboring dwelling,house, public road,or public,commercial or industrial building from any mine excavation.Locate all such structures on the mine map if they are within 300 feet of any proposed excavation. No buildings, dwellings, houses, or public roads located within 300 feet of mine excavation. 8. Describe what kind of barricade will be used to prevent inadvertent public access along any high wall area and when it will be implemented.Vegetated earthen berms,appropriate fencing and adequate boulder barriers may be acceptable high wall barricades.A construction detaftross section and location of each type of barricade to be used must be indicated on the mine map(s). Locking gate will be maintained at the site' s entrance to prevent access by others outside of normal business hours. 6-ft tall vegetated earthen containment berm will be maintained around the full perimeter of the mine activity area to discourage inadvertent entry. 9. Are acid roducing minerals or soils resent? YES XNO If yes,how will acid water pollution from the excavation,stockpiles,and waste areas be controlled? Not applicable. A)Describe specific plans(including a schedule of implementation)for screening the operation from public view such as maintaining or planting trees,bushes or other vegetation,building berms or other measures.Show the location of a# visual screening on the mine map(s)and provide cross-sections through all proposed berms or proposed spacing, sizes and species for free plantings. 6-ft tall earthen containment berm will be maintained around the full perimeter of the mining operation to discourage inadvertent entry. Berm will be sown with groundcover and natural foliation per guidelines. No further screening will be required due to 710-ft distance to closest public road with separation by cultivated field. B)Could the operation have a significantly adverse effect on the purposes of a publicly owned park,forest,or recreation area?If so,how will such effects(i.e.,noise,visibility,etc.)be mitigated? There are no known publicly owned parks, forests, or recreation areas in close proximity to this proposed mining operation. 11. Will 11YES sives be used? ONO If yes,specify the types of explosive(s)and describe what precaution(s)will be used to prevent physical hazard to persons or neighboring property from flying rocks or excessive air blasts or ground vibrations.Depending on the mine's location to nearby structures,more detailed technical information may be required on the blasting program(such as a third-party blasting study).Locate the nearest offsite occupied structure(s)to the proposed excavation(s)on the mine map and indicate its approximate distance to the proposed excavation.
