Additional Requested Comments: 1. Property owners have been added to the mine map. 2. The permit boundary does extend to the public road and is now shown on the mine map. 3. The stream buffers have been added to a new map labeled 3.5 of 5. This should better show the intended stream buffers on the mine map. Please reference map EC-200 for more detail. 4. Included in the packet is an updated page that can be included in the binder booklet showing an adequate plug and sequencing as requested by Mooresville Regional Office. 5. Sheet EC-4 page 41 has been updated to show adequate silt fencing along the entire project area. 6. Attached pages from DRG has added a note on the pump around with a minimum 3' top crest. 7. Sheet EC-4 page 41 has been updated showing sediment trap and erosion control at the bridge crossing. 8. All mine maps have been updated with legend matching colors on the maps. 9. I have kept the gray color on the mine maps for the areas NE and NW showing the pits that were permitted in 2008 and approved by Lincoln County Zoning. I have not shown these areas on Maps 3 and 3.5 and only highlighted the waste area. The waste area would be constructed in the areas of the approved pit area. After the waste area is removed once the Forney Creek project is complete, we still may mine these areas in the future. 10. I have updated the reclamation map to show the pit as a fixture water source. The areas shown as pits in the NE and NW areas of the map will not be mined below the water table and be left as pits. This concept could change and will be updated with additional mapping in the future if needed. THE CONTRACTOR WILL CONSTRUCT THE STREAM FROM UPSTREAM TO DOWNSTREAM WITHIN EACH REACH DETAILED BELOW. ALL DISTURBED AREAS SHALL BE SEEDED (WITH APPROPRIATE MIX) AND STRAW MULCHED AT THE END OF EACH DAY. EACH REACH (CHANNEL, FLOODPLAIN AND BANKS) SHALL BE COMPLETED AND STABILIZED BY MATTING PRIOR TO FURTHER CONSTRUCTION. CONTRACTOR SHALL ENSURE POSITIVE DRAINAGE FOR ALL CHANNELS DAILY PRIOR TO LEAVING THE SITE. ALL AREAS WITHIN THE LIMITS OF DISTURBANCE MAY BE CLEARED; HOWEVER, MAJOR GRADING ACTIVITIES WILL BE LIMITED TO THE AREAS WITHIN THE LIMITS OF CONSTRUCTION. THE LIMITS OF DISTURBANCE INCLUDE AREAS FOR STAGING AND STOCKPILING. I. I. INITIAL SITE PREPARATION 1. STAKE AND MARK SENSITIVE AREAS WITH BOUNDARY MARKING MATERIAL TO THE LIMITS AS INDICATED ON THE CONSTRUCTION PLANS. 2. PREPARE STAGING AND STOCKPILING AREAS IN AREAS LOCATED ON THE CONSTRUCTION PLANS. 3. INSTALL TEMPORARY CONSTRUCTION ENTRANCES. 4. INSTALL AND MAINTAIN AN ONSITE RAIN GAUGE AND LOG BOOK. 5. STAKE CONSTRUCTION AND LIMITS OF CONSTRUCTION AS SHOWN ON THE CONSTRUCTION PLANS. 6. INSTALL EROSION AND SEDIMENT CONTROL DEVICES. II. FORNEY CREEK APPROXIMATE STATION 5+20 TO 32+00 PLAN SHEETS: 17, 18, 19, 20, 21, EC-2, EC-3, and EC-4 1. CHANNEL SEQUENCE a. MAINTAIN FLOW WITHIN THE EXISTING CHANNEL. MAINTAIN A PLUG AT LEAST 5' IN LENGTH SEPARATING FLOW OF THE EXISTING CHANNEL AND THE NEW CHANNEL CONSTRUCTION UNTIL FLOW IS READY TO BE TURNED INTO THE NEW CHANNEL. b. CONSTRUCT OFFLINE CHANNEL AND ADJACENT FLOODPLAIN FROM 5+20 —32+00. c. CONSTRUCT OFFLINE TRIBUTARY CHANNEL AND ADJACENT FLOODPLAIN FROM 0+12—4+36. 2. OPEN UP ONLY THAT PORTION OF THE CHANNEL THAT CAN BE COMPLETED AND STABILIZED WITHIN THE SAME DAY. 34
