In order for this office to complete its review of the referenced project in accordance with N.C.G.S. §74-50 and §74-51 of the Mining Act of 1971, please provide the additional or revised information in accordance with the following comments: 1. Please add the applicant's name to the mine map,Trent Locklear, and please add the full name of the mine as it is shown on the application,Lonnie Locklear Jr.Hwy 72 Mine,to the mine map. 2. Please refer to Section D of the mine permit application and provide a reclamation map that clearly illustrates the reclamation plan for the entire site. The map should clearly illustrate the post mining use of the entire site. It should also include all italicized items listed in the application. 3. The berm for visual screening that lies along Highway 72 is shown to be in an "undisturbed buffer." Undisturbed buffers are not permitted to have any activity whatsoever. Please clarify if this buffer is intended to be unexcavated(no excavation, but roadways, berms, ad erosion& sedimentation control measures may be installed within) or undisturbed(no activity). Please clearly label the type of buffer on the mine map. 4. Please show all Erosion and Sediment Control(E&SC)measures on the mine map including diversion ditches, silt fence, etc. 5. Please provide construction drawings and maintenance requirements for all E&SC measures and for the construction entrance. 6. Please provide additional details for the activity that will occur in the waste disposal area. Please provide proposed cross section clearly indicated the intended side slope gradient, installation of any benches and/or slope drains (with supporting design information) if needed, and the method of final stabilization. 7. Please clarify the E&SC measures that will be utilized around the waste disposal area. Please clearly identify and label on the mine map. 8. Please provide construction drawings and maintenance requirements for the construction entrance. If you are unable to meet this deadline and wish to request additional time,you must submit information,in writing,to the Director clearly indicating why the deadline cannot be met and request that an extension of time be granted. If an extension of time is not granted, a decision will be made to grant or deny the mining permit based upon the information currently in the Department's files at the end of the 180-day period. Though the preceding statement cites the maximum time limit for your response,we encourage you to provide the additional information requested by this letter as soon as possible.
