• Original and five (5) copies of the completed application
• Six (6) copies of the location maps, mine maps and reclamation maps.
• Six (6) copies of the Notification of Jurisdictional Determination (SAW-2023-00588)
The certification mail receipts as proof of notification to adjacent landowners will be sent under
separate cover.
