On December 13, 2021, the Department sent a letter to you granting a 1-year extension of time to respond to the
Department's request for supplemental information regarding the issuance of the referenced permit.
The December 13, 2021, letter contained an error in the date for a response. The letter incorrectly stated that the
requirement to respond should be received by the Department by June 7, 2022. The correct date should read
December 4, 2022. Please see the corrected statement below.
The request for an extension has been granted. Should the Department not receive a complete response to its June
7, 2021, request for additional information by December 4, 2022, The Director will make a decision on the issuance
based on the information currently on file.
