Describe in detail the sequence of events for the development and operation of the mine and reference the sequence to the mine map(s).Attach additional sheets as needed. Overburden material(dirt)and sap rock present above the hard rock will be removed. The rock is drilled and blasted to produce a product that can be transported to the plant for processing. From there, the raw material is crushed and screened to produce desired products. A construction sequence of land disturbing activities is provided on the Mine Map, specifically as it relates to sediment and erosion control. Maintenance and stabilization efforts will be conducted as noted and as required in the mining act. 2. Describe specific erosion control measures to be installed prior to land disturbing activities and during mining to prevent offsite sedimentation(include specific plans for sediment and erosion control for mine excavation(s),waste piles,access/mine roads,and process areas),and give a detailed sequence of installation and schedule for maintenance of the measures.Locate and label all sediment and erosion control measures on the mine maps)and provide typical cross-sections/construction details of each measure.Engineering designs and calculations are required to justify the adequacy of any proposed measures. Sediment and erosion control measures will be installed in accordance with the Mine Map and construction details. These measures will be installed prior to disturbing the area they are designed to protect. The construction sequence indicates the steps that will be taken to maintain all of the sediment and erosion control measures. Temporary diversions are provided to direct all surface runoff through the sediment basins. Design calculations are provided on the Mine tia.rJ Map to support the dimensions and construction of the proposed sediment basins. The majority of runoff from the property will be maintained internally, either from the pit or the existing sediment basins. 3. A)Will the operation involve washin the material mined,recycling process water,or other wastewater handling? AYES NO If yes,briefly describe all such processes including any chemicals to be used. Process wash water from material washing is treated through a recycling system within the settling cells. The system is further described in the General NPDES permit issued to the mining industry and specifically, NPDES Permit#NCG020169 for this facility. B)Will the operation involve discharging fresh or wastewater from the mine or plant as a point discharge to the waters of the State? YES -]NO if yes,briefly describe the nature of the discharge and locate all proposed discharge points(along with their method of stabilization)on the mine map(s). Water discharged as a result of mine dewatering is comprised of storm water&processing area runoff and groundwater seepage. This water will gravity flow to the pit sump for clarification prior to discharge. Water collected in the pit will be used to replenish the recycling system or replenish the existing fresh water pond on the property. Any accumulated water that is not needed,will be discharged through the approved outfall location,specifically Outfall 001 shown on the Mine Map discharges into Killians Creek. Discharge water from the outfall will be tested for pH,Turbidity,Total Suspended Solids, and Settleable Solids; in accordance with the requirements of General NPDES Permit requirements to ensure that water quality standards are met. The close proximity of the rock to the surface of the ground and the low level of permeability of the rock in the quarry area indicate that the quarry development will have no detrimental impact on any wells in the area. D)If you answered yes to any of the above questions,provide evidence that you have applied for or obtained the appropriate water quality permit(s)(i.e.,non-discharge,NPDES,Stormwater,etc.)from the Stormwater Program.In addition,the applicant is required to register water use with the Division of Water Resources,Ground Water Management Branch,if the operation withdraws more than 10,000 gallons per day and needs a capacity use permit from the Division of Water Resources,Ground Water Management Branch,if the operation lies in a capacity use area and withdraws more than 100,000 gallons per day. The process is described in the General NPDES permit issued to the mining industry and specifically, NPDES Permit No. NCG020169 for this facility. This permit authorizes the discharge of storm water, process water,and mine dewatering to the surface waters of North Carolina. 4. A)Will the operation involve crushing or any other air contaminant emissions? 1^1YES FINO If yes,indicate evidence that you have applied for or obtained an air quality permit issued by the Division of Air Quality or local governing body. The NC DEQ Division of Air Quality has issued air Permit No.05571 for this facility. B)How will dust from stockpiles,haul roads,etc.,be controlled? Fugitive dust from the processing plant, stockpiles, etc.will be controlled through the use of a wet suppression system within the operating plant. A large water truck will be used to control the dust on the haul roads, pit area and within the stockpile area. A large portion of the annual production will consist of washed stone,which generates little dust. A)A buffer will be required between any mining activity and any mining permit boundary or right-of-way.It may be an unexcavated buffer(no excavation,but roadways,berms,and erosion&sedimentation control measures may be installed within it),an undisturbed buffer(no disturbance within the buffer whatsoever),or a combination of the two, depending upon the site conditions.Note that all buffers must be located within the mining permit boundaries. How wide a buffer will be maintained between any mining activity and any mining permit boundary or right-of-way at this site?A minimum buffer of 25 feet is recommended,although a wider buffer may be needed depending on site conditions.Show all buffer locations and widths on the mine map(s). The existing, previously approved, buffer as shown on the mine map along the mining permit boundary will be provided and maintained at this site. Along with the permit boundary buffers, overburden berms are/will be provided to visually screen the operation from the adjoining neighbors. The Mine Map clearly depicts all buffers and proposed berms that will be located at the site. B)A minimum 50 foot wide undisturbed buffer will be required between any land disturbing activities within the mining permit boundaries and any natural watercourses and wetlands unless smaller undisturbed buffers can be justified. Depending on site conditions,a buffer wider than 50 feet may be needed. How wide an undisturbed buffer will be maintained between any land disturbing activities within the mining permit boundaries and any natural watercourses and wetlands at this site?Show all buffer locations and widths on the mine map(s). Existing streams within the permit boundary will have a 50' undisturbed buffer. 6. A)Describe methods to prevent landslide or slope instability adjacent to adjoining permit boundaries during mining. Minimum 2 horizontal to 1 vertical slopes or flatter for clayey material and minimum 3 horizontal to 1 vertical slopes or flatter for sandy material are generally required unless technical justification can be provided to allow steeper slopes. To assure slope stability adjacent to adjoining properties during mining, slope and excavation configurations will take into account structural geology, rock mechanics, and mining engineering considerations. Final excavation grades for all unconsolidated overburden material will be on a 2:1 minimum slope around the outer quarry perimeter from natural ground to the top of bedrock. Further,a minimum ten-foot(10')wide safety bench will be provided at the base of any cut slope and the top of bedrock in this same area. B)Provide a cross-section on the mine map(s)for all fill slopes(berms,waste piles,overburden disposal areas,etc.), clearly indicating the intended side slope gradient,installation of any benches and/or slope drains(with supporting design information)if needed,and the method of final stabilization. Revised September 21,2021 P a g e ( 8 NORTH CAROLINA MINING State of North Carolina 1612 Mail Service Center PERMIT APPLICATION Department of Environmental Quality Raleigh,NC 27699-1612 Division of Energy,Mineral,and Land Resources (919)707-9220 C)In excavation(s)of unconsolidated(non-rock)materials,specify the angle of all cut slopes including specifications for benching and sloping.Cross-sections for all cut slopes must be provided on the mine map(s). Final excavation slopes of unconsolidated materials along the outer quarry perimeter will be graded on a 2:1 minimum slope from natural ground and will extend to a minimum of six feet(6') below the anticipated water level. Internal slopes will be graded on a 1.5:1, or flatter where such areas are anticipated to be above the final water elevation. A minimum ten-foot(10')wide safety bench will be provided at the base of any cut slope around the quarry perimeter where such areas are anticipated to be above the final water elevation. D)In hardrock excavations,specify proposed bench widths and heights in feet.Provide cross-sections of the mine excavation clearly noting the angles of the cut slopes,widths of all safety benches and mine benches,and the expected maximum depth of the excavation. A typical pit cross-section is provided on the Mine Map. The first bench is largely controlled by the existing topography. Lower benches will be consistent of the typical 40'vertical heights and 10'horizontal bench widths between each level. 7. Describe other methods to be taken during mining to prevent physical hazard to any neighboring dwelling,house, public road,or public,commercial or industrial building from any mine excavation.Locate all such structures on the mine map if they are within 300 feet of any proposed excavation. All structures within the vicinity of the existing mine excavation are indicated on the Mine Map. With the exception of any proposed on-site support facilities,the closet neighboring occupied structure is located greater than three hundred (300')from the future pit excavation. Blasting activities will be designed and monitored to ensure that vibration and air blast standards are not exceeded. 8. Describe what kind of barricade will be used to prevent inadvertent public access along any high wall area and when it will be implemented.Vegetated earthen berms,appropriate fencing and adequate boulder barriers may be acceptable high wall barricades.A construction detail/cross-section and location of each type of barricade to be used must be indicated on the mine map(s). The highwall area of the quarry will be"barricaded"by the use of earthen berms,fencing, oversized boulders, or a combination of these items to prevent inadvertent public entry. A ten-foot(10')wide horizontal safety bench will also be provided between the top of the bedrock and the toe of any excavated cut slopes around the outer quarry perimeter. 9. Are M roducing minerals or soils present? A)Describe specific plans(including a schedule of implementation)for screening the operation from public view such as maintaining or planting trees,bushes or other vegetation,building berms or other measures.Show the location of all visual screening on the mine map(s)and provide cross-sections through all proposed berms or proposed spacing, sizes and species for tree plantings. Existing trees, terrain, and visual berms will effectively screen the majority of our operation from public view. Trees and vegetation will be planted as needed to promote future long-term screening and soil stability. B)Could the operation have a significantly adverse effect on the purposes of a publicly owned park,forest,or recreation area?If so,how will such effects(i.e.,noise,visibility,etc.)be mitigated? None known to be present. 11. Will[exZ sives be used? YES NO If yes,specify the types of explosive(s)and describe what precaution(s)will be used to prevent physical hazard to persons or neighboring property from flying rocks or excessive air blasts or ground vibrations.Depending on the mine's location to nearby structures,more detailed technical information may be required on the blasting program(such as a third-party blasting study).Locate the nearest offsite occupied structure(s)to the proposed excavation(s)on the mine map and indicate its approximate distance to the proposed excavation. ANFO(ammonium nitrate and fuel oil)and water-based emulsions are the primary-blasting agents used. Blasting will be conducted under the direction of experienced and qualified personnel. Blasts will be monitored at all times by seismograph to ensure that vibration and air blast levels are within the guidelines set by the State. Shot patterns will be designed in consideration of the geological environment,(i.e.,faults,fractures,and lithology)maximize material breakage,and prevent any physical hazards to persons or neighboring property. Blast designs will also be based upon the distance to the nearest dwelling from the blast site and the weight of explosives per blast interval(pounds detonated per eight(8)millisecond delay),which will consistently yield predictable blast vibration levels well within all regulatory limits at subject structures.Records of each blast will be maintained,and will include pertinent data such as the number of holes,pattern,delay intervals,depth and size of holes,type and total pounds of explosives,maximum pounds per delay interval,amount of stemming and burden for each hole,and blast location. Blast and seismograph records will be kept in a NC DEQ approved blasting guidelines format. 12. Will fuel tanks,solvents,or other chemical reagents be stored on-site? The width, height,and gradient of a proposed culvert shall be such as to pass the average historical low flow and spring flow without adversely altering flow velocity. If the width of the culvert is wider than the stream channel,the culvert shall include multiple boxes/pipes, baffles, benches and/or sills to maintain the natural width of the stream channel. Placement of culverts and other structures in streams shall be below the elevation of the streambed by one foot for all culverts with a diameter greater than 48 inches, and 20%of the culvert diameter for culverts having a diameter less than or equal to 48 inches,to allow low flow passage of water and aquatic life. If the culvert outlet is submerged within a pool or scour hole and designed to provide for aquatic passage,then culvert burial into the streambed is not required. For structures less than 72" in diameter/width, and topographic constraints indicate culvert slopes of greater than 2.5%culvert burial is not required, provided that all alternative options for flattening the slope have been investigated and aquatic life movement/connectivity has been provided when possible (e.g. rock ladders,cross-vanes, sills, baffles etc.). Notification, including supporting documentation to include a location map of the culvert,culvert profile drawings,and slope calculations,shall be provided to DWR 30 calendar days prior to the installation of the culvert. When bedrock is present in culvert locations, culvert burial is not required, provided that there is sufficient documentation of the presence of bedrock. Notification, including supporting documentation such as a location map of the culvert,geotechnical reports, photographs, etc. shall be provided to DWR a minimum of 30 calendar days prior to the installation of the culvert. If bedrock is discovered during construction,then DWR shall be notified by phone or email within 24 hours of discovery. Installation of culverts in wetlands shall ensure continuity of water movement and be designed to adequately accommodate high water or flood conditions. When roadways, causeways,or other fill projects are constructed across FEMA-designated floodways or wetlands, openings such as culverts or bridges shall be provided to maintain the natural hydrology of the system as well as prevent constriction of the floodway that may result in destabilization of streams or wetlands. The establishment of native woody vegetation and other soft stream bank stabilization techniques shall be used where practicable instead of rip-rap or other bank hardening methods. Application of fertilizer to establish planted/seeded vegetation within disturbed riparian areas and/or wetlands shall be conducted at agronomic rates and shall comply with all other Federal, State and Local regulations. Fertilizer application shall be accomplished in a manner that minimizes the risk of contact between the fertilizer and surface waters.
