Statement of Compliance and Consistency
This permit is subject to compliance with this application, site drawing and attached general and specific conditions. Any
violation of these terms may subject the permittee to a fine or criminal or civil action; and may cause the permit to become
null and void.
This permit must be on the project site and accessible to the permit officer when the project is inspected for compliance. The
applicant certifies by signing this permit that I)prior to undertaking any activities authorized by this permit,the applicant will
confer with appropriate local authorities to confirm that this project is consistent with the local land use plan and all local
ordinances, and 2) a written statement or certified mail return receipt has been obtained from the adjacent riparian
landowner(s).
The State of North Carolina and the Division of Coastal Management, in issuing this permit under the best available
information and belief,certify that this project is consistent with the North Carolina Coastal Management Program.

Attached is the CAMA paperwork for the proposed project for Mr. Douglas Warinner, Jr. Please note that
it is in two (2) separate attachments. Since talking to you this morning I have realized that I was looking
at some of these dates wrong and that they actually are not expired yet. However, some of them are so I
only have two customers that I can send you paperwork for right now. The second set will be in a
separate email.
I also know that you requested that I use the same riparian form for all of our projects, however, the
customer obtained these riparians himself and used something he found online I guess. I sincerely hope
these will work. If not, let me know and I'll get them redone.

DESCRIPTION
We propose to install a new wooden 4' wide x 16' long and 4' wide x 40' long
finger piers on the right side of existing dock. This will also include removing an 8'
section of walkway and replacing with an 8' wide x 10' long platform extension.
We propose to install a new boat lift with six (6) new pilings.
