Pamlico County
In accordance with your application received March 1, 1999, we are forwarding herewith Permit No.
WQ0016541, dated August 30, 1999, to Bay River Metropolitan Sewerage District for the construction and
operation of the subject wastewater collection extension.
This permit shall be effective from the date of issuance until rescinded, and shall be subject to the
conditions and limitations as specified therein,
Please note Condition No. 7., which details the certification and as -built preparation requirements for this
project. In addition, pay particular attention to Condition No. 11. h., which establishes the flushing program for the
pressure sewers permitted herein. The flushing program is to be continued until such time that enough service
connections are made to the pressure sewers to maintain wastewater velocities equal to or greater than two feet per
second. Finally, Condition No. 12 states that the existing six-inch pressure sewer immediately downstream of
Booster Pump Station No. 6 must be upgraded when wastewater flows made tributary to the pressure sewer
extensions permitted herein cause the hydraulic capacity of the downstream pressure sewer to be exceeded.
If any parts, requirements, or limitations contained in this permit are unacceptable, you.have the right to
request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request
must be in the form of a written petition, conforming to Chapter 150D of North Carolina General Statutes, and filed
with the Office of Administrative Hearings, 6700 Mail Service Center, Raleigh, NC 27699-6700. Unless such
demands are made this permit shall be final and binding.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and
other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO
Bay River Metropolitan Sewerage District
Pamlico County
FOR THE
construction and operation of a pressure sewer collection system consisting of approximately 2,425 linear feet of
six-inch pressure sewer and 22 1,340-gallon combination septic tank/effluent pump stations (STEP systems), each
equipped with a 15-GPM simplex pump and an on -site audible and visual high waxer alarm to serve 22 three -
bedroom single-family residences along North Carolina State Road 306. This permit also authorizes the
construction and operation of approximately 6,950 linear feet of eight -inch pressure sewer along North Carolina
Highway 55 that will improve operation of Bay River Metropolitan Sewerage District's existing pressure sewer
collection system as well as provide additional capacity for future growth and development. Permission for this
project grants the discharge of 7,920 GPD of collected domestic wastewater into Bay River Metropolitan Sewerage
District's existing sewerage system, pursuant to the application received March 1, 1999, and in conformity with the
project plans, specifications, and other supporting data subsequently filed and approved by the Department of
Environment and Natural Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until rescinded and shall be subject to the following
specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of
this permit the approved plans and specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in the application and
other supporting data.
3. The facilities must be properly maintained and operated at all times.
4. The sewage and wastewater collected by this system shall be treated in the Bay River Metropolitan Sewerage
District's Wastewater Treatment Plant (NPDES Permit No. NC0066109) prior to being discharged into the
receiving stream.

5. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there -is
a name change of the Permittee, a formal permit request must be submitted to the Division of Water Quality
(Division) accompanied by an application fee, documentation from the parties involved, and other supporting
materials as may be appropriate. The approval of this request will be considered on its merits and may or
may not be approved.
6. . Construction of the pressure sewers and STEP systems shall be scheduled so as not to interrupt service by the
existing utilities nor result in an overflow or bypass discharge of wastewater to the surface waters of the
State.
Upon completion of construction and prior to operation of this permitted facility, a certification and the as -
built drawings must be received from a professional engineer certifying that the permitted facility has been
installed in accordance with this permit, the approved plans and specifications, and other supporting
materials. If this project is to be completed in phases and partially certified, you shall retain the responsibility
to track further construction approved under the same permit, and shall provide a final certificate of
completion once the entire project has been completed. Both the six-inch and the eight -inch pressure sewers
shall be indicated as a high -priority lines on the as -built drawings. Mail the Certification and one copy of the
"Record Drawings" to the Non -Discharge Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699-
1617.
3. A copy of the approved plans and specifications shall be maintained on file by the Permittee for a minimum
of five years from the date of the completion of construction.
9. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to an
enforcement action by the Division, in accordance with North Carolina General Statute 143-215.6A to 143-
215.6C.
10. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules,
regulations, or ordinances which may be imposed by other government agencies (local, state, and federal)
which have jurisdiction.
11. The Permittee shall provide for the STEP systems and pressure sewers the following items:
a. At least 24 hours of storage capacity above the .high water alarm activation level for any simplex pump
station serving a single building.
b. An air relief valve located at all high points along the pressure sewers.
c. A screened vent for the pump chambers.
d. Fillets located in the pump chambers at the intersection of the flooring and sidewalk.
e_ Three feet of cover (minimum) over the pressure sewers or the use of ferrous material where three feet
cannot be maintained.
f. Sufficient devices which will protect the STEP systems from vandals.
g. Flood protection if the STEP systems are located below the 100-year flood elevation.

h, Sufficient clean -out devices along and at the end of the pressure sewers to allow regular flushing of the
pressure sewers as required, but not less than once per calendar year, to remove solids that may deposit as
a result of wastewater velocities less than two feet per second, The Permittee shall maintain records of
each flushing event as well as line pressures at the time of flushing. The Permittee shall also maintain a
record of all STEP system high-water alarm occurrences in between flushing events and use this data for
predictive maintenance purposes. The flushing program shall be continued until such time that enough
connections are made to the pressure sewers to maintain wastewater velocities of equal to or greater than
two feet per second. The Permittee shall notify the Washington Regional Office Water Quality
Supervisor in letterform of its intent to cease the flushing program and remove the "high -priority line"
status from the permitted pressure sewers,
12. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions and
excessive STEP system high-water alarms, the Permittee shall take immediate corrective action, including
those that may be required by this Division, such as the construction of additional or replacement wastewater
collection facilities. Note that the existing six-inch pressure sewer downstream of Booster Pump Station No.
6 shall be up -graded when wastewater flows made tributary- to the pressure sewer extensions permitted herein
cause the downstream pressure sewer to exceed its hydraulic capacity,
13, Noncompliance Notification:
The Permittee shall report by telephone to the Washington Regional Office, telephone number (252) 946-
6481, as soon as possible, but in no case more than 24 hours or on the next working day following the
occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that render the facility incapable of adequate
wastewater transport such as mechanical or electrical failures of pumps, line blockage or breakage, etc.
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving waters without
. treatment of all or any portion of the influent to such station or facility.
Persons reporting such occurrences by telephone shall also file a written report in letterform within five days
following first knowledge of the occurrence. This report must outline the actions taken or proposed to be
taken to ensure that the problem does not recur.
14. The Permittee shall maintain on hand for immediate installation a supply of spare, fully operational pump
units of each type used in the pressure sewer system. The number of pumps on hand shall not be less than 10
percent of, the number of installed units or one unit, whichever is greater.
I5, Each pump station shall be clearly and conspicuously posted with the telephone number of the
owner/operator of the pressure sewer system and instructions to call the number in the event of high water
alarm activation_
