drawing and attached general and specific conditions. Any !
violation of these terms may subject the permittee to a fine,
imprisonment or civil action; and may cause the permit to be-
come null and void.
This permit must be on the project site and accessible to the
permit officer when the project is inspected for compliance.
The applicant certifies by signing this permit that 1) this pro-
ject is consistent with the local land use plan and all local
ordinances, and 2) a written statement has been obtained from
adjacent riparian landowners certifying that they have no
objections to the proposed work.

EIPARIA14 LAi1D0W- ER' S STATEMENT
General Statute 5113-229 requires that the riparian
landowners adjacent to a proposed project be given thirty (3.0)
days to object to the project. If an applicant wishes to obtain
a permit in a shorter time, it will be necessary for the
applicant to obtain a signed statement from each of the adjacent
riparian landowner stating that the landowner has no objections
to the project.
This form may be used by the adjacent riparian landowner as
a consent form:

submitted by Joyce E. Hussey dated February 9,1999, wherein
said applicant request a permit to dredge and/or fill on the land
or in the water adjacent to my property.
