violation of these terms may subject the permittee to a fine,
imprisonment or civil action; and may cause the permit to be-
come null and void.
This permit must be on the project site and accessible to the
permit officer when the project is inspected for compliance.
The applicant certifies by signing this permit that 1) this pro-
ject is consistent with the local land use plan and all local
ordinances, and 2) a written statement has been obtained from
adjacent riparian landowners certifying that they have no
objections to the proposed work.

SUBJECT: CAMA Permit
Well 6, Raw Water Line, and
Knollwood Banks Water Improvements
Town of Atlantic Beach
Dear Mr. Jones:
The following items and information are provided in support of subject request for a
CAMA Permit:
1. Application fee in amount of $50.00.
2. Location Map which illustrates location of proposed construction activities.
The proposed improvements include the installation of approximately 28,000 LF of
potable water lines to serve the Knollwood Banks annexation area, approximately 13,000 LF
of raw water line between the elevated tank on East Drive and proposed Well 6, and the
construction of Well 6 at Fort Macon State Park. Water lines will typically be located
approximately 3 feet to 10 feet from the edge of existing pavement and will be buried
approximately 3 feet in depth. Less than 100 LF of wetlands/marsh is anticipated to be
disturbed during construction. Any construction in wetlands/marsh will not re -contour
elevations. A DOT Encroachment Agreement will be required to accomplish the project. The
Town will execute DOT Encroachment Agreements prior to beginning construction.
