This permit is subject to compliance with this application, site drawing
and attached general and specific conditions. Any violation of these terms
may subject the permittee to a fine, imprisonment or civil action; and
may cause the permit to become null and void.
This permit must be on the project site and accessible to the permit of-
ficer when the project is inspected for compliance. The applicant certi-
fies by signing this permit that 1) this project is consistent with the local
land use plan and all local ordinances, and 2) a written statement has
been obtained from adjacent riparian landowners certifying that they
have no objections to the proposed work.
In issuing this permit the State of North Carolina certifies that this project
is consistent with the North Carolina Coastal Management Program.

Dear Mike,
Today you and I met to discuss a permit for Scott Dixon at 225 Thurman Rd. within
Fisher Point Subdivision. I failed to affix a sticker to the permit indicating that the project is
within the Neuse River Basin therefore subject to the Neuse River Buffer Rules implemented by
the Division of Water Quality (DWQ). The activity is considered allowable under their rules but
still requires notice to DWQ.
Please attach this correspondence to the permit issued today, thereby noticing Mr. Dixon
that his property is subject to the Neuse River Buffer Rules. If you or Mr. Dixon have any
questions please don't hesitate to call me at 252-808-2808.

Dear Brad,
Here are the riparian forms with both neighbors sign off. It is mostly a repair job that is under
50%, but he wants to add a 10' x 12' gazebo on his existing 16' x 20' L-head and wants to add
another slip (total of two) to the right side, and cut the left side slip down to a smaller size and
remove a 3' x 32' catwalk.
Since you are on leave I wanted to drop this off to your office. We are ready to start the leveling
and repairs, and will hold off on anZhing new until we hear from you. The right side neighbor Joe
Smith signed a waiver for the new right hand slip as it will be in the setback near the property line.
