PERMIT
as authorized by the State of North Carolina, Department of Environmental Quality and the
Coastal Resources Commission for development in an Area of Environmental Concern
pursuant to the Coastal Area Management Act, Section 113A-118 of the General Statutes.
Issued to John & Donna Moore, authorizing development in the Estuarine Shoreline (AEC) at 138 Discovery Trail In
Holiday Island Subdivision, in Hertford, as requested in the permittee's application, dated July 17, 2024, received on July
17, 2024 and received complete on August 19, 2024. This permit, issued on August 19, 2024, is subject to compliance
with the application and site drawing (where consistent with the permit), all applicable regulations and special conditions
and notes set forth below. Any violation of these terms may subject permittee to a fine, imprisonment or civil action, or
may cause the permit to be null and void.
This permit authorizes: Construction of Single Family Residence.
(1) All proposed development and associated construction must be done in accordance with the permitted work plat
drawings(s) dated received on August 16, 2023. [GS 113A-120]
(2) All construction must conform to the N.C. Building Code requirements and all other local, State and Federal regulations,
applicable local ordinances and FEMA Flood Regulations. [GS 113A-120]
(3) Any change or changes in the plans for development, construction, or land use activities will require a re-evaluation and
modification of this permit. [GS 113A-120]
(4) A copy of this permit shall be posted or available on site. Contact this office at 252-264-3901 for a final inspection at
completion of work. [GS 113A-120)

This permit action may be appealed by the permittee or other qualified persons
within twenty (20) days of the issuing date. This permit must be on the project
site and accessible to the permit officer when the project is inspected for
compliance. Any maintenance work or project modification not covered under
this permit, requirefurther written permit approval. All work must cease when this
permit expires on: DECEMBER31,2027
In issuing this permit it is agreed that this project is consistent with the local Land
Use Plan and all applicable ordinances. This permit may not be transferred to
another party without the written approval of the Division of Coastal
Management.
Check #4881. in the amount of $100, was received on 8/19/2024 for the
permit fee.

DONNA L. MOORE and husband, JOHNNY R. MOORE, do hereby certify and declare
as follows:
Donna L. Moore and husband, Johnny R. Moore, are the owners of the certain adjacent
parcels of real property located in Perquimans County, North Carolina, and more particularly
described as follows:
Lots 62 and 63, Section J, as shown on map or plat entitled "HOLIDAY
ISLAND, SECT. "J"", dated May 27, 1971, prepared by S. Elmo Williams,
Registered Surveyor, recorded Plat Book 4, Pages 261 and 263 of the
Perquimans County Public Registry.
Said property being described in those certain deeds recorded in Book 525,
Page 467, and Book 525, Page 469, Perquimans County Public Registry. Lot
62 is presently identified as Parcel Number 2-D082-J062-HI, and Lot 63 is
presently identified as Parcel Number 2-DO82-JO63-HI in the Office of the
Perquimans County Tax Assessor.
1. It is the desire of the Affiants that the Perquimans County Tax Assessor combine
all of Parcel Number 2-DO82-JO62-HI and Parcel Number 2-DO82-JO63-HI into one (1) unitary
parcel.
2. The Affiants intend to use the previously described property as a single parcel in
regards to any future convenances.
3. It is the intent of the Affiants/Owners that the property described hereinabove will
hereafter be listed, assessed and taxed as one real property tax parcel number for ad valorem taxes
in the above County in which the real property is listed.
