This permit, issued on necemher 10,2014 , is subject to compliance with the application (where consistent
with the permit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may
be subject to fines, imprisonment or civil action; or may cause the permit to be null and void.
1) Unless specifically altered herein,this minor modification authorizes the construction of the 6' X 18'
floating dock and gangway, as well as the associated uses located in or over the water that are
expressly and specifically set forth in the permit drawing. No other structure, whether floating or
d stationary, shall become a permanent part of this docking facility without permit modification. No
non-water dependent uses of structures may be conducted on, in or over public trust waters without
permit modification.
2) This minor modification authorizes 2 additional boat slips for a total of 7 boat slips associated with
this docking facility.

3) This minor modification shall be attached to the original of Permit No. 86-05, which was issued on
` il'/05, the major modification issued on 5/14/08, and copies of all documents shall be readily available
life when Division personnel inspect the project for compliance.
4) All conditions and stipulations of the active permit remain in force under this minor modification unless
specifically altered herein.
NOTE: This permit does not eliminate the need to obtain any additional state, federal or local permits,
approvals or authorizations that may be required.
NOTE: The U.S. Army Corps of Engineers assigned the proposed project COE Action Id. No. SAW-
2008-00407.

The enclosed permit constitutes authorization under the Coastal Area Management Act,and where
applicable,the State Dredge and Fill Law,for you to proceed with your project proposal.The original(buff-colored
form) is retained by you and it must be available on site when the project is inspected for compliance.Please sign
both the original and the copy and return the copy to this office in the enclosed envelope. Signing the permit and
proceeding means you have waived your right of appeal described below.
If you object to the permit or any of the conditions,you may request a hearing pursuant to NCGS 113A-
121.1 or 113-229.Your petition for a hearing must be filed in accordance with NCGS Chapter 150B with the Office
of Administrative Hearings,6714 Mail Service Center,Raleigh,NC 27611-6714,(919)733-2698 within twenty
(20)days of this decision on your permit.You should also be aware that another qualified party may submit an
objection to the issuance of this permit within twenty(20)days.
The project plan is subject to those conditions appearing on the permit form. Otherwise,all work must be
carried out in accordance with your application.Modifications,time extensions, and future maintenance requires
additional approval.Please read your permit carefully prior to starting work and review all project plans,as
approved.If you are having the work done by a contractor, it would be to your benefit to be sure that he fully
understands all permit requirements.
From time to time,Department personnel will visit the project site.To facilitate this review,we request that
you complete and mail the enclosed Notice Card just prior to work initiation.However, if questions arise
concerning permit conditions,environmental safeguards,or problem areas,you may contact Department personnel
at any time for assistance.By working in accordance with the permit,you will be helping to protect our vitally
important coastal resources.

Mr. Edward"Butch"Parrish, representing the permittee, has submitted a request to modify CAMA Major Permit
No. 86-05, which was issued on June 1, 2005 and later modified on March 14, 2008 for the construction of a commercial
docking facility at 720 Carolina Blvd., Topsail Beach,NC. The current permit was scheduled to expire on December 31,
2011; however, State Permit No. 86-05 was subject to extension by the Session Law 2009-406, and as amended by
Session Law 2010-177,the Permit Extension Act.The expiration date based on the extension act is October 18,2015.
The permittee is requesting authorization to allow for the installation of a new 6' x 18' section of floating dock
along the central portion of the properties shoreline. As proposed,the floating dock would be located between the existing
bulkhead and the existing 7' x 125' long floating dock that extends toward Banks Channel from the western property line,
and runs parallel to the shoreline. The applicant has indicated this dock would be used to moor two vessels. The proposed
structure would connect to the 6' wide access walkway and span toward the south. A 4' x 11' ramp would connect the
proposed dock to the existing walkway.
Considering the proposed floating dock would be located within the footprint of the existing "T-Head" docking
facility, no navigational concerns are anticipated. Additionally, current water depths(-3.0 to -4.0' @ NLW) in the area of
the proposed dock are sufficient to support the development activities. The proposed development would be located
within the permittee's area of riparian access, and outside of the 15' riparian corridor setback. The permittee has obtained
letters of no objection from the adjacent riparian property owners.
The permittee has submitted a check(#804) in the amount of$100.00 for the modification.

Dear Mr. Dail:
The Historical Society of Topsail Island requests a modification to our existing major permit
#86-05 (cover page enclosed).
We wish to add a floating dock on the southwest side of our existing pier, as shown on the
enclosed drawing. We expect to dock two additional boats in the 20-35 foot class as a result of
the addition.
We have obtained approval from our adjoining neighbors, Kim Quinn and Al Wallace. Their
approval letters are also enclosed, along with a check in the amount of$100 for the permit
change.

Looking through the request that we received for the Historical Society of Topsail Island, it appears that this project will
qualify for use of a RGP#56. We have no additional comments on the usage of the permit.
